My oil light came on around 91,000 miles. Kia is known for this issue being an oil pressure sensor issue. Happens around 80,000-100,000 miles. Its a pretty common issue and the labor for a $80 part costs up to $2,000.

In August 2025 my 2012 Jeep Grand Cherokee Laredo lost engine power and stalled while driving 15 mph. There was no warning. I was able to restart the car and drive. Then while driving in traffic at 45 mph on 18 Oct 2025, my Jeep abruptly lost engine power and stopped, putting my safety and the safety of my passenger at great risk as I had several cars behind me. Again, the Jeep stalled without
... warning. I was able to put my emergency warning lights on, then after 30 seconds, I put the car in park and restarted the engine with no problem. The Jeep drove normally. This unexpected loss of power while driving at speed in traffic on Hwy 431 could have been catastrophic. This incident appears to be linked to two previous Fuel Pump Relay recalls, the most recent being V62/NHTSA 19V-813. My Jeep was repaired by the Jeep dealer under both of these recalls which were prompted by no-start conditions and ENGINE STALL CONDITIONS. It appears the FCA repair has been ineffective as my Jeep has experienced two engine stalls while driving and does have intermittant problems with start up. This most recent problem has not yet been confirmed by a dealer as the Jeep Dealer cannot see my vehicle for another 2 weeks, which puts my safety at risk. There were no warning lamps or messages before the abrupt engine stalls while driving. However, for the past 6 months, I had had intermittant instances of the car not starting immediately, particularly in the morning. I would turn the key fob a few times before my Jeep would start. But it always started.Read more

With the car stopped & the transmission in the Drive or Reverse position with foot off the brake pedal: 1. Pushing the Power button once to turn the car off results in the transmission automatically shifting from Drive or Reverse to Neutral without automatically setting the Parking Brake. 2. If the car's front-end is on a slope pointed up-slo
... pe the car will freely roll backwards. If the car's front-end is pointed down slope the car will freely roll forward. The above sequence is very dangerous when on a steep slope. This problem was first experienced in a Home Depot parking lot that was uneven. Parked with the nose up-slope in a small depression. The Power button was pushed while the transmission was still in the Drive position & the car immediately started rolling backwards. Fortunately, nobody was behind the car. I quickly pushed the brake pedal to stop the rolling. The proper way to shut off the car, of course, is to put the transmission in the Park position first, which automatically engages the Parking brake, confirmed by a light on the manual Parking Brake switch. I also own a 2007 Prius, bout new. That car has a paddle trans lever, Parking Brake & Power switches. The parking brake is all mechanical with a parking brake pedal. Whereas the 2025 RAV4 does NOT have a parking brake pedal -- it has a switch. Pushing the Prius' Power Switch (off) automatically locks the transmission regardless of the trans mode (D, R, N) such that the front wheels lock& the car will not move, regardless of the parking brake setting. Thus, the Prius is intrinsically safe with locked front wheels unable to turn on a slope regardless of the parking brake setting. I discussed this with Toyota & my dealer. The dealer reported another customer had also complained about this problem. Toyota quickly closed my complaint #251006003193 with no explanation stated. See the attached PDF doc.Read more

Dear NHTSA, I am writing to formally report a serious safety incident involving my 2016 Kia Optima Hybrid, which caught fire and was completely destroyed on October 5, 2025. Incident Summary: While driving on a highway at approximately 65 mph, I noticed that the vehicle was not entering Hybrid Mode, which is abnormal for its typical operation. I reduced my speed to 45 mph, then down to 35 mph a
... s I began to observe smoke coming from the vehicle. At no point did the vehicle show signs of overheating, and there was no Check Engine Light until shortly before I was able to safely pull over. Within moments of stopping the vehicle and exiting with my passenger, the vehicle was engulfed in flames and ultimately burned to the ground. Fortunately, both occupants escaped unharmed. The vehicle has been inspected by my insurance provider; however, I have been waiting over two weeks for Kia to inspect the vehicle or provide any official response regarding a possible cause. This delay is concerning, especially given the severity of the incident and the potential safety risk to other drivers. Vehicle Details: Make/Model: 2016 Kia Optima Hybrid VIN: [XXX] Mileage: 54,300 Date of Fire: October 5, 2025 Location: US-60 NONCARD & MP-118 Morristown, AZ I am reporting this incident in hopes that NHTSA will investigate whether this is an isolated case or potentially part of a larger safety defect within this vehicle model or hybrid system. The lack of adequate warning and the total loss due to fire raises serious safety concerns. Please let me know if any additional information or documentation is required to support an investigation. Thank you for your attention to this matter. Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Incident date: May 24, 2025
There is a wobbling sensation on my Highlander during braking at highway speeds. There is no such feeling while driving around city. It appeared after my trip on highway in heavy rain. There was no traffic, and I were using cruise control. Suddenly, doing nothing from my side but steering, I started to feel heavy break impacts, which scared me (I was with my family). I switched off cruise control,
... and those break impacts disappeared. Then switched it again, and I started to feel the same, thus I switched off cruise control and drove the rest of rain area with my foot on accelerator pedal. After that I started to feel wobbling during braking at highway speeds. It seems - more on the right side. Maybe there was more water layer under the right side of the car, and the Electronic Stability Control tried to hold car on the straight line, or camera detect water on the windshield as an obstacle and applied breaks from time to time, which might case warped rotors. To my understanding it has to be warranty issue, as Toyota manual said that driving during heavy rain can cause unproper action of cruise control, but it does not say it can damage the car or warp the rotors.Read more

