ON JANUARY 28, 2022, AT 7:30AM, A CENTURION PIPELINE, L.P. (CPL) CONTROLLER NOTIFIED A CPL STATION OPERATOR(EMPLOYEE) OF A POTENTIAL LEAK AT CPL'S HALLEY STATION NEAR WINK, TEXAS IN WINKLER COUNTY, TEXAS. CPL'S EMPLOYEE ARRIVED ON SITE AT 8:45 AND CONFIRMED A LEAK AT HALLEY STATION. CPL ISOLATED THE LEAK AND DETERMINED THE SOURCE TO BE ON A HALLEY STATION'S 10'' RELIEF LINE. CPL REPORTED THE LEAK TO THE NATIONAL RESPONSE CENTER (NRC) AT 09:17AM AND TEXAS RAILROAD COMMISSION(TRRC) AT 09:15AM. NRC REPORT NUMBER IS 1327489 AND TRRC INCIDENT NUMBER IS 2563. A SUBSEQUENT NRC NOTIFICATION WAS MADE ON JANUARY 29, 2022 AT 4:54PM WITH NRC REPORT NUMBER 1327582. SEVEN HUNDRED AND THIRTY (730) BARRELS WERE RELEASED AND SIX HUNDRED AND EIGHTY (680) BARRELS WERE RECOVERED. NO INJURIES, FATALITIES OR FIRES WERE ASSOCIATED WITH THIS INCIDENT. REMEDIATION IS ONGOING PENDING SAMPLE RESULTS. REPAIRS ARE COMPLETE AND THE ROOT CAUSE FAILURE ANALYSIS WILL BE CONDUCTED PENDING THE SAMPLE EXAMINATIONS. THE RESULTING INFORMATION RELATED TO THE CAUSE OF THE LEAK, PREVENTATIVE AND MITIGATIVE MEASURES (P&M), AND UPDATE TO COSTS, IF APPLICABLE WILL BE SUBMITTED VIA SUPPLEMENTAL REPORTS. SUPPLEMENTAL REPORT FILED ON MARCH 25, 2022: PART E1 HAS BEEN UPDATED FROM '0' TO '7.8.' REMEDIATION HAS BEEN COMPLETED AS OF THE FILING OF THIS SUPPLEMENTAL REPORT. SAMPLE RESULTS HAVE BEEN COMPLETED AND THE ROOT CAUSE FAILURE ANALYSIS IS STILL PENDING. ONCE THE ANALYSIS HAS BEEN COMPLETED, A SUPPLEMENTAL FINAL REPORT WILL BE FILED WITH THE ASSOCIATED PREVENTATIVE AND MITIGATIVE MEASURES. SUPPLEMENTAL FINAL REPORT FILED ON MAY 12, 2022: DURING THE ROOT CAUSE ANALYSIS (RCA), THE RCA TEAM DETERMINED THAT NO FLOW WITHIN THE RELIEF LINE LED TO MICROBIOLOGICALLY INFLUENCED CORROSION (MIC) WHICH RESULTED IN AN INTERNAL CORROSION FAILURE OF THE HALLEY 10'' RELIEF LINE. AS PART OF PREVENTATIVE AND MITIGATIVE (P&M) MEASURES, A REVIEW AND REVISION OF THE DEAD-LEG GUIDELINE TO DETAIL SPECIFIC REQUIREMENTS FOR PMS IN RELATION TO DEAD-LEGS FOR THE PREVENTION OF MIC AND REQUIRES A PERIODIC REVIEW OF THE DEAD-LEG GUIDELINE. CREATE A DEAD-LEG REGISTRY THAT INCLUDES LINES THAT HAVE BEEN IDENTIFIED WHEREIN PMS ARE IN PLACE AS PART OF THE DEAD-LEG PROGRAM AND CONDUCT A RECURRING ASSESSMENT ON THE DEAD LEG REGISTRY. AFTER THE DEAD-LEG REGISTRY IS DEVELOPED, EVALUATE THE RISK ASSOCIATED WITH EACH DEAD-LEG AND PRIORITIZE THE MITIGATION EFFORTS. DEVELOP AND IMPLEMENT A TRAINING PROGRAM THAT DETAILS THE DEAD-LEG MANAGEMENT PROGRAM, THE ROLES AND RESPONSIBILITIES OF PERSONNEL, AND INCLUDES A REFRESHER MECHANISM. SUPPLEMENTAL FINAL REPORT FILED ON NOVEMBER 2, 2022: PART A- 21B. WAS UPDATED TO REFLECT THE CORRECT NRC NUMBER '1327489' INSTEAD OF '1327589' AS PREVIOUSLY REPORTED.
