Incident Narrative

On Saturday, 01/10/2015, at approximately 2:15 p.m., an employee was changing out a large piston when a failure occurred, crushing/smashing three of the employee's fingers on his left hand. The employee was hospitalized and underwent surgery on 01/11/2015.
