ON THE AFTERNOON OF 4/12/2022, WHILE MAKING A CHANGE OF PRODUCT FROM GASOLINE TO DIESEL ON THE 13201 CEDAR POINT TO FOSTORIA (TR-10) PIPELINE, THE PIPELINE CONTROLLER EXPERIENCED A PUMP/PRESSURE IMBALANCE BELIEVED TO BE DUE TO THE CHANGE OF PRODUCT. UPON FURTHER REVIEW, A LEAK DETECTION SYSTEM ALERT ALSO REGISTERED. THE PIPELINE CONTROLLER TOOK ACTIONS TO SHUT DOWN THE PIPELINE AND NOTIFIED LOCAL EMERGENCY RESPONDERS. INITIAL ISOLATION OF THE SEGMENT WAS MADE BY CONTROL CENTER CLOSING REMOTELY CONTROLLED VALVES AT CEDAR POINT AND FOSTORIA IMMEDIATELY FOLLOWING SHUTDOWN. NOTIFICATIONS WERE MADE TO INTERNAL CONTROL CENTER AND FIELD OPERATIONS PERSONNEL. FIELD PERSONNEL FURTHER ISOLATED THE SEGMENT BY CLOSING UPSTREAM AND DOWNSTREAM MANUAL VALVES AT COLLIN AND RECKER ROADS AT 15:45 AND 16:18 RESPECTIVELY. CONFIRMED FIELD DISCOVERY OF A GASOLINE RELEASE WAS VERIFIED BY OPERATIONS PERSONNEL AT 17:10 EASTERN TIME. CONTROL CENTER PERSONNEL FIELD OPERATIONS ACTIVATED OSRO, AND DETAILS WERE COMPILED FOR NRC, PHMSA AND OHIO EPA NOTIFICATIONS WHICH WERE MADE PROMPTLY AFTER THE INITIAL CONFIRMED DISCOVERY. FIELD PERSONNEL AND OSROS WORKED TO RECOVER PRODUCT RELEASED. PHMSA, OHIO EPA AND FEDERAL EPA MADE SITE VISITS TO ASSESS THE RELEASE AND RESPONSE. A REPAIR PLAN WAS DEVELOPED, REVIEWED WITH PHMSA AND IMPLEMENTED TO REPAIR THE DAMAGED SECTION. A RETURN TO SERVICE/RESTART PLAN WAS DEVELOPED, REVIEWED WITH PHMSA AND IMPLEMENTED. THE LINE WAS RETURNED TO SERVICE ON 4/17/2022. FAILED SECTION OF PIPE WAS CUT OUT AND REPLACED. THE FAILED SECTION WAS PRESERVED AND SHIPPED TO A 3RD PARTY LABORATORY FOR ANALYSIS TO DETERMINE FAILURE MECHANISM. SUPPLEMENTAL REPORT WILL BE FILED WHEN ANALYSIS HAS BEEN COMPLETED. SUPPLEMENTAL/FINAL UPDATE - RESULTS OF THE METALLURGICAL ANALYSIS INDICATED THAT THE RUPTURE INITIATED AT AN AREA OF EXTERNAL CORROSION APPROXIMATELY 12 INCHES LONG LOCATED BETWEEN THE 3 O'CLOCK AND 6 O'CLOCK ORIENTATION. THE DEEPEST PORTION APPEARED TO BE LOCATED IN AN AREA APPROXIMATELY 1 INCH LONG WITH A REMAINING THICKNESS OF 0.017 INCHES. THE WALLS OF THE EXTERNAL CORROSION WERE MAINLY GRADUALLY SLOPED BUT STEEPER AT THE ORIGIN WITH EVIDENCE OF UNDERCUTTING, INDICATING A LOCALIZED AGGRESSIVE ENVIRONMENT. THERE WAS NO EVIDENCE OF PRE EXISTING MILL FLAWS OR CRACKING THAT CONTRIBUTED TO THE FAILURE.
