This supplements my prior complaint, ODI #11751961, for the same vehicle. Following that complaint, an authorized dealer performed front camera and front radar ADAS calibration on 7/21/2026 and returned the vehicle as operating as designed; the manufacturer's representative confirmed in writing on 7/22/2026 that the camera had been within specification. On 7/23/2026, approximately 1:21–1:35 PM,
... driving in the left lane of a divided parkway with a concrete median barrier, with HDA2 and Lane Following Assist both active and indicating green, the steering wheel turned the vehicle toward the concrete barrier at least seven times, requiring manual correction each time. This occurred both with a lead vehicle approximately 4 seconds ahead and with no vehicle within more than 10 seconds ahead. Earlier the same day, driving the opposite direction with both systems active and green, the vehicle applied hard automatic braking twice with no vehicle within more than 10 seconds ahead (traffic present only in the adjacent lane); I had to manually accelerate because a following vehicle was close behind. My original complaint reported the automatic braking failing to activate; the system now also activates falsely. Additionally, the hands-on-wheel warning repeatedly illuminated while my hands were on the wheel, taking approximately 30 seconds to detect them. I have stopped driving the vehicle. The manufacturer was notified in writing the same day, and a second repair visit is being arranged. No crash or injury has occurred.Read more

The vehicle went into a limp mode due to programming and safety issue known by Honda. One of the six fuel injectors failed according to the fault codes at the dealer. They won't put anything in writing n
... or will they consent to any recorded calls. I have reached out to Brad Ortloff as well and he won't respond. What are they hiding? Why did NHTSA allow Honda to get away with extending the warranty on the fuel injectors for certain Honda Odysseys rather than recalling them? What is the difference between the vehicles that were included? There are numerous vehicles that weren't included that are failing for the exact same reason. I can't get any answers from Honda. They just say they are not licensed techs. Brad Ortloff won't respond. Why is NHTSA allowing Honda to put citizen's safety at risk? Will someone please investigate this safety issue?Read more

What component failed: The “regen on demand” switch broke while using it. Safety: This incident put me, the occupants of my vehicle (my children), and the vehicles around me at risk. Reproduction: There was no need. It is exceptional obvious that the switch is broken and has failed. Inspection: The truck and broken switch were observed by the dealership that services the truck.
... Warnings: There were no precursors to the failure. Narrative: On Thursday, 7/16/26, I was driving my 2026 Chevrolet Silverado EV. I normally drive in “single pedal drive” mode which provides energy recovery in lieu of mechanical braking. Additionally, I use the “on demand regeneration” for additional slowing and energy recovery. On demand regeneration is somewhere between single pedal drive and mechanical brakes. During this drive I was coming up to stopped traffic and slowing using the regen on demand switch. The switch snapped / broke and the additional slowing / energy recovery stopped instantly. I had to immediately apply the brakes in order to remain a safe distance from the vehicles in front of me. While I am able to safely operate the vehicle without this function, the manner in which the failure occurred is concerning and should be investigated. I am happy to speak with anyone who needs more information.Read more

On July 15, 2026, I took the vehicle to Lafontaine Ford for maintenance and to address open recalls. I was informed that the open recall for the low-pressure fuel pump (campaign 25S75) did not have a solution available. Due to the severity of the recall, I contacted the Ford Customer Care Center. On July 17, 2026, I followed up with Lafontaine Ford and confirmed they had received no contact or notation from Ford regarding prioritization. Lafontaine Ford also stated they had no information on when parts would become available and that nothing could be done at that time. I then contacted the Ford Customer Care Center again because no action had been taken, even though the recall has been open since approximately July 8, 2025. A Ford Customer Care Center representative confirmed that no remedy was available for my model year and that they did not know when one would be available. Due to the seriousness of the recall, I requested a courtesy vehicle. The Ford representative contacted Lafontaine Ford regarding this request and then disconnected the call. The Lafontaine Ford representative informed me they could not provide a courtesy vehicle without written confirmation from Ford due to the dealership having no reasonable timeframe of when parts/remedy would be available. When I called the Ford Customer Care Center back and explained what Lafontaine Ford had told me, I was treated indifferently and was disconnected twice. I eventually spoke with a supervisor who refused to escalate the concern or provide any assistance with the documentation Lafontaine Ford said they needed in order to provide a courtesy vehicle. Piston rings inside the engine failed due to a known oil consumption issue with this model vehicle. The engine failed while the car was driving 85 mph and consequently slowed down to 15mph on the highway, leaving the owner of the car stranded alone on the highway. Volvo has a known fix for this issue and there was a class action lawsuit for it, but since it was settled, there was no recall. This i

