The front brake master cylinder is leaking brake fluid slowly. This was first recognized by me in my garage ~8 weeks ago Included is a photo from of a leak-stain on the exhaust header that I presumed was from servicing in the dealership showroom as taken by my transporter. In retrospect, this appears to align with the brake fluid leak. The issue is continuing to this day. There is noticeable fluid
... loss and corrosion around the C-type spring clamp where the separate, remote reservoir connects into the master cylinder input. This does not appear to be where the leak is emanating from however. The leak appears to be coming from the master cylinder seal around the plunger mechanism. The issue was first noticed as I was preparing to wash the bike and witnessed a small drip on my fuel tank where the handlebars were turned left with the brake side grip in the 'high side' position. The drip was evidenced on the pivot bolt nut at the bottom of the master cylinder. The bike is a MY2023 purchased as a new, untitled motorcycle from an authorized Triumph motorcycle dealership by me in Nov-2025. My bike has ~700 miles now. Logging here as a safety concern. I've contacted both my local dealership and Triumph After-sales as well. The goal here is to document this issue with the NHTSA to be certain a) there isn't a prolific case of this leakage that I'm unaware of and b) to create awareness with NHTSA of a potential braking safety concern due to the leak preceding me seeking any repairs. I have only ridden my bike at low speeds in my neighborhood private (dead end) street until I can be assured this isn't a safety concern and seek a proper repair from a Triumph-authorized repair center free of charge under the OEM warranty. Pic's included are post- wiping to prevent drips onto my fuel tank.Read more

While uninstalling a Chicco Fit360 ClearTex rotating convertible car seat, we discovered that the load-bearing webbing connecting the car seat base to a lower-anchor connector had completely separated. The car seat had not been involved in any crash or impact, and nothing struck or contacted the seat during removal. The separation was not visible during normal use. The seat had become loose, but
... we did not know that the lower-anchor webbing itself had failed. Because this webbing secured the base to the vehicle’s lower anchor, the failure severely compromised the installation while our child had been riding in the seat. In a collision, the restraint might not have remained secured to the vehicle. The car seat has been removed from use. It has not yet been inspected by the manufacturer, police, insurer, or another party. The seat, separated webbing, photographs, and video have been preserved and are available for inspection upon request. We purchased two identical seats and are concerned that other units may experience the same safety-critical failure. We request investigation as a potential safety defect.Read more

I am extremely disappointed with the way Ford has handled the recall on the rearview camera for my 2016 Ford Escape. I received the recall notice about a year ago. Six months ago, I contacted Chapman Ford and was told the replacement parts were not yet available and that I would be notified when they came in. Now, a full year later, my backup camera still does not work. This is a safety issue--not
... a convenience feature. When I called Chapman Ford again today, I was told the part was still unavailable for the Escape and was only being supplied for Mustangs. They gave me Ford Corporate's customer service number. Ford Corporate assured me that they would resolve the issue. They contacted Chapman Ford, spoke with Valerie, and told me everything had been arranged. I was told my appointment was set, the repair would be completed at no charge, and the problem would be taken care of. About an hour later, Chapman Ford called me back and said they actually did not have the part after all, could not perform the repair, and there was nothing they could do. After waiting an entire year for a safety recall, this is simply unacceptable. Ford sent out a recall notice acknowledging there was a safety problem, yet they still cannot provide the parts needed to fix it. Owners are left driving vehicles with defective backup cameras and no clear timeline for when the repair will actually happen. Ford should be ashamed of the way this recall has been handled. If you're going to issue a safety recall, you should also have a plan to provide the parts necessary to fix the vehicles within a reasonable amount of time. Customers deserve better communication, better follow-through, and most importantly, a prompt resolution to safety-related defects.Read more

Summary: I am reporting two significant defects on my 2025 Nissan Sentra that pose safety risks. 1. Suspension/Steering: During low-speed maneuvers (approx. 20mph) such as navigating roundabouts, there is a persistent mechanical creaking/squealing noise from the front passenger side. The dealership technician verified the noise but provided a physically impossible diagnosis, claiming "tires were
... slipping," despite the vehicle being brand new with tires in perfect condition. This suggests a defect in the control arm bushings or strut mounts which affects steering predictability. 2. Power Window/Electrical: The driver-side window produces a loud, piercing screech when operated. More critically, the window switch has experienced intermittent functional failure where it becomes unresponsive. This is a safety hazard as it could prevent emergency egress or cabin ventilation. Manufacturer Response: > Both issues are documented and have been reported to Nissan Consumer Affairs (Case #56515863). However, the manufacturer stated they have "no solution" and directed me to argue with the dealership. No technical specialist has been assigned to inspect these safety-related mechanical and electrical failures. The vehicle is available for inspection.Read more

