On December 16 and 17, 2025, the manufacturer (Tesla, Inc.) utilized remote telemetry to modify the software state of my 2025 Model Y while it was being operated on public roadways. The manufacturer remotely switched the vehicle profile to "Inventory Mode" (Error Code displayed: "Delivery Type: PICKUP_SERVICE_CENTER"). The Safety Failure: This unauthorized remote manipulation immediately DISABLED
... the electronic Child Safety Locks without driver consent or warning. My minor children were seated in the rear of the vehicle at the time. The rear doors, which had been locked for child safety, suddenly became operable from the inside while the vehicle was in motion - and my children actually opened the doors while the vehicle was in operation. (Thus my discovery of the issue) The Hazard: This defect allows the manufacturer to remotely override and deactivate critical safety retention features (Child Locks) on a moving vehicle. This created an immediate risk of accidental door opening and passenger ejection for the minor occupants. Manufacturer Response: The manufacturer admitted in writing (email dated Dec 15, 2025) that there was "No Active Financing" on the vehicle, yet persisted in accessing the vehicle's safety computer systems to alter its status. This demonstrates a systemic defect where Tesla’s remote access tools lack safety interlocks, allowing them to degrade safety features on vehicles currently in motion/use by consumers. Status: The vehicle remains unsafe as the manufacturer retains the ability to toggle safety locks remotely without operator input.Read more

Vehicle: 2020 Subaru Ascent Issue: Cold weather software glitch If you are operating an Ascent or possibly other Subarus with the CVT in temperatures below -20F be certain to have either a small wrench or the appropriate size socket to loosen the ground clamp on the battery ground cable with you so you can employ what I call the magic Windows fix wherin one completely de-powers the system and reb
... oots it. With all the ridiculous interconnections, if the CVT temperature sensor thinks the tranny is too cold the cars operating system looses it mind and goes completely bonkers. Subaru obviously did no cold weather testing. We have had 2 failures in cold weather. It only happens at temperatures well below the-20F range. The first case happened last year while driving after being stopped for a couple of hours. Everything seemed normal and then suddenly several warning lights came on and all of the safety systems and the eyesight system shut down. The car ran and drove, but none of the associated systems would work. No traction control. No cruise control. Several other systems. The only solution was to disconnect the battery and get this rolling computer to reboot. This was a temporary fix as the condition repeated itself although the car was driveable but with none of the safety systems operational. This year we were out in cold weather, -25 to -32. We went to a place to have lunch and when I went to restart the car the app failed to do so. So I put on all my gear and went out to start it manually. It would not start and all the warning lights were on and the radiator fan was running full tilt at -25F with the car off. I tried twice to get it going by pulling the battery cable to get it to reboot. Evidently when this happens you must leave the battery disconnected long enough (about 5 minutes) for all the capacitors in the electronics to drain. Once restarted, the car drove normally but we did not shut it down for any reason for the rest of the day.Read more

I have spoken with other agencies and the same issue is also in other Durango's. The term ghosting I learned after google defined it as that. This is a risk as it causes excessive strain on the eyes of the operator. The other officer on the department has been through several contacts after I brought up my issue she realized that her windshield had the same issue. The image shared with this is how all lights look through the windshield. The further you are from the source the greater distance between source and ghost image.Read more

My Volvo S60 has developed excessive oil consumption consistent with the condition described in Volvo’s “Engine Oil Consumption” extended-warranty notice. I only received this notice after my vehicle had already passed the 8-year/100,000-mile eligibility period, preventing me from accessing the program intended for this defect. I visited an authorized Volvo dealer regarding the oil-consumpti
... on issue, and the dealer recommended performing Volvo’s official oil-consumption test. Before authorizing the test, I asked Volvo to cover the cost and confirm reimbursement eligibility. Volvo declined these requests, requiring me to pay for the test and any related repairs. I proceeded with the official oil-consumption test at my own expense, and the results confirmed abnormal oil consumption. Despite this, Volvo again declined reimbursement or goodwill assistance and did not provide any written, technical explanation based on the confirmed test results. This condition poses safety concerns, including possible engine damage, stalling, or sudden loss of power if oil drops unexpectedly. To maintain safe operation, I must frequently check and top off the oil. I also previously owned another Volvo model (XC60 2015.5) equipped with the same engine family, which experienced a piston-ring failure in late 2021. At that time, the recommended repair was piston-ring replacement—the same repair now recommended for this S60. Volvo declined coverage for that prior failure as well. Experiencing similar defects in two Volvo vehicles with the same engine family suggests a broader pattern that may impact safety. I am reporting this issue to NHTSA because: • Volvo sent the program notice only after my vehicle was no longer eligible. • Volvo denied reimbursement despite a confirmed defect. • The defect creates safety risks due to potential loss of power. • My experience aligns with reported patterns of excessive oil consumption and piston-ring failures in this engine family.Read more