Water frequently leaks down the A-pillar directly onto the Central Electronic Module (CEM) causing unsafe electrical issues and potential engine failure. This was a known issue on other models, but it is also present in this model vehicle. When I tried to fix this under a warranty contract, I was denied by the certified repair shops.Read more

I am submitting a second and more urgent safety complaint regarding the Driver Monitoring and Driver Assistance systems in my vehicle. ... as clearly not been recognized or addressed. This defect has now placed my life in immediate danger. While I was driving, the system falsely determined that it could not properly see my face. It sounded repeated warnings and then abruptly disabled driving assistance. That sudden withdrawal of assistance created a near-crash situation. I was attentive and was not experiencing a medical emergency. The danger was created by the system’s false detection and defective response. A safety system should reduce danger. This system detected a supposed problem and made the situation more dangerous by removing the assistance that was helping keep the vehicle under control. Had this occurred beside a concrete barrier, near stopped traffic, in a construction zone, on a curve, or next to a large truck, the result could have been a fatal collision. I could have been killed because of this system defect. When a driver cannot be verified, the vehicle should maintain lane control, reduce speed, activate hazard warnings, and perform a controlled safe stop. It should never abruptly withdraw assistance and create a more dangerous condition. I request that NHTSA investigate why a previously reported life-threatening defect remains unresolved and require Hyundai to correct this system logic before someone is seriously injured or killed.Read more

I am reporting a safety concern involving a delayed transmission gear engagement and delayed rearview camera activation. Intermittently, when shifting the vehicle from Park into Reverse or Drive, there is a noticeable delay before the transmission actually engages the selected gear. During this delay, the rearview camera also fails to activate immediately. Because there is no immediate indication
... that the requested gear has engaged, I have experienced situations where I believed the vehicle was in Reverse or Drive, but it remained in the previous state for several seconds. In some instances, the transmission engages unexpectedly after the delay, causing the vehicle to move in a direction different from what I anticipated. For example, after selecting Reverse, the vehicle may hesitate before moving, or after changing between Reverse and Drive, the delayed engagement can result in the vehicle moving in the previously engaged direction before finally shifting into the selected gear. This creates a significant safety hazard, particularly when parking, backing out of driveways, or maneuvering in areas with pedestrians or other vehicles. The delayed rearview camera activation further increases the risk because it prevents the driver from immediately seeing behind the vehicle while attempting to reverse. The issue occurs intermittently and without warning, making it difficult to predict. I am concerned that this condition could lead to a crash, property damage, or injury if the vehicle moves unexpectedly due to the delayed gear engagement. I request that NHTSA investigate this issue to determine whether it is related to a transmission, electronic shift control, or software defect affecting the safe operation of the vehicle.Read more