I am writing to formally request a review of my vehicle for inclusion in NHTSA Recall 21V-635, issued May 11, 2022, concerning fuel leaks at the fuel tank flange assembly. My vehicle is a 2012 Land Rover LR4, VIN [XXX] . Although my local Land Rover dealer has informed me that my VIN is not currently included in the recall population, my vehicle is experiencing the exact condition described in th
... e recall notice — a fuel leak originating from the fuel outlet flange mounted on the fuel tank. The recall letter for 21V-635 states that all Land Rover LR4 vehicles model years 2010–2016 are included. My vehicle falls squarely within that model year range. Given this, and the fact that my vehicle is exhibiting the precise defect identified in the recall, I am concerned that my VIN may have been inadvertently excluded from the recall population. This issue presents a serious safety concern due to the risk of fire associated with fuel leakage. The symptoms and failure location match the recall description. I respectfully request that NHTSA review my case to determine whether: My VIN was mistakenly omitted from the recall population, or The recall population should be expanded or corrected to include my vehicle. I am prepared to provide photographs, repair estimates, diagnostic reports, or inspection documentation confirming the fuel leak at the flange assembly upon request. Because this defect involves fuel leakage and a significant potential fire hazard, I believe it warrants careful review to ensure all similarly affected vehicles are properly covered. Thank you for your time and consideration. I appreciate your assistance in reviewing this matter and advising me on the appropriate next steps. I have experienced two near-fatal incidents involving a sudden, uncommanded loss of motive power while traveling at highway speeds (approx. 70 MPH). On February 11, 2026, the vehicle entered "Limp Home Mode" without warning, dropping speed to 60 MPH and 2000 RPM in heavy traffic. This caused an immediate and dangerous speed differential, nearly resulting in a high-speed rear-end collision. Simulta
... neously, the AC system disabled, indicating a severe engine protection event. Manufacturer Negligence: The vehicle was taken to Courtesy Hyundai of Tampa twice for this issue (Code P0087). They performed a "Knock Sensor" replacement and cleared data points, assuring me the vehicle was safe. The failure recurred immediately. I subsequently took the vehicle to Brandon Hyundai on February 13, 2026. Despite my reports of near-fatal highway failures, Brandon Hyundai was instructed by Hyundai Corporate to merely repeat the "Knock Sensor" replacement—a repair that had already failed to resolve the dangerous condition. Critical Safety Admission: On 02/23/2026, Brandon Hyundai Service Advisor Abel Gomez explicitly admitted that this repair is "not a fix, it is a temporary thing until it shows again," yet stated they must follow Corporate’s directive to release the car. Requested Action: Hyundai is knowingly releasing a vehicle with a recurring, life-threatening defect (P0087/Sudden Power Loss) by performing "band-aid" sensor swaps instead of addressing the high-pressure fuel system or internal engine failure. This practice is a direct violation of the Safety Act. I request an immediate investigation into Hyundai’s refusal to perform permanent repairs on GDI engines displaying these catastrophic failure codes.Read more

While driving on the freeway in a construction zone, I smelled a burning oil smell. I considered the possibility it was in relation to road repairs being done (roads being repaved, ie tar resurfacing). I was cut off by another vehicle and depressed my brakes and I saw a poof of smoke over the front passenger area near tire. I noticed all of my gauges were in the normal ranges (temperature, batt
... ery voltage, etc.) In less than a minute my truck went into limp mode and advised it had a DEF system issue and it would be limited to a set MPH and a set number of miles. Check engine light came on and another driver alerted me that the underneath of my vehicle was on fire. I pulled over to the side of the highway and my truck became engulfed in flames in. Truck is under factory warranty, unmodified, never been in an accident and has had all recommended services performed at local dealership. There was a significant amount of traffic and luckily I did not collide with anyone or set neighboring dry grass field on fire. Fire department responded and extinguished the fire, but it is a total loss. Insurance company has been notified, and vehicle will be available for inspection by appropriate agencies. While local law enforcement did respond, they did not take a report because I didn't hit anything, and the only property damage was my own. (There is documentation of the event, but just dispatch notes.) Prior to the situation above the truck had been at operating temperature for at least 20 minutes and I had traveled for about 20 miles on the highway. The total distance traveled from when I smelled the burning smell was maybe 1.5 miles.Read more
