Incident Narrative
AT APPROXIMATELY 1238 HOURS ON SEPTEMBER 30, 2014, PG&E WAS NOTIFIED OF A THIRD-PARTY DIG-IN IN SAN JOSE (NEAR THE INTERSECTION OF RIDDER PARK DRIVE AND SCHALLENBERGER ROAD). THE PG&E GAS SERVICE REPRESENTATIVE (GSR) ARRIVED ON SCENE AT APPROXIMATELY 1256 HOURS. THE THIRD-PARTY, J.J. ALBANESE, STRUCK A 4-INCH STEEL DISTRIBUTION MAIN WITH A BACKHOE, CAUSING AN UNINTENTIONAL RELEASE OF NATURAL GAS. THE EXCAVATOR HAD CALLED USA (TICKET NO. 388378), BUT FAILED TO DELINEATE THE AREA WHERE THE DIG-IN TOOK PLACE, THUS THE GAS FACILITIES IN THE AREA WHERE THE DIG-IN TOOK PLACE COULD NOT BE LOCATED AND MARKED. PG&E REPAIR CREW ARRIVED AT 1312 HOURS. GAS WAS SHUT-IN AT 1652 HOURS BY EXCAVATING AND SQUEEZING THE STEEL DISTRIBUTION MAIN (NORTH OF THE DIG-IN). REPAIRS WERE MADE BY APPROXIMATELY 2000 HOURS ON OCTOBER 1, 2014. AT APPROXIMATELY 0800 HOURS ON OCTOBER 2, 2014, PG&E BEGAN RESTORING GAS SERVICE FOR CUSTOMERS. AS OF APPROXIMATELY 1600 HOURS ON OCTOBER 2, 34 OF THE 35 SERVICES HAD GAS SERVICE RESTORED (ONE SERVICE FEEDS A MASTER METER FOR A MOBILE HOME PARK). A TOTAL OF 216 MOBILE HOME UNITS LOST GAS SERVICE FROM THE DIG-IN INCIDENT. RELIGHTING EFFORTS FOR THE MOBILE HOME PARK BEGAN OCT. 2, 2014 AT 1900 HOURS AND FINISHED AT APPROXIMATELY 0030 HOURS (OCT. 3, 2014). PG&E RELIT 189 MOBILE HOME UNITS. PG&E WAS UNABLE TO ACCESS 32 UNITS DURING THE RELIGHT EFFORT. AS OF OCT. 3, 2014 AFTERNOON, THE MOBILE HOME PARK CONFIRMED THEY FINISHED RELIGHT FOR THE REMAINING 27 UNITS AND CONFIRMED THAT 5 UNITS WERE VACANT. THERE WAS NO IGNITION, NO EXPLOSIONS, NO INJURIES, AND NO FATALITIES DURING THE INCIDENT. NO MEDIA WAS OBSERVED ON SCENE. THIS INCIDENT WAS REPORTED ONLY TO THE CPUC BASED ON OPERATOR JUDGMENT AS OVER 200 CUSTOMERS LOST GAS SERVICE AS A RESULT OF THE DIG-IN. THIS INCIDENT WAS REPORTED TO THE DOT ON OCTOBER 1, 2014 BECAUSE THE COST OF DAMAGES WAS EXPECTED TO EXCEED $50,000.
