Now that it’s cold again, similar codes are being thrown with new ones: Service Trailer Brake System, Service Air Suspension System, Service Electronic Braking System. I purchased the 2022 Lexus RX 350 used a couple of months ago. While driving down the highway the instrument panel went blank. I was terrified. The light on the instrument panel began to darken. My husband was driving and I was in the passenger seat I was able to access the light app on the dash and restore the light to the original setting. This car is not on the recall list. It should be because this is a matter of life or death. The car had 59000 miles on it when we bought it so it is not under warranty. I contacted Lexus and they refused any help until I take it to a Lexus dealer and have it diagnosed. During these occurrences, safety systems and alerts may become unavailable or inoperable, leaving the driver without audible or visual warnings. Audio alerts and system feedback have also stopped functioning during these events, further increasing the safety risk. The vehicle has been brought in for service approximately 8–9 times for various concerns, including these safety issues. Despite repeated service visits, the problems remain unresolved. Restarting the vehicle sometimes restores functionality, but the defects continue to recur unpredictably. This vehicle is used to transport young children, and these failures present a significant risk of serious injury or death. I am reporting a safety-related defect involving the Automatic Emergency Braking (AEB) system in my vehicle. The AEB system has repeatedly activated without any apparent reason, causing the vehicle to brake suddenly when there are no obstacles, vehicles, pedestrians, or road hazards present. ... least five times (possibly more) for this same issue. Each time, the dealer has stated that the vehicle is “operating as designed” or that they were unable to identify or correct the problem. Despite these visits, the unintended braking continues to occur. There have been passengers in the vehicle during these incidents who witnessed the sudden, unnecessary braking and can confirm that no hazard was present at the time. This defect creates a serious safety risk, as unexpected braking could easily result in a rear-end collision or loss of control. I am increasingly fearful that this issue will cause an accident. ... s a structural failure of the tire (broken internal cords/belts) and creates a risk of sudden blowout while driving at normal highway speeds with my family in the vehicle, including my young child in a car seat. Because a sidewall bulge can lead to catastrophic tire failure without warning, I consider this an immediate safety hazard and stopped using the tire as soon as the defect was noticed. Pattern / Recurrence: This is my second Volvo XC60 lease equipped with Pirelli Scorpion Zero All Season tires in size 255/40 R21. On my prior XC60 lease, two of these same model tires experienced severe, premature tread wear and had to be replaced at low mileage due to safety concerns. In researching this issue, I found numerous other Volvo XC60 owners reporting similar problems with Pirelli Scorpion Zero All Season tires, including tread chunking, premature wear, and sidewall bulges at relatively low mileage. Volvo service advisors have stated to me that this is a “known problem” with certain Pirelli tires on Volvo vehicles. Reproduction / Confirmation by Dealer or Service Center: The problem has been observed and confirmed visually by my Volvo dealer’s service department, which documented the sidewall bulge and recommended immediate replacement of the tire for safety reasons. They advised that Pirelli treats this under a prorated warranty, which would require me to pay a portion of the replacement cost, but they did not dispute that the tire is unsafe to drive on. DISAPPEARING SAFETY RECORDS: I possess a prior screenshot from the official portal confirming that Recall 95B (High Voltage Battery) was also successfully REPAIRED by the same dealer. However, this record has recently DISAPPEARED from the active recall list on the manufacturer's website. This removal of a safety record, combined with the current denial of service, raises serious concerns about data integrity and safety compliance. 3. PROVEN TECHNICAL CAPABILITY: The successful completion of Recall 95B and 04B-both involving high-voltage systems and software-proves the dealer is fully equipped. I called them to get an update but Pablo had no update and said he had to call the dealer! The Service Tech I had talked to on 1/5 about the car left me a voice mail at 5:05 PM on 1/5 stating that the foreman has been in contact with Volvo Engineering and that they had a few questions -- how many times had this issue occurred, how did I get the car to stop running and they requested a photo of the shoes I was wearing. I sent that photo that evening but I never received any response from numerous texts asking for a status. They have not provided a courtesy loaner for the past 15 days so I have had to incur incidental rental charges. 