EXTERIOR LIGHTING:TURN SIGNAL
Mack Trucks, Inc. (Mack) is recalling certain 2015-2023 GU and GR vehicles. The turn signals may be positioned incorrectly, reducing their visibility. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 108, "Lamps, Reflective Devices, and Associated Equipment."
Remedy: Dealers will add additional turn signal lighting, free of charge. Owner letters were mailed September 25, 2024. Owners may contact Mack's customer service at 1-800-866-1177. Mack's number for this recall is SC0448.

SUSPENSION:FRONT:HUB
Mack Trucks, Inc. (Mack) is recalling certain model year 2012, 2014-2015 and 2017 Granite (GU), 2012-2017 Pinnacle (CHU, CXU), and 2013-2015 and 2017 Titan (TD) trucks manufactured January 7, 2011, to September 29, 2016. The affected vehicles were built with front wheel hubs that cannot support the gross axle weight rating of the front axle.
Remedy: Mack will notify owners, and dealers will replace the hub assemblies, free of charge. The recall began on February 16, 2017. Owners may contact Mack customer service at 1-800-866-1177. Mack's number for this recall is SC0406.

Mack Trucks, Inc. (Mack) is recalling certain model year 2016-2017 Granite (GU) and TerraPro (MRU) trucks manufactured from November 9, 2015, through January 19, 2016. The affected trucks may be missing a roll pin to secure the upper steering yoke to the steering shaft.
Remedy: Mack will notify owners, and dealers will inspect the vehicles and replace the steering shaft if the roll pin is missing, free of charge. The recall began on April 1, 2016. Owners may contact Mack customer service at 1-610-709-2131. Mack's recall campaign number for this recall is SC0400.

THIS REFERRAL INVOLVES A DUMP TRUCK INJURY ACCIDENT RESULTING IN THE VEHICLE OVERTURNING. I AM A KY OSHA COMPLIANCE OFFICER ASSIGNED A COMPLAINT IN REGARDS TO THIS VEHICLE. THE COMPLAINANT ALLEGES THAT THE TRUCK NEEDED NEW TIRES. THEY ALLEGE THAT A BLOWN WORN TIRE CAUSED THIS ACCIDENT. THE ACCIDENT HAPPENED NEAR MARION, KY. THE TRUCK IS NOW IN A COLLISION REPAIR CENTER AT 945 NANDINO BLVD IN LEXINGTON, KY. I INSPECTED THE VEHICLE AND IT HAD A WORN INNER RIGHT REAR TIRE. THE TRUCK WAS LOADED WITH 25 TONS OF GRAVEL AND TRAVELING AT APPROXIMATELY 45 MPH WHEN THE DRIVER LOST CONTROL OF THE VEHICLE WHICH EVENTUALLY OVERTURNED. NO OTHER VEHICLES WERE INVOLVED.
