Incident Narrative
On January 28, 2015, an employee was using an 8-foot A-frame ladder to change out pipe fitting at the Hydro Cracker Unit. The employee fell from the ladder while descending, breaking his lower right leg in the ankle area. Employee was transported by ambulance to Pauls Valley, then transferred to the Norman Regional Hospital where he had surgery to set the bone.
