We took delivery of the vehicle in March 2025. The vehicle has never been involved in any accident or collision during our possession. • In April 2026, we discovered that the passenger-side rear brake light was no longer functioning. • Upon inspection, dealership personnel informed me that a small crack—approximately one inch in length—in the plastic housing of the driver-side rear turn si
... gnal allegedly allowed moisture intrusion that ultimately caused the passenger-side brake light failure. • I was further advised that the repair would not be covered under warranty and was presented with an estimate totaling $1,377.54 (copy enclosed). • The dealership speculated that the crack may have resulted from a minor parking-lot incident. However, no evidence was provided to establish either the cause of the crack or when it occurred. My concern is not merely the cost of the repair, although the quoted amount appears disproportionate to the nature of the issue. More importantly, the explanation provided raises serious concerns regarding the integrity and durability of the rear lighting assembly itself.Read more

Depressing the accelerator pedal fully does not disengage the automatic braking, stripping the driver of vehicle control and exposing the vehicle to a severe risk of being rear-ended at high speed. Simultaneously, the vehicle experiences a persistent "In-Unit Telematics Malfunction" error on the dashboard, accompanied by rapid-fire mobile app notifications (14 alerts in a single day) stating "Critical Vehicle Malfunction: Vehicle cannot be driven." These communication module errors have persisted despite the dealership applying Mazda's latest software updates, indicating a critical network failure or hardware defect that compromises active safety systems.Read more

I am filing a safety complaint regarding my 2023 Kia Sportage Hybrid. The vehicle developed a “Check Hybrid System” warning and was diagnosed by a Kia dealership as having a failure involving the high-voltage hybrid system, specifically the MCU (Motor Control Unit). The dealership informed me that the vehicle may be unsafe to drive due to the possibility of fire risk associated with the high
... -voltage system. I was told technicians had to use specialized protective equipment while diagnosing the issue and that the diagnosis process took nearly a week. The repair estimate was approximately $5,000, in addition to approximately $750 charged for diagnostics alone. The vehicle is reportedly out of warranty due to mileage, even though this appears to involve a major hybrid system component and a potential safety defect. This concerns me because: - the vehicle is a relatively new 2023 model, - the failure involves the high-voltage hybrid system, - Kia advised the vehicle may catch fire, - and independent mechanics generally cannot service or repair these proprietary hybrid components. I believe this may represent a broader safety issue or defect involving the hybrid system or MCU components in Kia hybrid vehicles. I am requesting that this matter be investigated for possible safety defects, recall eligibility, or manufacturer responsibility.Read more

Vehicle: 2016 Toyota RAV4 Color: Factory Super White / Blizzard Pearl (whichever applies) I am filing a complaint regarding a well-known latent manufacturing defect involving peeling factory paint on my 2016 Toyota RAV4. The original factory-applied white paint is peeling and delaminating from the exterior metal body panels under normal use and maintenance conditions. The paint failure appears un
... related to any accident, impact, misuse, neglect, or environmental damage. The failed component is the factory-applied exterior paint and coating system on the vehicle’s metal body panels. The affected areas are available for inspection upon request. The paint is separating from the primer/base layer and peeling off in sheets, exposing underlying surfaces and creating risk of corrosion and reduced vehicle value. Toyota has publicly acknowledged widespread issues involving “Super White (040)” and “Blizzard Pearl (070)” paint on numerous Toyota models, including RAV4 vehicles, through Customer Support Program ZKG and multiple Technical Service Bulletins. Toyota documents specifically state the issue involves paint adhesion failure during the manufacturing process causing peeling on exterior metal body panels. This issue creates safety concerns because exposed metal surfaces may eventually corrode or rust, reducing long-term structural integrity and potentially creating visibility distractions while driving as paint continues to separate and peel unexpectedly. Large peeling sections can also detach while driving and create roadway debris hazards for other motorists. The condition has been widely reported by consumers across online forums, complaint databases, and dealerships for many years. Numerous owners of white Toyota vehicles have reported identical failures involving factory paint peeling and delamination. The problem has been visually confirmed by dealership personnel / independent body shop personnel (modify if applicable). And they have refused to cover it at allRead more

I am submitting this updated complaint regarding ongoing issues with Safety Recall 69PZ (Passenger Occupant Detection System) on my 2024 Volkswagen Atlas Cross Sport. Despite multiple repair attempts at Gossett Volkswagen in Memphis, TN, the recall remains incomplete, and the vehicle now exhibits additional safety‑system failures that began immediately after dealership service. I purchased the
... vehicle from Gossett Volkswagen and have had all warranty and recall work performed exclusively at this dealership. The first recall attempt occurred on April 20, 2026. The second attempt occurred on April 29, 2026, when the dealership picked up and returned my vehicle; this visit is confirmed by home security footage and text messages, but no repair order was issued. A third attempt on May 4, 2026 was also unsuccessful. On May 8, 2026, the dealership produced a repair order stating “SUCCESSFULLY COMPLETED RECALL,” but later acknowledged in writing that the recall was not complete. On May 18, 2026, another repair order inaccurately stated that I “declined” the recall. Immediately after the [XXX] visit, my vehicle displayed new safety‑system errors, including Drivetrain Error, Emergency Assist Error, and Front Assist Error. These issues were not present before the visit and were documented on video. On May 20, 2026, during a recorded call, the Service Manager stated that the dealership would not schedule, diagnose, or repair any issues on my vehicle unless I authorized the recall repair, even though the new defects are unrelated to the recall and should be covered under warranty. He also stated the dealership is a “private company” and can refuse service. As a [XXX] who relies on custom hand controls, these unresolved safety issues and the dealership’s refusal to perform warranty repairs have left me with an unsafe and unreliable vehicle. I am requesting NHTSA’s review of the recall handling, documentation inconsistencies, and the resulting safety concerns. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (Read more

