Incident Narrative
On or about 1/4/15 at 6:05 pm, an employee was preparing to clean a room and put a door stopper at the room door. The stopper slipped catching her hand between the door and its frame resulting in the amputation of her left index finger. Employee was taken to the hospital to reattach the finger.