My vehicle experienced a sudden and immediate loss of motive power due to a catastrophic internal engine failure, creating a severe road safety hazard. While driving and attempting to accelerate or merge, the vehicle exhibits extreme hesitation, severe misfiring on cylinder #2, and rough, unstable idling. This unexpected loss of drive power made the vehicle unpredictable and unsafe to operate und
... er normal driving conditions, significantly increasing the risk of a crash. The vehicle is currently at the Hyundai dealership (Fox Ann Arbor Hyundai) for diagnosis. The dealer has confirmed and documented the following internal mechanical defects at only 86,000 miles: Compression loss in cylinder #2. Cylinder wall scoring and pitting. Evidence of blow-by and heavy carbon buildup in the combustion chamber. Misfire and rough idle. Oil consumption as a result of the cylinder failure. This failure is a premature mechanical defect, not normal wear. Failures of this nature (sudden loss of motive power due to internal engine defect) have been cited by NHTSA in other recalls and are known to increase the risk of a crash or fire. The manufacturer has acknowledged the defect by offering only 50% goodwill toward a repair, but this defect remains a critical, unresolved safety hazard that requires a full, safe engine replacement."Read more

Complaint Summary: Steering system failure remains unresolved after Honda warranty repair. Complete loss of electric power steering. Complaint Details: I experienced a sudden and complete loss of electric power steering on my 2013 Honda Crosstour V6. The EPS warning light illuminated, steering effort increased drastically, and vehicle control at low speeds became difficult and unsafe. The vehicle
... was brought to a Honda dealership, which replaced the EPS control unit under Honda’s warranty extension 19-036. However, after the repair, the power steering is still completely inoperative. The same original diagnostic trouble code (DTC 32-09) returned immediately, showing that the underlying defect was never resolved. The dealership now claims the EPS steering gearbox has failed and quoted approximately $5,000, even though the failure began before the control unit replacement and the EPS system remains non-functional after the warranty repair. This appears to be an incomplete or incorrect repair of a safety-critical steering system. The vehicle continues to have zero steering assist at all times, causing significantly increased steering effort, especially at low speeds and during parking maneuvers. This creates an ongoing safety risk to the myself and others on the road. Loss of power steering after a warranty repair suggests a potential defect trend or misdiagnosis involving the EPS system. I am requesting that NHTSA review this incident for possible safety implications affecting steering reliability.Read more

My wife and I purchased this vehicle brand new from the dealer in Feb 2025. To date, we barely have 4,000 miles on the car. The entertainment system works, but no sound, the blinkers work but no sound. More important, the
... car heat/cooling systems don’t operate, the light indicating cars are in your blind spots do not operate. We brought our vehicle back to our local dealership each time. In September, the dealer reached out to Volvo to have the computer “brain” replaced. They had the car for three days only to be told Volvo denied the replacement because what is occurring is a known issue and an update release was coming in November to fix the issue. November came and went and still no update. We spoke to the dealership and they had no idea. They recommended us contacting Volvo directly. We reached out to express our frustration and was told someone would get back in a few days. About a week later, we received an email from Volvo expressing their sorrows for our inconvenience and to open a case with our dealership. I was extremely disappointed as I responded indicating that’s what we have done time and time again and there’s nothing they can do. It’s a known error and the only way to reset to have the car parked and hold the menu select button. It takes about 3 minutes or so for the system to reboot. The last time this occurred we were driving home and the system shut off. This left us without heat and no safety features. Volvo is clearly aware of the issue and it appears there’s no urgency to get this resolved. We paid a lot of money to purchase this vehicle and it’s been a big disappointment We are looking for some assistanceRead more

