The second incident in In&Out the car died suddenly, tried to turn the car but it was on Drive so change to stop then tried to turn on multiple times but faulted , the crews decided to help us push the car out of the drive thru since we are blocking all the customers car, the steering wheel so stiff could hardly turn. The engine failed while driving on the interstate at 65+ mph. The malfunction indicator lights came on and beeped loudly, including Engine Malfunction and AWD Malfunction (which I don't have). The vehicle lost power, and the engine began making a frightening noise. I had to get over several lanes with no working gas pedal, and I'm lucky nobody was hurt. I had to be towed thirty miles to the neares
... t dealership. The dealership, who didn't answer when I called ahead to let them know I was coming, was unhelpful, and employees made jokes about just trading it in instead of fixing it. They said it could be months, and that there's no remedy for the recall. I have to wait until they re-open on Monday to speak with anybody. The only symptom prior was that sometimes the vehicle wouldn't recognize my key fob being near/in the vehicle, but I thought it was a battery problem. I thankfully knew about the recall and mentioned it to the dealership, but they don't seem to be agreeable to loaning me a vehicle in the meantime, so this will be a difficult repair process.Read more

I was driving to work on the morning of Nov. 12th, 2025 in East Greenwich RI. The car had been fully charged on my home Level 2 charger the night before. It was a chilly morning so I turned on the heated driver seat and steering wheel. Less than a mile from my house, and without any warning or prior issues, I heard a loud pop from underneath the vehicle. Immediately the car displayed an error on t
... he dash reporting electrical system issues. The car still seemed driveable so I began to drive back home, noting that the car wouldn't exceed ~30 mph, but before I went very far at all the error changed to "Stop vehicle and check power supply." I pulled the car over on a side road and called for a tow to the nearest Hyundai service center in North Kingstown, RI. The next day the Hyundai service center reported that their diagnostics revealed that the ICCU had failed. I subsequently learned that they had been unable to drive the car even from their lot into the service bay - even with a jump charger the car wouldn't drive and they had to carry the scanner to the lot for the diagnosis. The Hyundai app reports 96% battery charge even now, which says that the 12V battery has been fully discharged, consistent with ICCU failure. The service center has not reported that the ICCU has been inspected by any other authorities other than them. The replacement part is due to arrive today, 11/15, and should be installed on Monday 11/17. This exact problem is apparently very common to Hyundai and Kia EVs, and was the subject of a recall for the 2022-24 IONIQ 5 model years.Read more

The multimedia head unit and Apple CarPlay system are malfunctioning. The screen randomly disconnects, goes black, and often fails to connect to iPhones. It shows “Retry,” and sometimes Apple CarPlay will only reconnect through the Bluetooth settings instead of normal wired or wireless CarPlay. Both of my new-generation Tacomas are available for inspection upon request. This issue creates a s
... afety risk because the screen disconnects or goes black while driving, causing loss of navigation, hands-free calling, and vehicle information. This forces me to look down or try reconnecting the phone while the vehicle is in motion, which increases the risk of distraction and a possible accident. I have taken both of my Tacomas to Toyota for inspection, but the dealer was unable to reproduce the problem. The issue happens repeatedly during normal driving and occurs on multiple iPhones, so it is not a phone-specific issue. The vehicles have only been inspected by Toyota at the dealership level. No inspections have been done by the manufacturer, police, or insurance. The problem is ongoing and the vehicles are available for inspection at any time. The symptoms include the screen showing “Retry,” disconnecting without warning, going completely black, switching to Bluetooth-only connection, and failing to recognize iPhones at startup. These issues appear randomly on different days and with multiple iPhones. No other vehicle warning lights were present.Read more

