Toyota requires the ABS system to be recalibrated. The only way is by going to a Toyota dealership and having the technicians use the fancy expensive diagnostic tools and computer software. I got my brakes changed by a reputable shop. However, I did not know whether they knew this or not. Now my front passenger pad is always rubbing on the rotor. These issues may cause high fuel consumption, strai
... n on the transmission and driveline components, and even the possibility of fire. It would be really helpful if Toyota built vehicles that can easily be maintained as well as less complicated systems requiring owners to pay dealership prices for maintenance that should allow the owner to address it or any mechanic shops.Read more

The original factory engine is malfunctioning, causing excess oil consumption -- at or over one quart consumption per 1000 miles -- believed to be caused by fouled piston rings. It is a known defect for this and similar KIA engines. Issues and remedies outlined in KIA Technical Service Bulletin 222. The vehicle and engine are available for inspection upon request. Our safety and the safety of o
... thers is continuously at risk in the event of engine failure and/or vehicle breakdown in city or freeway traffic, and in our rural location. Safety is also a factor in the event of vehicle breakdown which would put vulnerable passengers (five year old granddaughter, etc.) in danger if such event occurred in traffic or during weather extremes. The oil consumption problem has been confirmed by Butler KIA. The engine was inspected by the manufacturer via Butler KIA only in the form of three 1000-mile oil consumption tests from May through October 2025. Dealership personnel began the first test with an oil change, and then inspected/verified excess oil consumption at each 1000 mile interval. An oil warning light illuminated between regular (4489 historical average interval vs 7500 manufacturer-specified intervals) oil change intervals, and immediate inspection revealed the engine oil was 2.5 quarts low. Oil was added and owner tracking was commenced, verifying excess oil consumption.Read more

Contacted the dealer, they said that "tires blow" at any time, new or used. Instructed us to report it to Hyundai; the insurance company did that, and Hyundai stated there is no known issue with the vehicle. Tire make is Kumho brand, solus TA3 to the best of my knowledge. A police report was filed, and it was deemed related to a malfunction; the driver was not cited. Speed, drugs, and alcohol were not a contributing factor.Read more

Issue started with hearing a clunking noise from the front of the car, had the dealer inspect at 37,000 miles. 2019 Ford Escape 1.5L EcoBoost. 74,000 miles. Check engine light came on out of nowhere. Had auto shop read the code and it was a misfire of cylinder 3 and needed ignition coil and spark plug replaced. A few days later the message "High Engine Temperature, Stop Safely" came on and the engine temperature shot up to High. Pulled over and let it cool down. Started driving it again and it shot back up
... to High again. Took the car to a mechanic who did a compression test and said that the coolant ran green which meant the coolant is infiltrating the cylinders. Per another mechanic this is a known problem with 2019 Ford Escapes that the engine is a open deck block that allows the cylinders to move and be unstable. I received a new engine. Went in 8/15 - 9/10. Truck was dead on 9/22 when State Farm sent out a guy to replace the windshield that came back broken from the dealership. I had put in a new battery on 4/17, before I even knew about the recall. I got the battery out and replaced under warranty from AutoZone. Before we connected the 2nd new battery, my friend checked the oil...just for fun. There's ba

LOL. 7 days later, truck was dead again. Last week, 10/21, I'm told by GM Customer Support to Not have it towed in yet until I hear back from a senior advisor, after telling her my long winded story. She said it would be within 48 hours. 72 hours passed and nothing. Supposedly I'm receiving a call at 12:51pm today, yes that's correct, that's literally what they told me when I called in yesterday. I'm having the truck towed in today, 10/29. I continue to be jerked around, ignored, lied to from all angles of GM. My truck has been inoperable and/or unsafe to drive for over 6 months now, costing me all of my income from my 2 jobs that require my truck. GM needs to be held accountable.Read more

