On [XXX], I was returning to my home in Virginia from Maryland on an 8 lane highway with a 55mph speed limit when I had a very dangerous complete failure of my car. Simultaneously, the car started bucking back, the speed was decelerating regardless of my pressing the accelerator to keep my speed the same as traffic, and I received a flashing large red message that said "CHECK HYBRID SYSTEM - STOP SAFELY." Fortunately I was in the far right lane and was able to veer it over to a narrow shoulder. It was 8pm and getting dark. After a short while, the car lost all power so I was on a narrow shoulder with no lights flashing anymore. I was in this dark car with a lot of traffic speeding my with my 2 dogs. I became incredibly stressed about how we were going to get home. I contacted AAA, a police officer and Uber Pet who worked together in this unsafe situation on the side of the highway to get myself and my dogs home into the Uber vehicle with only the front door opening (no power). After several days of diagnostics the shop let us know our vehicle needed the following repairs: Bank One lifters replacement, head gasket service, and a water pump replacement, we ok'd the repairs. Although these components were replaced, we were informed on May 15, 2025, that the vehicle continues to misfire and now requires a camshaft replacement as well. The total cost of the repair has now risen from $5,182.16 to $13,549.79, which is a significant financial burden as the vehicle is just outside of warranty coverage. We are aware that there is an active recall covering similar issues for 2021–2024 Chevrolet Tahoes equipped with 6.2L engines. However, our 5.3L engine model is experiencing the same mechanical failures, including misfires, knocking noises, low RPM idling, and performance degradation. There is growing evidence that the valve lifter issues are not limited to the 6.2L engine variant. Our experience—and the experiences of others reported online—suggests that the 5.3L L84 engine may also suffer from the same design or manufacturing flaws. We respectfully urge NHTSA to investigate whether this issue warrants expanding the scope of the existing recall to include the 5.3L engine models, and we ask that GMC/General Motors be encouraged to provide assistance. Given the severity and known pattern of these issues, we believe GMC should cover at least 75% of the repair costs, or ideally, offer to perform the repair at no charge. The dealer put a new battery in it for $485 and we were told it was good to go. The next day, the car would not go into Reverse. Later we were called and told even though the part that had failed is in fact the same part in the open recall, that our VIN was not on the recall list and the cost to fix the vehicle would not be covered. In addition to all the headaches and money out of pocket (rental cars, tows, new battery) we now have to wait at least 4 - 6 weeks to receive the recalled part but since our VIN is not on the 2022 Tahoe recall list (even though it is the same part as the recall that failed), we are having to pay out of pocket for the repairs. I am submitting this report to formally document a severe safety-related failure involving my 2013 Volvo S60, VIN # [XXX] . This vehicle is part of the Volvo brand, widely recognized and marketed as among the safest in the world. Incident Summary: On [XXX], at approximately [XXX] in Norwalk, Connecticut, the vehicle experienced a catastrophic mechanical failure while in operation. The auxiliary
... (serpentine) belt suddenly failed, shredded itself, and wrapped around the timing pulley. Debris from the failed belt entered the timing belt housing — a supposedly sealed and protected compartment — causing the timing belt to jump. This resulted in an immediate loss of engine power while driving, placing me in a highly vulnerable position. Fortunately, I was not on a multi-lane highway or in fast-moving traffic. Had I been traveling on a busier route, the sudden loss of power could have led to a serious crash and potential injury. This failure represented a genuine threat to my safety. Further inspection revealed that: The lower crankshaft sprocket had skipped three teeth due to the intrusion of the auxiliary belt. The timing belt, although not broken, was off track. A new timing belt was installed in an attempt to restart the engine. The engine would crank but failed to start. A compression test revealed zero compression in all cylinders. The valve train was severely damaged (bent valves), and the engine sustained internal mechanical failure beyond repair. Conclusion and Context: This is not an isolated incident. Upon further research, I discovered that this failure — specifically the auxiliary belt disintegrating and compromising the timing belt system — is a known and well-documented problem in vehicles equipped with the B5254T12 Turbo engine. From my research, this is a huge problem with Tesla vehicles that thousands of individuals are loudly complaining about online and now in court cases in the country. I see there are thousands of reports to your organization about this, but it says you have only complaints from 2021 and later years. However, I have a 2019 and it's happening to me. And it's happening to every single model year out there, just from researching and talking to Tesla owners. This is a major safety hazard, plain and simple. I was inches of being in a rear-end collision, and drivers behind me are extremely upset and vengeful on the road after an incident like this (understandably!). Because of this I fear for my safety and well-being on the road using Tesla's cruise control features. Very disappointing. Above for the ADAS option, I wasn't able to select 2 options, but "automatic emergency braking" is also applicable here as that is what happened. I have taken my vehicle to 3 seperate mechanics (GMC, AAMCO and Independent). They all agree that my valve body inside the transmission must be replaced. The problem is that valve bodies are on a national back order and cannot be fullfilled for months, putting my transmission at risk for further failure. Dashcam footage from the right repeater camera confirms the presence of multiple “Do Not Enter” signs visible at the point of the turn. We have requested the Tesla Event Data Recorder (EDR) logs to confirm that Autopilot was active and that steering input came from the system, not the driver. I am reporting this as a serious safety concern related to Tesla’s Autopilot system and its decision-making at intersections. If this had occurred in a busier traffic environment, the result could have been far worse. This type of navigational or perception failure should be investigated immediately.