The infotainment touchscreen in my 2021 Honda Accord Hybrid intermittently registers inputs that I am not making. During these "ghost" episodes, the touch capability seems to also be intermittent preventing from leaving the current screen or app that the "ghost" clicks on. This problem occurs while the vehicle is being driven and creates a serious distraction. The unexpected sounds, screen changes, and navigation changes draw the driver’s attention away from the road and may require the driver to attempt to stop or correct the malfunctioning system. This increases the risk of a collision and could result in serious injury or death. I contacted a Honda dealership regarding the issue. Vehicle Component Affected: Electrical System / HVAC / Blower Motor Resistor Incident Description: While operating my 2023 Kia Sorento, the HVAC system suddenly suffered a complete failure, causing all airflow to cease instantly. A subsequent professional diagnostic inspection by an authorized Kia dealership revealed that the contacts at the blower motor resistor were severely burnt, causing the
... blower motor relay to burn out. This failure presents two critical safety and fire hazards: Fire Risk: The physical burning and overheating of electrical components and wiring within the dashboard assembly create an active fire hazard in the passenger cabin. Loss of Visibility: The sudden, complete loss of the climate control system severely compromises passenger safety by preventing the vehicle from defogging or defrosting the windshield during inclement weather, presenting an immediate crash hazard. This exact failure mechanism matches the description of Kia Safety Recall SC351 (NHTSA Campaign 25V649) word-for-word. However, Kia has stated that my specific VIN is excluded from the current recall population. Because this identical safety defect is actively manifesting outside of Kia's restricted recall range, I am requesting that the NHTSA investigate an expansion of SC351 to include all affected 2023 Kia Sorento vehicles to mitigate the risk of electrical fires and sudden visibility loss. 06/29/26 Experienced symptoms Inspected and diagnosed on 07/01/26 at JT Kia (Killian Road) Columbia, South CarolinaRead more

I own a 2022 Hyundai Palisade SEL. On XXX, while driving at the intersection of [XXX] and [XXX] in [XXX], the engine abruptly lost power, causing a sudden loss of engine power while the vehicle was in motion. This is my family's primary vehicle, and my wife and children were in the car during this incident. On XXX, I brought the vehicle to Courtesy Hyundai in [XXX]. The dealer found essentially no
... oil in the engine despite my keeping up with scheduled oil changes. The dealer replaced the oil and performed a software update described on the repair order as "9C9 C ECU SW UPDATE EMISSIONS IMPRV 26-01-010H," and I was instructed to return after 1,000 miles for an oil-level recheck. At that recheck, the dealer's service assistant reported the oil was full. On XXX, before a planned family road trip, I returned the vehicle for another check. The service department found the engine was two quarts low — this was only 4,000 miles after the oil service on April 10, 2026. The dealer stated they requested an oil consumption test but that Hyundai denied the request. Per the dealer's instructions, I returned again after another 1,000 miles, and the engine was one quart low. I am filing this complaint because I believe the excessive oil consumption and the resulting loss of engine power represent a safety defect that has not been remedied. An engine that abruptly loses power while driving creates a serious hazard, including loss of power steering and reduced braking assist, and I am concerned for my family's safety while this condition remains unresolved. I looked to see what kind of recall is for on it and I'll see there is a lot of complaints related to the transmission and when I called Ford they said mine was not included with the recalls on the transmission although my transmission is also jerking, hard shifting, along with a shuddering, loud clunking when yo
... u shift and to reverse or drive, Also my park break has been kicking on by itself and it's done it to me while sitting in traffic, decrease in acceleration (it about got me hit on the highway) as the car could not get gas It would drop dramatically from 70 down to nothing almost on interstate I-75 highway. Also I've had a burning smell coming from the engine. My car is now at the transmission shop and they are trying to figure out what's going on with it I've had it to AutoZone for them to put it on their tester and a lot of transmission codes came up and one of them so happen to bend the clutch solenoid which could be caused by leaking fluids! I called Ford the corporate office that and they said well you're 20 20 Ford Escape It's not the one in the recalls or the will with the transmission issues but yet it is doing the exact same thing as the other ones that are listed!! It has just a little over 100,000 mi on it The car's not driven hard whatsoever. I was not giving much warning about the transmission The little wrench thing came on flashing and then suddenly these things start happening while on the highway, but then did not stay on! I had to drive my car 45 mi at speed about 30 to get it home.. I do use my car once in a while to drive Lyft which now I'm without being able to make some extra money for groceries and whatnot because of this car! Also the one of the recalls that is on this car pertains to the transmission updates and according to Ford they don't have the tool to update it but yet this update could affect the functioning of the transmission.. I was at risRead more