(invoice attached) On or about August 1, 2025 we were traveling on an interstate at 70mph. Without any forewarning, the vehicle began to violently shake. Just as before, the skid warning light simultaneously came on, followed by the check engine light came on, and then the oil pressure guage started bouncing around. As there was heavy traffic on the interstate, the failure created a very dangerous situation for us as we tried to maintain our lane, and stay in control of the vehicle until which time we were able to safely maneuver to the side of the road. The vehicle was again towed to Vann York GMC in Asheboro, NC. (invoice attached) On December 3, 2025 we received a call from VannYork GMC saying that the vehicle repairs were complete. It had been in their repair shop for 4 months. This repair began with lifter and camshaft replacement. When this repair was unsuccessful, they decided that the vehicle needed a 'new' rebuilt engine. Since GMC has not offered any assistance with the expense of this new re-built engine, we will have to satisfy the $12,000 bill when we pick it up. After experiencing two very dangerous episodes, we are happy that we did not wreck, but certainly, this not the situation we expected from a high-end vehicle with only 76,169 miles on it.Read more

I am still driving this vehicle as well as my wife because we have no other choice but I do feel that this situation poses a safety hazard for us and our family, as the windshield is a distraction and can cause the driver to lose focus on objects while driving.Read more

Let me Preface this with, my Make, Model, and year for these problems have been recalled back in 2021, however I was not the owner then and my VIN was not listed as an affected vehicle i guess, nonetheless the problem is exactly the same as experienced by those that were listed on the recalls in the past. Im the 2nd owner of this vehicle and I have owned it for almost 3 years now. In July 202
... 5 after a summer rain in so. cal, I was a couple blocks from home running errands and after being stopped at a light, i began to accelerate and felt like the vehicle was loosing power, I made it home but definitely not able to go the speed limit and once i got home I couldnt shift it into any gear and the vehicle would not turn off. So I had to remove the fuses under the hood, I left them out for a few hours and let the car sit off in my garage during that time. I later came back put the fuses back in and then the car wouldnt turn over. I had it taken to the dealership by AAA and thats when i first learned of the sunroof leak, I contacted Volvo Headquarters and got no help and explained all of the same to the service dept at the dealership working on my car. Long story short the dealership allegedly found water under my interior carpet which obviously damaged modules, I am still paying on that repair. Fast forward to October 2025, same thing all over again only this time the dealership tells me essentially its on the house, however they had my car 3 weeks and did no repairs only got water out of it once again and “brushed off some corrosion”. When I pick it up, it immediately starts funny, i turn the car off and walk right back into the service dep t& raise my concern, to which i was told they had no problem with a hard start up, and we go out to my car & it starts up fine, so i leave 4 days after I got my car back, the check engine light comes on, & immediate warning of reduction of engine power & sure enough thats what it did. today it enters safety mode & wont startRead more

Monday (11/17/2025) after work: car vibrated <10 seconds then stopped when turning on car. Tuesday (11/18/2025): ALL warning lights came on. Schedule appt at Honda for Saturday when they could see us. Wednesday (11/19/2025) was driving to AutoZone to have battery checked (>3 years ago all lights went on and it was the battery) when car went from 70mph on highway to not exceeding 20mph with my fam
... ily in the car during rush hour. I had to stay on the road as there was no shoulder resulting in a near mass car accident as everyone slammed brakes to avoid me. Went <20mph to closest car shop (right off the exit). Car shop without looking said most likely the head gasket which is a known Honda CRV 2017 issue. Said maybe spark plugs so we tried as its a significantly less costly fix which did not solve the issue. Had it towed to Baierl Honda on Friday (11/21/2025) before the car was even at Honda the worker told us it was a head gasket issue. Once looking they confirmed $5000 head gasket issue with potential for another $5000 in damage once they open the engine. There are several class action lawsuits out for this very issue which is a huge safety risk to those in the car as well as those around as the engine stops working no matter what speed you are traveling. We reached out to Honda corporate and have yet to hear back. We have not cleared Honda Baierl to work on the problem yet so it has been sitting in their lot.Read more

