90sWhile driving, the vehicle will suddenly display a warning message that says “Stop Safely Now”. When this message appears, the hybrid system overheats and the vehicle immediately loses power. The gasoline engine and electric motor both shut down without warning, even at highway speeds. I am forced to coast to the side of the road, which is extremely dangerous in traffic. This happens after
... the vehicle has been driven for a short period of time. The cooling fans appear to be running, but the hybrid electronics system overheats and triggers the shutdown safety mode. After waiting for the vehicle to cool down, it will restart, but the problem continues every time the car reaches normal driving temperature. This issue matches the symptoms covered under Ford’s previous safety actions and customer satisfaction programs for the 2005–2008 Ford Escape and Mercury Mariner Hybrid (hybrid electronics overheating and shutdown). The dealership states that no recall appears under my VIN, but the safety failure is identical. The vehicle becomes unsafe and inoperable with no advance warning beyond the brief “Stop Safely Now” message. This defect poses a serious risk of a crash, injury, or death due to the sudden loss of propulsion in traffic.Read more

I was driving to work when after running across a minor pothole I heard a clunking sound. It turns out it's the top cap for the front coil-over Bilstein shocks had broken allowing the shaft and the bump stop to protrude through the upper mounting plate. The coil did its job of keeping the vehicle suspended, but the shock piston has no ability to alleviate any stresses put upon the coil. Dealer sta
... tes that because there was an outside contribution, i.e. a pothole, which had been driven around and over many times before, wasn't warrantable. There was no more than standard bump and sway than when driving around as usual. There was no shudder as though I was going too fast for the defect in the road. I dont know if there was pre existing stress damage from a recent collision(opposite corner of truck) or if the was a defect with the part. The top cap is made out of aluminum rather than steel, which I find disturbing after learning this is not the first to have broken. This vehicle has only been off pavement in gravel driveways and parking lots. I hope you can investigate this thoroughly.Read more

While driving our 2022 Hyundai Tucson Limited with the 2.5L engine and about 39,000 miles, the check engine light suddenly came on. Right away the car started behaving strangely. The throttle response became jerky and uneven, and when idling the engine ran at a higher RPM than normal and sounded off. The vehicle also struggled to accelerate, which made it feel unsafe to drive. Thankfully this happ
... ened while we were not on the highway, but if it had occurred while merging or keeping up with traffic, it could have been dangerous. Even in city driving, the jerky response and slower acceleration created risk. What would normally be a safe turn across or into traffic could easily have turned into a slower, delayed turn and a potential accident. When we got home, I checked the code and it said it was a cylinder misfire on cylinder 3. The car had been regularly serviced at the dealer and had never shown signs of this issue before. Based on the symptoms, I suspect it may be related to the fuel injectors, but we haven’t been able to get the vehicle in for inspection because local dealers are backed up. One dealer even told us it was fine to keep driving as long as the check engine light wasn’t blinking, which feels concerning given the way the car is performing.Read more

1. The 3.5L V6 Engine (2GR-FE) failed due to the subsequent loss of lubrication caused by the rupture of the Oil Cooler Pipe (Rubber component), a manufacturing defect known under TSB T-SB-0201-11 (ZE2 Program). The failed engine is currently available for inspection by request at the independent service center before it is disposed of. 2. The defect causes a sudden and severe loss of engine oil.
... This creates a high probability of the engine seizing (losing all power) while driving at highway speeds, resulting in a complete loss of propulsion and placing occupants at severe risk of collision. 3. The failure was confirmed, not reproduced. The vehicle was initially diagnosed with misfire codes (P0300, P0301). New spark plugs and ignition coils were installed in an attempt to resolve the issue. As the misfire persisted, the mechanic performed a compression test, confirming catastrophic zero compression failure in cylinders 1 and 2. Professional diagnostics then revealed critical codes: P1603 (ECM Malfunction), P1604 (Startability Malfunction), and P1605 (Rough Idling). These codes confirm a severe internal engine issue, consistent with permanent damage sustained during the oil loss event caused by the ZE2 pipe defect in July 2024. 4. No, not physically inspected by the manufacturer. The manufacturer (Toyota Executive Office Case Manager) was provided with certified documentation of the failure, including the photo of the faulty pipe, maintenance records during our ownership, and the dealership purchase statement for the replacement part. Toyota refused to authorize a physical diagnostic inspection of the failed engine at a dealership before denying goodwill assistance. 5.Yes. As a result: 1. **Internal scoring and scratches (zadir)** have been detected within the engine, consistent with **metal shavings / debris** circulating in the oil system. 2. There is **metallic residue (shavings)** found in the oil pump, which strongly suggests that worn or damaged components (likely from a manufacturing defect) were never replaced during the recall. 3. Given the extraordinarily high mileage (≈ 250,000 miles), the continued use without the required recall repairs poses **significant safety risks** and potential for further engine failure or fire. 4. I was not provided with a proper remedy when recall notices were issued — in my case, the relevant parts were not replaced in a timely fashion (or at all), despite the recall campaigns being active, leading to the deterioration of the engine. --- ## Legal and Technical Basis ### Recalls Applicable to My Vehicle - NHTSA Recall **20V750** for the 2012 Kia Optima Hybrid: This is a recall related to engine defects, including risk of engine fire. I am reporting a safety-related issue on my 2021 Honda Accord Hybrid involving a failure of the heating and climate control system. The vehicle is unable to produce heat or properly defrost the windshield, which significantly reduces visibility in cold weather. This directly impacts safe operation during winter conditions and should be considered a safety concern rather than a comfort issue. A Hon
... da dealership diagnosed the problem as a heater core failure. The heater core itself is approximately $150, but the repair requires extensive disassembly and is estimated at nearly $2,000 due to labor. This type of failure should not occur on a vehicle this new. I recently purchased the car and do not believe this is normal wear or maintenance. Honda was contacted and denied goodwill assistance, stating they have no record of other complaints. I have since found similar reports from other Accord owners experiencing heating failures, particularly on hybrid models. This failure prevents windshield defogging and makes the vehicle unsafe for winter driving. I am documenting this issue with NHTSA so that the safety implications of heater failure on this model can be formally reviewed. I can provide the dealer diagnostic report, repair estimate, and Honda case number upon request. I drive 20+ miles each way to work every day. Much of this drive is on highways, and currently the bridge connecting Ohio and Kentucky is down to three lanes with no berm. On a windy road with low visibility and a steep hill near my home, the car completely lost power and acceleration without any warning. I wasn't able to drive it completely off the road before it came to a stop and due to the los
... s of power was unable to move into neutral to push it and the hazards would not work. Thank God that this didn't take place on the bridge or with my family in the car. I made the decision to exit the car while it was still on the road and find a safe area in the grass where I could call for help. When I exited the vehicle, it locked me out and the only way to get in was through the driver's side door, which would put me back on the road in traffic and at risk. The electrical failure prevented me from opening the other doors. The car's complete system failure put my safety/life at risk and could jeopardize the lives of others if it wasn't investigated. To make matters worse when the tow truck came via OnStar they weren't equipped to move a car stuck in park or drive due to the electronic gear shift so they drug it with the tires locked onto the bed. After several days of testing the electrical system, the dealership found a loose wire and informed me that it had been fixed. I picked up the car and after a mile or so, the check engine light came on. I contacted the dealership to come pick it up and that's where it stands at the moment. My intention was to pass this on to my boys when they start driving, but this experience will alter that plan.Read more