1. What component or system failed or malfunctioned, and is it available for inspection upon request? The multimedia head unit / Apple CarPlay system is malfunctioning. The screen randomly disconnects, goes black, and frequently fails to connect to iPhones. The system displays “Retry,” and at times Apple CarPlay will only reconnect through Bluetooth settings instead of normal wired/wireless op
... eration. Both vehicles are available for inspection upon request. 2. How was your safety or the safety of others put at risk? The failure creates a safety hazard because the screen disconnects or goes black while driving, causing sudden loss of navigation, phone functions, and display information. This forces the driver to look down, attempt to reconnect the phone, or search through Bluetooth menus while the vehicle is in motion, increasing the risk of distraction and a potential collision. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? No. I have already taken both of my new-generation Tacomas to Toyota for inspection. The dealership was unable to reproduce the malfunction, even though the issue happens repeatedly during normal driving. The problem occurs on multiple iPhones, confirming it is not phone-specific. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? The multimedia system has only been inspected by Toyota, but no police, insurance, or manufacturer inspection beyond the dealership has taken place. The issue remains ongoing and can be inspected at any time. 5. Were there any warning lamps, messages, or other symptoms prior to the failure, and when did they first appear? Yes. This report covers 2 events for the same problem. The first occurred in the spring of 2025 and I did not document the date or time. The second one occurred on November 10, 2025 @ about 10:15 a.m. (1) Last spring my wife was driving the Venza up the street to our residence going a slow rate of speed. As she approached the house, she took her foot off on the gas and coasted toward the left curb
... so she could back into our driveway. At that point, car suddenly surged forward and accelerated toward the curb. My wife rapidly applied the brake and averted any problem. (2) The second event occurred on November 10, 2025 @ 10:15 a.m. My wife was starting to park at a parking lot in the Tanasbourne Town Center. She was going to back into a parking spot so she drove forward going slightly left. Then, with the car still in drive, the car surged forward. She quickly stepped on the brake and the car stopped abruptly. (She was angling the car and was about ready to stop and put the car in reverse when this happened) Again, with her quick response braking, she was able to avert hitting person or thing. As to which component or system failed, there appears to be a random malfunction of sensors or perhaps the CPU...something that is calling for full throttle when the gas petal is not pushed. The safety risk is possible harm to the driver, outside property, or the worse situation would be harm to pedestrians that might be in the way of the car. We have not been able to reproduce the problem. The same maneuvers have been repeated 100's of times without incident. As of this writing, the vehicle is being taken to the Toyota dealer to have the oil changed and at that time they will be asked to do a thorough inspection of the systems that might cause this, check for any recorded malfunctions, etc. To the best of our knowledge, there were no warning lamps or messages that appeared.Read more

The 3rd generation Tacomas rear cabin vents (2 vents) allow water to enter the cabin because they are not tight to the body or sealed properly. The water leaks down the back wall and forward onto the floor soaking the carpet and padding underneath. The water in the carpet/padding has yielded mildew/mold throughout (water goes all the way to the front of the truck). The water also leaks through a f
... loor grommet into a section where the cab mount bolts are and significant rust develops. Not only is the exposure to mold a health hazard, but the corrosion/rust on the cab mount bolts is a significant structural concern. There are many online posts with this problem, and i have just recently found this on my Tacoma. My vehicle has had the 3rd brake light recall, and this issue is completely separate from that recall. Toyota has a history on these vehicles not sealing them as they should. 1. The 2 rear cabin vents fail to keep water from entering the cab. I am happy to provide the vehicle for inspection. 2. Exposure to toxic mold is a health concern. Significant structural damage to the cab mount bolts is a concern if the vehicle were to be in an accident. 3. Numerous people online have confirmed this issue, and they have indicated dealerships are well aware of the issue. I put a garden hose to the back of my truck myself and saw water entering the vehicle. 4. Vehicle has not been inspected by manufacturer, police, insurance, etc. 5. Unknown when water leak first started. In the past 2 years i have started to notice a mildew smell after a rainy day. I am happy to provide additional photos, but the internet is full of this very issue. Please see the following sites with photos/videos/descriptions of other Tacoma owners having the exact same issue. Thank you very much for your time.