I’m writing this message out of frustration, exhaustion, and genuine concern for public safety. Over the past months, I’ve been renting and driving multiple cars from different brands and models — Toyota, Subaru, Nissan, and now a 2023 Ford Mustang Mach-E and all of them show the same dangerous behavior: sudden phantom braking for no reason, traction control malfunctioning, throwing the ca
... r from side to side, unstable handling, especially on highways or during light rain. This is not an isolated case it’s happening across several brands and systems, which makes me question what is really going on. Why are drivers being forced to deal with unreliable technology that clearly isn’t ready for real-world conditions? Are these systems being tested using real drivers as data sources to “teach” self-driving cars how to handle special situations? Because that’s what it feels like we are being used without consent, forced to correct constant software mistakes that could cost lives. This situation has destroyed my professional and financial stability. I work as a rideshare driver (Uber and Lyft), and passengers have reported me as “unsafe” because the car suddenly brakes or moves erratically something completely out of my control. I’ve lost access to stable work, fell behind on my car payments, and ended up returning my own vehicle because of all the expenses and stress trying to fix something that was never broken in the first place. And I have to ask: Why did NHTSA approve a regulation that prevents drivers from fully turning off these driver assistance systems? In what world does it make sense for a safety-critical technology to have no emergency off switch? In any other context like electricity we have breakers to stop the system when it becomes unsafe. Cars should be no different. I’ve lost almost everything I worked for because of this. Please, take this seriously. People’s lives, jobs, and mental health are being destroyed by technology that we can’t control.Read more

While driving on a dry, clearly marked two-lane highway in Montana, I had Lane Tracing Assist and Active Lane Keep/Track Assist turned on. My hands were on the steering wheel, speed was within the limit, and visibility was clear. The truck unexpectedly steered itself out of the lane toward the right-side guard rail. There were no audible or visual lane-departure alerts, no “steering assist cance
... led” message, and the Pre-Collision System did not apply braking before impact. The vehicle struck the guard rail. The system failed on multiple safety layers — no lane warning, no steering correction, and no automatic braking — under normal conditions (dry pavement, well-painted lane lines, daylight). I believe this represents a malfunction or defect in the driver-assist software or sensor calibration for Toyota Safety Sense 3.0. I request NHTSA investigate similar incidents for 2024–2025 Toyota Tacoma models equipped with LTA/LDA and Pre-Collision System to determine whether a systemic defect exists.Read more

I am writing to formally document a series of serious safety incidents and ongoing concerns regarding my 2024 Toyota Tundra, which I believe reflect a critical design flaw and a failure by Toyota to act in accordance with its obligations to customer safety and product reliability. In August 2025, while driving on the interstate with my minor child in the vehicle, I experienced a sudden and unexpe
... cted engine malfunction. The failure required me to swerve off the road at high speed, nearly resulting in a serious collision. This incident placed both my life and my child's life in immediate danger. The vehicle was towed via Toyota’s SOS service to Green’s Toyota of Lexington, KY. Upon evaluation, I was informed by Service Manager Justin that the issue was consistent with the engine failures that led to a recall of the 2022 and 2023 Toyota Tundra models. However, because my 2024 model has not yet been included in the recall—despite sharing the same apparent defect—I was told that Toyota would rebuild the engine rather than replace it. I expressed strong concerns regarding the safety and long-term reliability of a rebuilt engine in a new vehicle. I was reassured that a rebuilt engine would be “just as good,” despite clear evidence to the contrary based on subsequent events. As a result of this dangerous failure and Toyota’s inadequate response, I submitted a formal buyback request to Toyota Customer Care, citing serious and ongoing safety concerns. The dealership nevertheless indicated I would likely be denied a buyback, which I found deeply troubling given the known defect and the high-risk circumstances involved. During the initial repair, I visited the dealership to retrieve personal belongings and observed the vehicle in a completely dismantled state. The cab had been removed, and the engine was fully disassembled with parts scattered on the ground. This unprofessional and chaotic environment caused further loss of confidence in the repair quality and safety of thRead more

I had to take 2 days almost to fix it in the middle of the road, since yesterday morning. They also began locking up suddenly on the front end, the wheel bearing, CV Joint, Tie Rods, CV Axles, CV Boots, and sway bars began failing out of nowhere as well. The brakes
... may or may not be able to be replicated, as they are partially fixed and yes my entire front end can be replicated. The danger is my brakes locked up as I was driving, in the middle of the road at that. I have not been able to get the front end fixed as it is expensive, however the danger of this is the tires falling off or the front end failing all together. ALL of the other people who drive these that I have talked to and met have had the same problem and yes they are factory parts! Even the people who have only 30 to 60000 miles on their 2011 Ford Flex Titaniums have these issues! Not only this but also the sensors go bad almost immediately off the lot, which can cause the car to brake without warning, stop running out of nowhere, make the power steering module fail, make your vehicle stutter because ALL of the sensors are bad. My battery died while the vehicle was parked. I needed to physically move the vehicle but could not because the electronic button shifter would not allow me to put the transmission in Neutral. Safety concerns were that the vehicle was blocking traffic at a business and no documentation by Honda of how to fix or even shift the vehicle into Neutral. I replaced the battery myself, but this is a well documented issue with electronic shifters, that will never make a dealership or service departments records or statistics!