I was slowly entering a handicapped parking place right in front of a business. The distance from the front of the parking space to the building wall was about 15 feet, one car length. I approached the place to stop in the parking spot when in a split second the car violently surged forward and would have crashed into the cinderblock wall had there not been a supporting post on the building and a
... metal frame with equipment which stopped the forward movement. The surge was explosive, almost like the acceleration of an airplane taking off. The whole incident took a second or two. There was no opportunity for any driver response to the surge. Had we not been using seatbelts, the front seat passenger & myself would have gone into the windshield and perhaps been killed or very seriously injured. Had I not been driving toward the building when the violent episode occurred, I likely would have hit someone in the parking area and killed or seriously injured people or crashed into park cars. Our injuries were relatively minor due to the seatbelts. The passenger will need therapy on her neck and back. A week later we are still emotionally stressed. There were no indications there was anything wrong with the car prior to this incident. It had only 27,000 miles with new tires and no apparent service issues. This hybrid car, which goes about 22 miles on a charge, was about at the point when it would have transitioned from electric to use of the gasoline engine. The badly damaged car was towed to a lot and later picked up and taken to the insurance facility for evaluation. I have not yet heard back. A NYS trooper investigated. He told me twice to have the Insurance Company look at possible vehicle defects when they examined the vehicle for insurance purposes. I passed on that information, but it appears the insurance company only evaluates the car for damaged. I have not yet gotten a report back yet, but it appears the car is totaled.Read more

1. What component or system failed or malfunctioned, and is it available for inspection upon request? The issue involves the Dynamic Radar Cruise Control system (vehicle-to-vehicle distance control mode) in my 2018 Toyota Highlander. This system automatically cancels when the vehicle’s speed drops to approximately 25 mph (40 km/h) or below. The system disengages without driver input during norma
... l driving conditions such as heavy traffic or congestion. The component remains functional but poses a safety concern due to its operational design. The vehicle and system are available for inspection upon request. 2. How was your safety or the safety of others put at risk? This malfunction creates a significant safety risk when driving in stop-and-go or congested traffic. The sudden loss of adaptive speed control forces the driver to quickly resume manual braking and acceleration, which can cause delayed reactions and potential rear-end collisions. The unexpected disengagement may also confuse drivers, especially those relying on the system to maintain safe following distances. This behavior increases cognitive load and the chance of driver error, particularly for elderly or less experienced drivers. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. The issue has been observed and replicated multiple times during normal driving conditions, such as during city traffic or highway slowdowns. It is also acknowledged in the vehicle owner’s manual, which specifies that the system automatically cancels below approximately 25 mph. However, this design flaw has not been formally addressed or corrected by Toyota dealerships or service centers as a safety concern. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The vehicle has not been inspected specifically for this concern by Toyota or other third parties. However, the condition is consistent across the same model and year,Read more

