Incident Narrative

An employee was assisting in the positioning of a table that is used to guide steel forms. A crane and signalman were used to pick up and position the table. The employee went under the table to ensure that it was going to set level and the table was set down on the employee's left middle finger. Later that day the tip of the finger was removed by a doctor.
