Incident Narrative
Employee was taking papers out to a truck driver. She left the office through the south office door. Walking to the truck, she slipped on ice in the parking lot and fell. She landed on her left hip, striking the ground. She injured her left hip and possibly her back. She was transported to Mercy Hospital in Springfield, Missouri, where she was admitted.
