Incident Narrative

Employee was operating a drill tap machine. As he was making an adjustment for a new job on the tooling, his hand contacted the rotating spindle shaft. His glove got caught on the shaft, and his hand was entangled. This resulted in amputation to the first knuckle of his left-hand middle finger.
