Incident Narrative

On January 13, 2015, at approximately 3:00 PM, Employee #1 was using a vacuum hose inside a tank when the vacuum caught his hand and pulled his arm in. The vacuum was caught on Employee #1's arm for approximately 15 seconds before he alerted Employee #2, the attendant operating the equipment, who turned the vacuum off. Approximately one hour later, Employee #1's arm began to swell. He was taken to the hospital, and the swelling continued. Incisions were made into the arm to relieve pressure, and the employee was admitted overnight.
