Incident Narrative
HOUSTON CONTROL CENTER RECEIVED AN ALARM ON A UPS FAILURE AT MAINLINE BLOCK VALVE 455 AND NOTIFIED FIELD PERSONNEL AT 2:10PM. IN ROUTE TO THE BLOCK VALVE, THE OPERATOR PASSED BY THE MRD SAILES METER STATION AND NOTICED A FIRE COMING FROM TUBING. THE OPERATOR INITIATED THE STATION ESD AT 3:08PM. AFTER THE ESD WAS ACTUATED AND THE ISOLATION VALVE WAS CLOSED THE FIRE WAS EXTINGUISHED DUE TO PRODUCT NO LONGER FLOWING THROUGH THE STATION. AT APPROXIMATELY 3:30PM, THE METER STATION WAS DEEMED SAFE BY OPERATOR PERSONNEL AND INTERNAL AND EXTERNAL NOTIFICATIONS WERE MADE AT THIS TIME. THE ISOLATOR FITTING WAS REPLACED, AND THE LINE WAS BACK UP AND RUNNING AT 5:30PM. AFTER INVESTIGATION IT WAS DETERMINED THAT THE FAILURE WAS CREATED DUE TO A GROUND FAULT ON AN ADJACENT 3RD PARTY ELECTRICAL HIGH LINE. THE CROSS BRACING ON A NEARBY POLE COLLAPSED AND CREATED A GROUND FAULT/ARC FAULT, WHICH IN TURN CAUSED ELECTRICAL CURRENT TO BLEED TO THE PIPELINE. THE ELECTRICAL FAULT RESULTED IN TRIPPING THE BREAKER FOR THE UPS AT BLOCK VALVE 455 AND CAUSING THE TUBING FAILURE AT THE METER STATION.
