Incident Narrative
ON JANUARY 23, 2024, AT 13:00 FLINT HILLS RESOURCES (FHR) RECEIVED A CALL FROM THE RECEIVING FACILITY'S PERSONNEL INDICATING A RELEASE OF PRODUCT FROM A DRAIN LINE VALVE AT OUR SOUTHLAKE DELIVERY STATION. AT 13:03 AN FHR OPERATOR NOTIFIED THE PIPELINE CONTROL CENTER (PCC) OF THE REPORTED RELEASE. THE PCC SHUTDOWN THE PUMPS AND BEGAN CLOSING UPSTREAM AND DOWNSTREAM VALVES. AT 13:05 AN FHR QUALIFIED INDIVIDUAL WAS NOTIFIED AND AN OSRO WAS CONTACTED AT 13:10. AN FHR OPERATOR ARRIVED ON-SITE AT 13:20 AND CONFIRMED A RELEASE FROM 1/2 INCH TUBING ON A 2-INCH TWIN SEAL VALVE LOCATED ON A DRAIN LINE ON THE PROVER. PRODUCT WAS OBSERVED ON RAINWATER AND THAT PRODUCT HAD MIGRATED OFF SITE TO AN UNNAMED DRAINAGE DITCH DUE TO LEAK IN CONTAINMENT DRAIN/WALL. THE ONSITE FHR OPERATOR BEGAN EFFORTS TO CONTAIN THE PRODUCT BY DEPLOYING ABSORBENT BOOMS AND PADS IN THE SECONDARY CONTAINMENT AND THE DRAINAGE DITCH LEAVING THE STATION. THE DRAIN LINE VALVE WAS REPAIRED ON JANUARY 23, 2024. THE CONTAINMENT VALVE AND DIKE WALL WERE ALSO REPAIRED, AND THE LINE WAS RESTARTED ON JANUARY 25, 2024. CLEANUP OF SOIL AND SURFACE WATER IMPACTS CONTINUED AND WERE COMPLETED ON JANUARY 31, 2024. LEAK DETECTION DATA INDICATED THAT THE RELEASE OCCURRED DURING TWO DELIVERIES; ONE BEGINNING ON JANUARY 22, 2024, AT 00:58 AND THE SECOND ON JANUARY 23, 2024, BEGINNING AT 09:20. AN NRC REPORT WAS MADE 14:34 (# 1389726). AN UPDATE TO THE NRC REPORT WAS MADE AT 18:53 (# 1389749) TO REFLECT PRODUCT WAS A MIX OF GASOLINE AND DIESEL. FHR'S INVESTIGATION DETERMINED THAT THE ROOT CAUSE OF THE RELEASE WAS DETERMINED TO BE MANUFACTURING DEFECT DUE TO INCORRECT ASSEMBLY OF THE VALVE TUBING. IT WAS IDENTIFIED THAT THE TUBING WAS NOT FULLY INSERTED INTO THE FITTING DURING INITIAL MAKEUP, NOT ALLOWING THE FERRULE TO SECURE THE TUBING IN THE FITTING. THE VALVE AND TUBING WERE PRESSURE TESTED BY THE MANUFACTURER PRIOR TO INSTALLATION AT THE SITE ON FEBRUARY 15, 2023.
