Incident Narrative
ON FEBRUARY 10, 2022, AT 11:00 AM THE LANDOWNER ADJACENT TO BROTHERS STATION ADVISED THE ONSITE TRUCKING SUPERVISOR THAT THERE WAS SHEEN OF OIL ON THE PROPERTY. WHEN THE TRUCKING SUPERVISOR WAS NOTIFIED, HE THEN PROCEEDED TO NOTIFY THE MAINTENANCE SUPERVISOR AND THE MANAGER PIPELINE INTEGRITY. THE MAINTENANCE SUPERVISOR IMMEDIATELY NOTIFIED THE CONTROL ROOM TO SHUT DOWN THE SYSTEM. THE MANAGER PIPELINE INTEGRITY PROCEEDED TO NOTIFY VP OF PIPELINES AND HAVE OTHER CVR EMPLOYEES RESPOND TO THE SCENE. CVR RESPONSE CREWS AND ACME ENVIRONMENTAL WERE MOBILIZED DURING THE INITIAL CALL OUT FOR THE ACCIDENT. THIS ACCIDENT UTILIZED VAC TRUCKS, EXCAVATORS, DUMP TRUCKS, ROLL OFF CONTAINERS AND EXISTING TANKAGE ONSITE. SPOILS WERE TAKEN TO LANDFILL SITES CAPABLE OF RECEIVING OILED SOIL. THIS SECTION OF THE LINE HAS BEEN DISCONNECTED AND CAPPED ON BOTH ENDS OF THE LINE. ROOT CAUSE INVESTIGATION: IN MARCH 2020, CRCT PUT A MEMO TO FILE DOCUMENTING THE PURGING AND IDLING THE VALLEY 8' PIPELINE. THIS MEMO STATED: 'COFFEYVILLE RESOURCES CRUDE TRANSPORTATION (CRCT) HAS ELECTED TO IDLE THE SEGMENT OF PIPE FROM THE 6-8 TRAP TO THE BROTHERS STATION TIE-IN. IN ACCORDANCE WITH ADB-2016-05, CRCT WILL CONTINUE TO CONDUCT ROUTINE INSPECTION AND TESTS ON THIS SECTION OF PIPE, WHICH INCLUDE BUT ARE NOT LIMITED TO: ' VALVE INSPECTIONS ' ROW INSPECTIONS ' APPLICABLE FACILITY INSPECTIONS ' ONE CALL RESPONSE ' PUBLIC AWARENESS MAILINGS CRCT WILL DEFER INTEGRITY ASSESSMENTS ON THIS SECTION IN ACCORDANCE WITH THE AFOREMENTIONED ADVISORY BULLETIN SINCE THIS SEGMENT HAS BEEN PURGED USING THE FOLLOWING PROCESS: CRCT LAUNCHED A CLEANING PIG FOLLOWED BY APPROXIMATELY 50 BBLS OF WATER FOLLOWED BY AN ADDITIONAL CLEANING PIG. THIS SLUG ASSEMBLY WAS PUSHED WITH NITROGEN. THE LINE REMAINS PURGED WITH NITROGEN AND HAS BEEN BLIND FLANGED ON BOTH SIDES AND ALL SOURCES OF PRODUCT HAVE BEEN DISCONNECTED.' UPON INVESTIGATING THE ACCIDENT, CRCT IDENTIFIED DIRECT AND INDIRECT CAUSES RESULTING IN THE RELEASE. THE PRELIMINARY DIRECT CAUSE OF THE LEAK WAS IDENTIFIED TO BE INTERNAL CORROSION ISSUES ON THE PIPELINE THAT LED TO METAL LOSS RESULTING IN TWO (2) PIN HOLE LEAKS IN THE 6 O'CLOCK POSITION OF THE PIPE. CRCT PRELIMINARY FINDINGS SUGGEST THAT PRODUCT SAT IDLE IN THE DEAD LEG INCREASING THE RATE IN WHICH INTERNAL CORROSION OCCURRED. THE INDIRECT CAUSE FOR THIS ACCIDENT WAS A RESULT OF NOT PROPERLY ISOLATING THIS SEGMENT FROM THE BROTHERS TO HOOSER PIPELINE. ADDITIONALLY, CRCT REVIEWED THE CAPABILITIES OF THE LEAK DETECTION SOFTWARE AND IDENTIFIED THAT THE LEAK DETECTION PARAMETERS CANNOT BE CALIBRATED TO DETECT LEAKS ON THE SYSTEM WITH SUCH SMALL INCREMENTAL LOSSES. THEREFORE, CRCT IS INVESTIGATING ADDITIONAL LEAK DETECTION POSSIBILITIES SUCH AS INSTALLING BACK PRESSURE VALVES PROVIDING THE LINES WITH POSITIVE PRESSURE WHEN NOT IN USE. THIS WILL ALLOW CRCT TO IDENTIFY IF THERE IS A LOSS OF PRESSURE (STEADY OR SUDDEN) WHILE PRODUCT IS NOT BEING TRANSFERRED. ADDITIONALLY, CRCT IS CONDUCTING A REVIEW OF ALL OTHER SEGMENTS TO ENSURE NO OTHER SYSTEM HAS THIS SAME ISSUE.
