Incident Narrative

An employee was delivering a cylinder of gas to a customer. The cylinder was being moved with a cylinder cart. As the employee was walking the cart and cylinder to the customer's door, he slipped in the snow. His hand got caught between the cylinder and the concrete parking lot. The employee crushed the tips of his index and middle finger on his right hand. In-patient hospitalization was required for surgery on the fingers.
