Incident Narrative

The employee was driving the powered industrial truck, stacking product on the top of the third pallet. The employee stacked the pallet and put the forklift in reverse. The employee then placed the forklift in forward, assuming that the forks were still elevated. The forklift fell on its side, causing the employee to break an ankle.
