Incident Narrative
On 1/31/15, an employee amputated the tip of the left middle finger in a Garrido paper sheeting machine while removing a paper jam. The employee's finger was caught between a metal pusher attached to a belt and the machine itself, removing about 1/2" of the employees left middle finger.