The vehicle experienced a sudden unprovoked, catastrophic, and total electronic and mechanical failure of the primary braking and ADAS. Driving at expressway speeds, the vehicle immediately lost all power brake assistance, threw immediate multi-system critical alerts on the instrument cluster, and became instantly undrivable. The vehicle is available for inspection at a local licensed Lincoln deal
... ership service center. The failure occurred completely without warning while navigating heavy traffic at active highway speed on the expressway. The instant loss of power braking functionality dramatically extended the vehicle's stopping distance and significantly reduced control of a SUV in a high-speed environment. This sudden, unmanaged deceleration created an immediate, life-threatening risk of a high-speed rear-end or multi-vehicle pileup, placing the occupant of the vehicle and all surrounding motorists in extreme danger of severe injury or death. The driver was able to bring the vehicle to a stop in a precariously exposed location on a very narrow shoulder when one became accessible. Miraculously, the vehicle was not struck by fast-moving trucks while waiting for a tow-truck. Yes. The local authorized Lincoln dealership service technician team has confirmed the catastrophic code failures. Due to the severity and complexity of the total system shutdown, the local dealership escalated the vehicle to Lincoln factory corporate engineers. However, the root cause of the catastrophic failure was never identified and the technicians and engineers have given up further attempts. There were absolutely zero pre-existing symptoms, warning lamps, soft codes, or performance degradation prior to the exact moment of the failure. The system failure was instant. The instrument cluster simultaneously illuminated multiple critical alerts, including the primary braking system warning, ABS fault, and ESC failure alerts, accompanied by an immediate audible safety chime.Read more

I started my Jeep in my driveway and a warning quickly flashed onto the center screen just before the Check Engine Light illuminated. I attempted to test drive it. The vehicle started cutting out before the engine started racing. I was able to stop at the end of my street. I made a U-Turn to return to my home and the Jeep started bucking and jerking wildly as I was attempting to stop in front of m
... y house. It was at that time it stopped running and locked itself in PARK. I called a tow to the dealer. The tow driver was also unable to move the vehicle as it was still locked in PARK. He was able to drag it onto the flatbed. Upon inspection at the dealer, they stated that the hybrid battery had failed and needs to be replaced. This was a dangerous enough situation but had this of happened in traffic or worse yet on the highway, it would have made for a much more dangerous situation where someone could have been killed. This is not the first time I've had drivability and loss of motive power issues with this vehicle. The last time a few months ago I brought it to the dealer's attention and explained the problem and the danger. The dealer advised me that the software after the 68C Recall will reduce power to protect the hybrid battery. It seems that Jeep is trying to save money rather than do the right thing and buy back these dangerous vehicles. Jeep customer service basically told me that it's my problem. I just want the Jeep fixed or bought back. It is not a safe vehicle to drive without fear of being smashed into from the rear. The Jeep is currently at the dealer waiting for a replacement hybrid battery. It is my understanding that the replacement battery will be the exact same as the defective one and runs the same chance of being defective and dangerous. How is this even allowed to happen?Read more

The failed/malfunctioning components involve the rear backup camera system and related electronic driver assistance/safety systems in this 2016 Ford Flex, including the blind spot warning system and parking collision warning features. The condition is intermittent but ongoing and the vehicle is available for inspection upon request. The backup camera intermittently goes blank, freezes, displays im
... properly, or becomes unavailable while the vehicle is in reverse. At times, the blind spot warning system and parking collision warning features also intermittently fail or become unavailable. These failures create a significant safety risk by impairing rear visibility and reducing driver awareness of nearby vehicles, objects, or pedestrians during backing and parking maneuvers. The problem has been occurring for more than one year. The issue has been reproduced and confirmed by the dealership, which has stated that the vehicle is subject to an open safety recall. However, the dealership reports that parts and/or the final repair remedy are still unavailable, and no repair has been completed despite repeated attempts to obtain recall service. The vehicle has been inspected by the dealership/manufacturer representative in connection with the recall condition. No police or insurance inspection was involved because no crash has yet occurred. Prior to and during the failures, warning messages and symptoms intermittently appear, including loss of the camera display and temporary unavailability of driver assistance features. The failures may occur without warning and have been recurring for over one year. The unresolved condition affects the safe operation, usability, and value of the vehicle. Backup cameras and related driver assistance systems are important federally regulated safety features, and the manufacturer has not remedied this safety recall within a reasonable period of time.Read more

