Incident Narrative
Employee was installing a TIW valve assembly for Blow Out Preventive (BOP) testing to get a pressure reading from the pipe. During the BOP process, the TIW assembly was sitting on top of the pipe when the pipe fell and struck the employee in the left foot causing lacerations. Employee was Medevac'd to hospital to receive sutures in left foot and was kept overnight for observation.
