I had been driving for over an hour and heard a sudden explosion that made me think there was a gunshot and heard glass shattering but couldn't see anything. I was fearful so I drove further down to the fire dept to assess to the damage and ensure no one was following me. Called 911 for help because I was so shaken. Discovered the back portion of my panoramic sunroof had exploded.

Vehicle: 2016 Toyota RAV4 Color: Factory Super White / Blizzard Pearl (whichever applies) I am filing a complaint regarding a well-known latent manufacturing defect involving peeling factory paint on my 2016 Toyota RAV4. The original factory-applied white paint is peeling and delaminating from the exterior metal body panels under normal use and maintenance conditions. The paint failure appears un
... related to any accident, impact, misuse, neglect, or environmental damage. The failed component is the factory-applied exterior paint and coating system on the vehicle’s metal body panels. The affected areas are available for inspection upon request. The paint is separating from the primer/base layer and peeling off in sheets, exposing underlying surfaces and creating risk of corrosion and reduced vehicle value. Toyota has publicly acknowledged widespread issues involving “Super White (040)” and “Blizzard Pearl (070)” paint on numerous Toyota models, including RAV4 vehicles, through Customer Support Program ZKG and multiple Technical Service Bulletins. Toyota documents specifically state the issue involves paint adhesion failure during the manufacturing process causing peeling on exterior metal body panels. This issue creates safety concerns because exposed metal surfaces may eventually corrode or rust, reducing long-term structural integrity and potentially creating visibility distractions while driving as paint continues to separate and peel unexpectedly. Large peeling sections can also detach while driving and create roadway debris hazards for other motorists. The condition has been widely reported by consumers across online forums, complaint databases, and dealerships for many years. Numerous owners of white Toyota vehicles have reported identical failures involving factory paint peeling and delamination. The problem has been visually confirmed by dealership personnel / independent body shop personnel (modify if applicable). And they have refused to cover it at allRead more

The vehicle cannot be remedied under standard recall procedures. By refusing the repair without providing an alternative solution The manufacture is failing its federal obligation to remedy a known safety defect. We did not reach back out to the dealership as we feel they definitely caused more damage and already treated us poorly. Something needs to be done because Toyota is stalling on sending out a critical safety component that has been recalled in thousands of vehicles. It is the HV floor wire connector, when the corrosion is beyond repair, the parts have been back ordered for months. Toyota and dealerships are not approving loaner vehicles. This is a serious safety issue because: If the wire corrodes or shorts out, the
... vehicle can experience unexpected power loss, stalling, or complete hybrid system failure. this could happen in the middle of driving in fast, traffic, leading to serious injury or loss of life. I have been waiting since February for this part and it is still on backorder. Toyota is not taking this seriously because it’s a $10,000 part and they don’t want to pay for the thousands of people who are affected by this. My life is more important than that, as is the life of my child. I exhort you to put pressure on Toyota. Thank you.Read more

Toyota made a defective coolant valve in their 2019-2021 cars that leaked spontaneously in these vehicles. Over 950,000 cars have been affected to date, including my 2020 Rav4. They are aware of the issue and made a customer support program for this issue which led to engines overheating/electrical code problems, which can cause a severe safety issue if left unrepaired. The program however has a l
... imitation until November 2025 or 100,000 miles whichever comes first. When I was alerted of this, I brought my car to the dealer for service, but they wouldn't replace the part because it wasn't leaking too much at the time to need repair. However, just 4 months after the primary coverage expired, my engine code went off and when I brought my car, the coolant valve turned out to be defective and leaked into the electrical wire harness which then also had to be replaced in addition to the coolant valve. The repair cost was $1200 and both were required to keep the car driving safely and reliably on the road. Because my car had 120,000 miles and the known issue presented after Nov2025, Toyota refuses to cover the service, although it is a known manufacturing defect. Toyota is committing fraud to avoid covering this safety issue. The repair is expensive and if left unrepaired, the cars can spontaneously catch on fire on highways and in parks leading to wild fires and endanger others on the road. The toyota technician specifically informed me the issue was similar to other cars and known and the defect led to the destruction of the wire harness. To avoid a safety recall, Toyota reached a private settlement against class action lawsuit and created a limited CSP to avoid covering the defect in cars like mine to maintain their reliability status in consumer reports so cars like mine with little higher mileage won't be reported. It is fraud. I believe they are skipping on consumer repair bills that consumers on entitled to for their manufacturing defect.Read more

