Incident Narrative
ON APRIL 01, 2023 A GUERNSEY TERMINAL EMPLOYEE WAS CONDUCTING THE MORNING FACILITY INSPECTION WHEN HE DISCOVERED A SMALL PUDDLE OF CRUDE OIL NEAR THE RISER ON THE 24 INCH TANK DISCHARGE LINE FOR TANK 8. THE EMPLOYEE IMMEDIATELY CONTACTED ANOTHER FACILITY EMPLOYEE AND THEY PROMPTLY ISOLATED THE TANK DISCHARGE LINE AND THEN CALLED THEIR SUPERVISOR TO NOTIFY HIM OF THE LEAK. THE SUPERVISOR SUBSEQUENTLY NOTIFIED COMPANY MANAGEMENT OF THE LEAK. AFTER NOTIFYING THEIR SUPERVISOR, THE EMPLOYEES THEN CONTACTED A LOCAL VAC TRUCK OPERATOR AND ONCE HE ARRIVED ON SITE THEY RECOVERED ALL FREE LIQUIDS ON THE GROUND SURFACE. THE VAC TRUCK WAS THEN USED TO DRAIN UP THE TANK DISCHARGE LINE. ALL RECOVERED LIQUIDS WERE UNLOADED INTO THE GUERNSEY TERMINAL TRUCK OFFLOADING FACILITY AND INJECTED BACK INTO THE SYSTEM. DUE TO WEATHER CONDITIONS, THE TANK DISCHARGE LINE WAS NOT EXCAVATED AND EXPOSED UNTIL APRIL 12, 2023. UPON EXPOSING THE PIPELINE, FIELD EMPLOYEES DISCOVERED A SMALL PINHOLE LEAK IN THE BOTTOM OF THE PIPE. AFTER REMOVING THE COATING, NO EVIDENCE OF EXTERNAL CORROSION WAS OBSERVED AND IT WAS SUBSEQUENTLY DETERMINED THE PINHOLE LEAK WAS THE RESULT OF INTERNAL CORROSION. THE SECTION OF LEAKING PIPE WAS REMOVED AND REPLACED. AFTER THE SECTION OF PIPE WAS REMOVED, THE INTERNAL SURFACE WAS EXAMINED AND IT WAS CONFIRMED THE LEAK WAS DUE TO LOCALIZED INTERNAL CORROSION. THE RELEASE WAS CONTAINED COMPLETELY IN THE TANK DIKE AND NO SURFACE WATER, GROUND WATER, OR WILDLIFE WAS IMPACTED; HOWEVER, APPROXIMATELY 180 CUBIC FT. OF SOIL WAS CONTAMINATED. THE CONTAMINATED SOIL WAS REMOVED AND WILL BE MANAGED IN ACCORDANCE WITH WYOMING DEPARTMENT OF EQUALITY REQUIREMENTS. THE RELEASE DID NOT MEET ANY STATE OR FEDERAL NOTIFICATION REQUIREMENTS, SO NO NOTIFICATIONS WERE MADE.