The vehicle began showing serious safety related symptoms before the engine failed. These issues caused loss of speed information, loss of vehicle status, and loss of navigation visibility, creating un
... safe driving conditions. Despite these warning signs, the engine later failed suddenly at around 71,399 miles with no warning lights, no oil pressure alerts, and no overheating indicators. The sudden loss of power created a highway safety hazard due to loss of propulsion and reduced steering and braking assist. The manufacturer and dealership also provided conflicting and misleading information about warranty coverage, including a claim of “no warranty on engine,” which contradicts the official warranty coverage documents. This has delayed proper repair of a safety related defect.Read more

Traffic has to flow around you like water around a rock in a stream while you try to use what’s left of your forward momentum to pull to the side of the road. Absolutely terrifying!!! The dash has also gone completely blank during some of these episodes. Nothing changes the situation: pressing “D”, shifting the paddles, nothing gets the vehicle back in gear. We have video taped these events twice, and provided those videos to our Hyundai Service Center, and to the Hyundai National Consumer Affairs team. They claim that they can’t reproduce it on demand, so they have done NOTHING. We are scared to drive the car. 2) As you start accelerating from a stop, the powertrain will miss the next gear. The engine revs, but you are not in gear, then it will catch a gear and continue to accelerate. These events happen often, but seemingly randomly. After several episodes last year, and no Service Center resolution, we reached out to the Hyundai National Consumer Affairs team, and have continued to be in contact with them, but they have not helped in any way to resolve this. They have assigned numerous Case numbers and Reps, and have emailed me over 20 times instructing me that I should go AGAIN to the Service Center, and then that they have no updates. We are terrified to drive this vehicle, and it is the only one we have. With these issues, we are in danger every time we leave home for our Dr. appointments and necessary trips.Read more

May 11, 2026 approx 9:35am. I was in a flat parking area at a gas station waiting for a friend. There were 2 other cars parked near me. No one was in them. Neither were moving. I backed out of my space into a large, empty, open area to turn my car around to have a better view. Because the audi pre-sense chronically malfunctions, I always turn it off before I drive. But I left it on this t
... ime because I was only driving about 15 feet in a parking area. I backed out, then stepped on the brake to turn the wheel. While I was AT A FULL STOP, the seatbelt motor started up loudly and grabbed my chest with the seatbelt, slamming me back against the seat. No one was there. No cars were moving. I was at a stop. When the seatbelt let me go, I noticed a red lighted image of some sort on the screen. I believe there was a red star-shaped image and some text indicating a collision. Then the image disappeared. I turned off pre-sense and backed into my parking space. It was painful and frightening - especially as I have osteoporosis. This car has previously slammed the brakes on an empty road while I was driving 45-50 mph. Dealer says there are no codes or errors and I need to clean the sensors. I bought it new. Likewise, the rear cross-traffic, pedestrian detection etc. rarely works and sometimes fires for no reason. If the brakes slam when there's a car behind me, I'll be rear-ended. If the seatbelt slams while I'm on the road, I'll certainly swerve from the discomfort and being startled. Very dangerous. NOTE: My previous report seems to be removed from this website. Bad idea.Read more

own a 2019 Infiniti Q50 3.0t Sport with approximately 80,000 miles. The vehicle began experiencing engine knocking/ticking noises, rough idle, hesitation under acceleration, shaking, and intermittent misfire symptoms. The issue continued even after replacing spark plugs and ignition components. The dealership/mechanic reported possible internal engine damage and metal shavings in the oil, indicat
... ing potential failure of internal engine components. The vehicle has been regularly maintained with oil changes completed on schedule. This appears to be a widespread issue affecting many Infiniti and Nissan vehicles equipped with the VR30 3.0 twin-turbo engine. Numerous owners online report similar symptoms including engine knocking, turbo failure, metal contamination, loss of power, and premature engine failure at relatively low mileage. The issue creates a serious safety concern because the vehicle can lose power, hesitate during acceleration, or potentially fail while driving in traffic or on highways. I am requesting that NHTSA investigate repeated engine failures involving the VR30 engine platform used in Infiniti Q50/Q60 and related vehicles.”Read more

This has happened multiple times. When driving the engine shuts off and an error message appears to shift into neutral or park and restart the engine. Brought the car to liberty Kia in Ramsey NJ and they said found no errors in the computer and test drove and could not recreate. My odometer did not change from drop off to pick up so I can tell it was not test driven.

The vehicle's head gasket failed prematurely, resulting in coolant intrusion into the engine cylinders and combustion chamber. This caused the engine to unexpectedly overheat while driving at highway speeds, creating a severe safety hazard and loss of acceleration in active traffic. Additionally, it had a very rough idle when starting.

While driving on the interstate, my sunroof suddenly exploded and glass shattered from the moveable portion of the sunroof. It sounded like a shotgun or a very loud pop. There was nothing above me on the road (i.e. no overpasses, etc) and no vehicles in front of me at the time. The sunshade was closed which saved me from pieces of glass falling on me, in my eyes, and potentially cutting me. It