1/9/26, I spoke to Jocelyn at Volvo - she was unable to give any updates but she sent me a form (one of the questions asked where the vehicle was!) -- I emailed the completed form that same day but received no acknowledgement from them until 1/15. They sent me a form authorizing an investigation of my car. I have not received any further communication from Volvo Cust. Care or the dealer. The vehicle has been inspected by authorized BMW dealerships and BMW North America has been notified, though I have not received guidance from corporate BMW regarding a plan moving forward or instructions on how to proceed should a fourth brake failure occur. Brake related warning lamps and system messages appeared following both the first and second recall repairs, and after the October 30, 2025 recall repair the vehicle experienced a brake system failure within approximately 22 miles of driving despite the presence of warning indicators. Given the repeated failures after recall repairs, the prior warning messages, and the lack of direction from BMW North America, I am concerned about operating the vehicle, particularly for longer distances, due to the unpredictable nature of the brake failures and the unresolved safety risk.Read more

The vehicle had to be towed from the shoulder of the freeway. This incident posed a serious risk of severe injury or death, not only to my passengers but also to other motorists. This was not an isolated incident. This is the second time this vehicle has experienced a sudden shutdown. The first incident involved a warning reading “Critical Propulsion Fault,” which also resulted in loss of vehicle function. Following the Christmas Day incident, the vehicle has been at an authorized Volvo dealership for over three weeks. As of today, Volvo has been unable to identify the root cause or provide a clear explanation or repair. The uncertainty surrounding the failure of a brand-new vehicle (1,754 Miles) further reinforces the seriousness of this defect. A vehicle that can unexpectedly lose all power at freeway speeds is fundamentally unsafe. Based on my experience, I believe the Volvo EX90 presents a significant safety defect, and I am deeply concerned that these vehicles continue to be sold and driven on public roads. ... bration and impact on maintaining control. Fortunately, she was in the right lane and had the presence of mind to pull off the road on to the shoulder. There was heavy traffic at that time and this incident could have resulted in loss of control of the vehicle and a collision. I had recently checked the tire pressures and they were all set correctly. She called me on her cellphone and I drove my car to meet her. I replaced the damaged tire/wheel with a temporary spare. I am a retired Technical Compliance Manager for Nissan where I was responsible for dealing with any safety issues on Nissan vehicles and ensuring compliance with NHTSA regulations. I inspected the failed tire very carefully and saw no evidence of a puncture. This appears to me to be a catastrophic failure due to a manufacturing defect. The failed tire is a Bridgestone 255/50R21 Alenza Sport A/S which is the factory-equipped tire. I lease this vehicle directly from Nissan North America as a retiree. I returned the vehicle to Priority Nissan in Chantilly Virginia to have the tire replaced. I also advised the service manager to report this incident to Nissan and save the tire for their inspection. ... updated (TSB 21-2145), the earliest appointment I was able to get was 1/13/2026. I drove the truck again on Sunday (1/11/2026) for church, and upon returning home, the wrench icon appeared on my dash board. I did not drive the truck again until 1/13/26 for the appointment. I took the truck in on my appointment and shared my concerns with the technician about it being a bigger problem than a software update, and he assured me he would complete proper inspection and diagnostics. After the tech completed the software update (& reset shift tables) the truck drove worse. He informed me, he believes the truck will need a replaced transmission. On my way home the truck drove worse, Slamming gears harder, and hesitating worse, worse delayed acceleration. The truck is not safe to drive. I took it to another reputable transmission shop in town 1/15 for a 2nd opinion, and he concluded the same. Transmission needs to be replaced. The frustrating thing in all of this, is I had an extended warranty on the truck through 100k miles. I took the truck in to the local Ford Dealer for a full inspection at 99,xxx miles to verify all was good, and if it wasn't then I would like to use my warranty for repairs. I was given a bill of good health. And approx 5000 miles later, I now need a full transmission replacement? I am getting no-where with Ford.