2021 Volvo S60 T8 AWD, VIN [XXX] , ~71k miles Component: SUNROOF / DRAINAGE SYSTEM / ELECTRICAL SYSTEM Summary: Water entered the passenger compartment through clogged sunroof drain hoses and accumulated under the front passenger seat, causing catastrophic failure of wide angle module (WAM) and related electronic components (incl. front camera and wipers). Description: After moderate rain, a la
... rge amount of water was discovered under the front passenger seat. The vehicle began displaying multiple warning messages (camera faults, wiper faults). The authorized Volvo retailer diagnosed failure of the WAM and other modules due to water intrusion originating from the sunroof drainage system. This is a known, recurring defect acknowledged by Volvo in multiple Technical Service Bulletins: - TJ 31372-2 (Water leakage via sunroof drain hoses) - TJ 35382 (Updated drain hoses - new part number) - TJ 29322-3 (Water ingress passenger footwell / CEM area) The original drain hoses are too narrow and prone to clogging, causing water to overflow into the passenger compartment instead of exiting the vehicle. This creates a serious safety risk: - Potential failure of SRS airbag system - Loss of electronic stability control and ABS - Loss of 360 camera and blind-spot monitoring - Risk of electrical fire or short-circuit - Failure of wipers (mine stopped working when I was driving under the rain) Repair estimate: $17,825 Repair is going to be delayed over 2 months awaiting parts from Sweden. Vehicle was properly maintained; 70,000-mile service performed at authorized Volvo retailer on 9/24/25 - no drainage issues were noted or addresses at that time. This identical failure has been reported by hundreds of owners of 2019-2024 SPA-platform Volvos (S60, V60, XC60, XC90) on NHTSA complaints, Volvo forums, and in ongoing litigation. Despite multiple TSBs and redesigned parts, Volvo has not issued a recall or notified owners of the need for preventive maintenance. The contact owns a 2021 Volvo XC90. The contact stated that upon starting the vehicle and shifting to drive(D), the vehicle failed to respond unless the brake pedal had been released for 2-3 seconds. Additionally, the contact stated that upon starting the vehicle and releasing the brake pedal to reverse out of the driveway, the vehicle rolled back at a faster speed, and the vehicle failed to respo
... nd as intended upon depression of the brake pedal. The contact noticed a pattern with the brakes malfunction. The contact stated that the brake pedal required to be depressed 2-3 seconds after the first depression for the brakes to operate as intended. The contact stated that he was a master mechanic and had concluded that the failure was considered a safety issue because the vehicle was only designed with electronic braking, which left the driver with no manual option for braking. The contact stated that the failure had only occurred upon starting the vehicle. The contact related the failure to NHTSA Campaign Number: 25V392000 (Service Brakes, Hydraulic, Service Brakes); however, the VIN was not included. The vehicle was taken to the dealer, where it was diagnosed that the brakes had failed and Software Update Number: 3.5.14 was needed. The vehicle was not repaired. The manufacturer was informed of the failure, and it was confirmed that the repair was not covered under the recall or under warranty. The failure mileage was approximately 78,500.Read more

The serpentine belt on my 2018 Infiniti Q50 Sport became contaminated with oil immediately after dealer turbo replacement at the end of August 2025, causing the belt to slip, shred, and be pulled into the engine. This resulted in sudden loss of essential engine functions and poses a significant safety hazard. This failure was caused by improper repair at the dealership.”

I am filing this complaint regarding a serious defect in my 2021 Toyota Corolla (Manual). At approximately 42,000 miles, the clutch slave cylinder failed. This defect caused hydraulic fluid to leak and contaminate the clutch assembly, resulting in significant damage and loss of proper function. Despite the fact that this issue stems from a defective part, the dealership has refused to cover the r
... epair of the clutch assembly, stating that the clutch is not covered under warranty. Instead, they are requiring me to pay for the repair out-of-pocket, which would not have been necessary if the damage had not been caused by the defective clutch slave cylinder. I believe this is unfair and unsafe, as the failure of a clutch slave cylinder directly impacts the drivability and safety of the vehicle. This is a significant safety issue. A defective clutch slave cylinder can lead to sudden clutch failure, loss of vehicle control, and increased risk of accidents. This is not a wear-and-tear issue but a manufacturing defect that compromises the safety of the vehicle. I respectfully request that NHTSA investigate this matter to determine whether similar failures are occurring in other Toyota Corolla vehicles of this model year. If this is a widespread defect, I urge NHTSA to take appropriate action, including a recall or requiring Toyota to cover repairs.Read more