I understand that this is not a legally mandated recall, but the defect is clearly safety-related, with potential to cause cuts or injury while driving. I have documented my communications with Cadillac, including my requests for a goodwill repair exception due to the safety risk, but they have declined to help. As a consumer, I feel unprotected and at risk. I am submitting this report to NHTSA to request that the agency review this safety concern and ensure that Cadillac addresses it appropriately. I seek guidance on what actions can be taken when a manufacturer refuses to remediate a known safety issue that manifested during the coverage period. Vehicle: 2023 Toyota RAV4 Hybrid Issue: Persistent brake noise, safety hazard, manufacturer negligence I leased a 2023 Toyota RAV4 Hybrid from Clermont Toyota (Florida). Well before reaching 5,000 miles, the brakes began squealing and grinding, posing a potential safety hazard. Multiple service visits failed to correct the issue. Toyota Motor Sales, U.S.A., Inc. and its distributor, Southeast Toyo
... ta Distributors LLC, have long been aware of this defect, as documented in Technical Service Bulletin TSB-0001-22 and numerous NHTSA consumer reports describing similar brake failures and noises in 2019–2023 RAV4 models. Despite repeated notice, Toyota refused to repair or replace the defective vehicle and denied my Lemon Law claim through an arbitration panel connected to manufacturer counsel. This unresolved brake defect creates danger to the driver, passengers, and others on the road. Toyota’s continued sale and lease of vehicles with known braking issues constitute a serious public safety concern. I request that NHTSA investigate Toyota’s knowledge of these defects, evaluate the adequacy of its response, and consider a recall to protect consumers nationwide. Vehicle Information Make: GMC Model: Yukon XL Denali Year: 2021 VIN: [XXX] 1. What component or system failed or malfunctioned, and is it available for inspection upon request? The engine failed due to defective lifters and camshaft components, along with excessive oil consumption. These issues rendered the vehicle inoperable and required over $10,000 in repairs. The failed components are avail
... able for inspection upon request. 2. How was your safety or the safety of others put at risk? The engine failure occurred while the vehicle was in motion, creating a dangerous situation for my family and other drivers. Sudden loss of engine power could have led to a collision or left us stranded in unsafe conditions. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. The issue was diagnosed and confirmed by DeFouw GMC in Lafayette, Indiana. GMC has acknowledged the defect and agreed to cover all but $2,000 of the repair costs. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The vehicle has been inspected by DeFouw GMC. GMC is aware of the issue under Case #[XXX]. No inspections have been conducted by police or insurance representatives. 5. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? No, it all happened at once. The vehicle was extremely well cared for, and GM has all of the oil change records. Additional Comments: We have been without our family vehicle since [XXX], and GMC has not provided an estimated arrival date for the replacement engine. We are now entering month four without a vehicle, which has severely impacted our family’s mobility and safety. This delay is unacceptable and reflects poorly on GMC’s commitment to customer safety and service. Has also cost us over $10,000 out of pocket, which I will seek reimbursement for legally, if I must. After driving for 35 minutes (in the rain), I pulled up to work and let someone out. While car was sitting there in park, it bucked in the back, I then put it into park and almost immediately, it shut off with this on the display "Engine Off, Steering Power Low, Do Not Drive". My lights and windshield wipers remained on. I tried to turn it back and and it shut off again. I waited a few minutes, tu
... rned it on and went to pull out onto main road and the car shut off again with the same message. Thankfully I was turning onto the main road so close to shoulder and able to keep my car pretty close to side of the road. I called the Toyota dealer who told me not to drive it and it would need to be towed. I towed it to the closest dealer and when they checked the codes (the next day), they said there were no codes and they could not replicate the problem. As a note, the rain was off and on, nothing that was worse than anything I have driven in previously. I drive main roads and did not go through any large puddles or anything similar. This is a major safety issue and I am terrified that it could happen while driving on a highway where I may not have time to get out of the way of other traffic (car literally stopped with no option of continuing to coast). I am also shocked that there were NO codes triggered as there was obviously some event and that the car is only one month outside of warranty.Read more

I was traveling on an interstate going home from work. My speed was approximately 65mph. I noticed traffic slowing up ahead. I began to drop my speed (cost down), as I was approaching the traffic the car in front of me was applying their brakes; I had already began to apply mine. I was applying more pressure to my brakes to slow down more as the person in front of me began to slow more quickly. Th
... e person in front of me then had to slam their brakes on and I was not able to stop in time to avoid collision with the car in front of me. That driver did not come in contact with any other vehicle. The impact was at approximately 50mph. I was then very quickly hit from behind by the car behind me. The vehicle behind me hit of center and more on the right back corner of my car. My vehicle sustained significant front-end damage; the hood was folded, the grill was gone, the radiator was pushed back. As I looked up after the impact and saw my hood folded I then noticed that my air bag had not gone off. How could the air bag have not gone off in an impact such as that. My safety was compromised by the possibility of hitting the steering wheel or worse such as impacting the windshield if the crash had been worse. I do not remember if my vehicle had side curtain air bags as well. NO air bags deployed. Law enforcement was called by the driver of the car I rear ended as we agreed that she would. The driver of the vehicle that hit me I believe also called 911. Highway Patrol responded along with state incident response vehicles to protect the scene. I have filed a claim with my insurance, and the vehicle was looked at and deemed a total loss. It is not available for inspection and was taken to a salvage yard per the insurance company. I did not notice any lights on on the dash after. I also did not encounter any issues before this. I had the vehicle just over a year.Read more
