"EMPLOYEE WAS DOING A STATION CHECK WHEN NOTICED A FROST BALL ON PUMP #2. ON FURTHER INVESTIGATION IT WAS FOUND TO HAVE A HAIRLINE CRACK IN THE WELD ON THE PUMP. PUMP WAS ISOLATED, BLOW DOWN, TAKEN OUT OF SERVICE AND SCHEDULED FOR REPAIR."