The vehicle experienced a sudden, catastrophic brake-by-wire system failure while operating in one-pedal driving (Regeneration Boost mode). While decelerating at a low speed behind an 18-wheeler semi-truck, the vehicle failed to continue slowing down. I immediately depressed the physical brake pedal, but the vehicle completely ignored the foot pressure. The pedal and braking system became complete
... ly unresponsive, and the vehicle kept rolling forward uncommanded. Despite keeping the brake pedal fully depressed, the vehicle did not decelerate, resulting in a front-end low-speed collision with the metal rear bumper of the semi-truck. Because the collision occurred at a very low speed, the operator of the commercial truck was unaware of the impact and safely continued driving forward without stopping. The vehicle's brakes entirely refused to re-engage until I frantically pumped the physical brake pedal and cycled/pressed vehicle control power switches. This action seemingly forced the electronic braking modules to reboot or re-establish hydraulic pressure, allowing the vehicle to finally come to a stop. The vehicle was subsequently removed from the active roadway. This is a severe, life-threatening safety defect involving a total loss of electronic brake-by-wire and friction braking application without any mechanical override intervention, presenting an extreme hazard. The vehicle has been towed directly to an authorized Toyota dealership for a comprehensive electronic diagnostic pre-scan of the Skid Control ECU, Electronic Parking Brake system, and 12-volt auxiliary power systems to retrieve the stored DTC fault logs and freeze-frame data.Read more

A vehicle was sold and delivered while under an active stop‑sale safety recall (manufacturer recall 26TB03). The recall was not disclosed at the time of purchase. When questions were raised about whether any recalls were present, the issue was dismissed and no recall information was provided. According to the recall documentation, the potential defect involves the rear door system possibly openi
... ng while the vehicle is in motion. Because the stop‑sale indicates the vehicle should not have been sold, the safety risk is that the vehicle was driven with occupants in the rear seats without anyone being informed of the potential hazard. This created a risk to the people inside the vehicle and to others on the road. The problem was later confirmed through the manufacturer’s official recall database and the published recall information. The issue has not been inspected or repaired by the manufacturer or dealer because the remedy is not yet available. The defect is not something the owner can see or detect on their own.Read more

I am reporting a serious safety defect in my brand-new 2025 Toyota Land Cruiser (Hybrid J250 series). Vehicle Information: VIN: [XXX] Mileage at time of failure: approximately 4,000 miles Date of purchase/delivery: [XXX] Purchased new from: Koons Toyota Tysons Corner, VA Problem Description: On [XXX], while driving, a suspension component detached/fell off, causing loss of control and a cr
... ash. This was a complete mechanical failure of the suspension system with no prior warning or impact from another vehicle. The failure occurred despite the vehicle having extremely low mileage and despite me previously reporting suspension issues to the dealer. Prior to the crash, I took the vehicle to the dealer on April 17 , 2026 for suspension problems. The dealer inspected it and explicitly stated that the vehicle was safe to drive. Shortly afterward, the suspension failed catastrophically, resulting in the crash. This indicates a manufacturing, design, or assembly defect that poses an unreasonable risk of crash, injury, or death. No modifications, abuse, or lack of maintenance contributed to this failure. This is a new 2025 model with no prior owner history. I believe this is a safety defect that may affect other vehicles, especially given the extremely low mileage at failure. I request that NHTSA investigate this as a potential safety defect, consider opening a defect investigation, and issue a recall if warranted. I attached letters i sent to dealer service advisor and service manager and i haven't gotten respond at all. I mistyped date for service and service was done on March 17th, 2026. New, strange noise started in Dec'25 (41K miles; 2yrs 9 months). No warning lights. Diagnosed as failed brake accumulator (first at an independent shop, later at the dealer). Apparently the accumulator is part of the full master cylinder assembly which must be replaced. Toyota has quoted $3,200 to complete this repair and is telling me it must be out of pocket.
