Technical Description of the Defect: The issue stems from an improperly molded fuel pump impeller—a component responsible for fuel delivery within the pump. The molding defect results in the production of low-density impellers that are prone to deformation. Over time, these impellers can degrade and interfere with the fuel pump body, ultimately rendering the fuel pump inoperative. Should the fuel pump module fail during vehicle operation, the engine may not start or could stall without warning, which significantly increases the risk of a crash or injury due to the sudden loss of engine power, particularly at high speeds or in heavy traffic conditions. Manufacturer’s Response and Limitations: Honda has stated that it will replace the affected fuel pump modules free of charge, and claims that no crashes or injuries have been reported at the time of the announcement. ... lership. I had a friend drive us to enterprise where they put us in a Jeep Grand Wagoneer. On Monday, I received a call from the dealership stating they had good and bad news. The good news was the repair is covered by warranty but the bad news was it will be 2-3 months before they can get the part. The part is on back order and several people are waiting ahead of me. The said part is a valve body for the engine/transmission. The dealership also informed me that warranty will not cover a non-GM vehicle and that they will only pay for a rental up to 44.00/day. It’s not my fault the part is on back order, nor that enterprise gave me a non-GM vehicle. Also, I have a family of 6, which is why I have a Tahoe and I cannot be put in a small sedan. My children play sports, we have upcoming vacation, and quite frankly, I didn't pay 90,000.00 for a vehicle to be placed in a vehicle that I don’t feel is safe for my family. If I had wanted a sedan, I would have bought one. GM should take better care of their customers, especially when something is like this is not the customers fault!!! Eventually the truck regained normal power and we continued our journey back home to the St. Louis Missouri area. We had to stop several time out of fear to check engine oil, transmission fluid and engine temperature out of precautions. We were very stressed after this hard downshift that almost resulted in striking other vehicles on the interstate from the downshifting and rapid decrease in speed on the interstate. No warning light came on just the crazy shift indicator lights going haywire. The next day on [XXX] I was going to refuel and the transmission downshift again on me from 6 gear to first without warning. I took and dropped it off at a local shop for inspection and fixing. The problem was finally diagnosed as the transmission frame module and had to be replaced at a cost of over $900. I had to take it to another local shop here in the St. Louis Area who specializes in Transmission Repairs because my truck was out of warranty, CP Transmissions and Performance replaced the Frame Modular and told us to keep monitoring the status of recalls, but currently my 2016 did not fall under any. I am writing you now as I've seen several article indicating that my year has been noted as having this issue. I have the repair receipt on file and can share that information if needed. While on the same incline as incident #1, after removing my foot from the accelerator, the vehicle continued to accelerate, even during braking. Total time was approximately 2 sec. There was no damage to the vehicle, and no physical injury. I checked, and the floor mat was locked in place. Both incidents were reported to Volvo, and permission was given to extract data from the vehicle. The data was extracted at a Volvo service center. Unexpected Acceleration Incident #3 (after data extraction, before Volvo report). While turning right and into a parking space in front of a sidewalk and coming to a stop on an uphill incline, the vehicle quickly moved forward approximately 2 ft after my foot was removed from the brake. I was able to brake again to regain control and put the car in park. Unexpected Acceleration Incident #4 (after data extraction, before Volvo report). While traveling on an uphill incline, after removing my foot from the accelerator, the engine remained at a high enough RPM to sustain forward motion. Even after full-stop braking, the vehicle would move forward on its own, on an uphill incline, without depressing the accelerator. After incident #4, Volvo reported that they concluded that the vehicle was operating as designed at the time of their inspection and during a test drive, that no fault codes were found, and that no unintended acceleration was observed.Read more

CONCERN: Volvo cars’ sunroofs are negligently designed, allowing sunroof drain obstructions to flood the vehicle’s internal electronics. This results in catastrophic and simultaneous system failures, including brake loss, airbag failure, and disabled traction control—posing an extreme and imminent safety risk. SUMMARY OF EVENTS: While driving at speed, I received a “BRAKE FAILURE / PLEASE
... STOP SAFELY” warning, followed by “ABS / ESC” and “SRS Airbag” failure messages. This led to a sudden and dangerous loss of braking power and airbag system failure, forcing me to pull over in an emergency maneuver. Diagnostics confirmed that water leaked into the car’s critical electronic systems via the sunroof. Like other manufacturers, Volvo sunroofs degrade over time and require drainage. However, Volvo’s design is fatally flawed—clogged drains lead to direct water ingress into the electronics, causing total vehicle system failure while in motion. No warning is provided before catastrophic failure occurs. This is not an isolated case. An identical failure occurred in our second Volvo (2020 V90 Cross Country), owned and operated in California. This proves a systematic design defect—the same critical safety failure occurred in two separate vehicles, in different environments, under identical circumstances. I am deeply alarmed by Volvo’s negligent sunroof drainage design, which puts drivers and the public at severe risk of fatal system failures. I am reporting this to NHTSA because I believe this fundamental design flaw in Volvo sunroofs endangers lives and demands urgent regulatory action.
