Almost caused high speed accident merging onto toll road! The vehicle is experiencing a severe transmission slippage, shudder, and grinding noise, consistent with TSB-0008-21 (UA80 transmission failure). This defect causes hesitant acceleration and erratic shifting, creating a safety hazard when merging onto highways or crossing intersections. I am concerned the transmission will fail completely
... while driving, resulting in a sudden loss of motive power and increasing the risk of a crash. Toyota has covered this exact safety defect in 2017–2019 models (CSP ZJC) but has failed to recall the 2021 models despite identical symptoms. First reared it's head as slipping when putting vehicle in reverse, 25k+ ago. Was told by Toyota Certified mechanics it was an operator error. Morphed into larger issue.Read more

I am reporting a serious safety defect involving the rear brake system of my 2021 Toyota Highlander, purchased from Penn Toyota (Glen Cove, NY). The dealership represented the vehicle as “Toyota Certified” and indicated that it passed all safety inspections. On November 15, 2025, I discovered that the rear brake rotors and pads were in extremely unsafe condition, showing severe corrosion, dee
... p pitting, uneven pad wear, and structural deterioration. This damage clearly developed over a long period of time and indicates that the vehicle could not have passed a legitimate safety inspection at the time of sale. The rear brakes had to be replaced immediately to prevent potential brake failure. After replacing the rear rotors and pads, the vehicle passed the New York State Inspection on the same day, proving that the previous brake condition was unsafe and improperly inspected. This raises concerns of: Brake safety failure Improper or falsified dealership inspection Risk of reduced stopping power Potential loss of vehicle control I am submitting photos, the repair invoice, and the inspection result that confirmed the brakes were unsafe prior to repair.Read more

TOYOTA 4RUNNER 2013 VIN# [XXX] LP# XXX The gas needle will be above the half (middle line), and the fuel range on top of the dash would read anywhere from 160-213 miles, going 45 mph up a hill or 70 mph on the freeway, all of a sudden starts to stall and completely shut off while still driving. ... o shift into neutral because it's a push/start button. The car will only turn back on after putting some gas into it, so I make sure to never let the gas needle go below the middle line or have the gas range reading go below anything around 160-200 miles since the vehicle has shut down at a different amount each time, and there's been multiple times. Also, the fuel level needle never matches with how many miles is shown on the range, displaying inaccurate readings. It has poor acceleration, poor mileage, and now making a loud whining noise that is heard from outside the vehicle near gas tank. I have tried looking up the VIN and it does not show that it falls under the recall, despite it having all the symptoms of a faulty fuel pump/sending unit. I love my 4Runner but I am in fear for my life whenever I drive it and not showing accurate fuel levels. I pray that nobody runs into me if the vehicle stalls out in the roadway, while going 30-80 mph (it has done it on regular roads and on the highway). There are no warning lights or check engine light. It's at about 208,000 miles, with the issue slowly getting worse over time. I am requesting for it to be inspected free of charge, as well as getting the necessary repairs that the recall is for free of charge. **Nature of Safety Defect Within 13,000 miles of normal mixed highway and light off-road use on a Toyota Highland, one tire developed a sidewall bubble and all four tires exhibited extreme and uneven tread wear—far below the advertised 50,000-mile treadwear warranty. The remaining tread depth on all four tires is already under 2/32" in multiple locations, with severe cupping and feathering that

Pirelli has refused any prorated adjustment, replacement, or refund, and has kept possession of the failed tire. The A-pillar contains the SRS curtain airbag system, its wiring, and connectors that should never be exposed to moisture. Continuous water intrusion over many months can cause corrosion, increased resistance, short circuits, or damage to the airbag inflator/squib circuit, which may lead to unintended deployment or failure to deploy in a crash. Water reaching the dashboard controls and electrical switches could cause intermittent electrical faults or malfunction of driver controls. These issues present a significant safety risk because the vehicle’s airbag and electrical systems may not function as designed. Has the problem been reproduced or confirmed? Yes. The Toyota dealer has repeatedly observed the active leak, interior rust streaks, and bubbling/corrosion along the roof seam. The water intrusion occurs every time it rains. Has the vehicle been inspected? Yes. Toyota has inspected the truck multiple times, including by a Field Technical Specialist. However, Toyota has been unable to provide complete documentation or photos from the FTS inspection, and the underlying safety issue remains unresolved. No inspections have been done by police or insurance. Warning lamps or messages? No warning lamps appeared. The front suspension on my 2021 Tesla Model Y developed progressive creaking and groaning noises during low-speed turning, especially in parking lots or when maneuvering. The noise began intermittently around 45,000 miles and became much louder by around 86,000 miles, which made me concerned about a potential safety issue before a long family trip. The sound comes from the front lower suspension
... area and indicates that something may be loose or failing. I am worried about the possibility of losing steering control if the suspension components separate or shift while driving, especially with my children in the car. The Tesla Service Center in Lisle, Illinois inspected the vehicle and informed me that both front lower lateral links and both front lower compliance links must be replaced (all four front suspension links). The repair estimate is about $1,600. This problem appears identical to the condition described in Tesla’s campaign/recall SB-21-31-003 / NHTSA 21V835, which involves improperly torqued front lower lateral link fasteners on 2020–2021 Model Y vehicles. My vehicle shows identical symptoms and identical components, but my VIN does not appear in the recall. I contacted Tesla corporate support by email requesting a safety/goodwill review, but I have not received any response. I believe this issue is safety-related and may indicate that more vehicles should be included in the existing recall. I am attaching the Tesla service diagnosis confirming the required replacement of all four front suspension links.Read more

Subject: Safety Defect Complaint – 2017 Toyota Sienna (VIN: [XXX] ) Dear NHTSA Safety Division, I am submitting a safety complaint regarding my 2017 Toyota Sienna, VIN [XXX] . In June 2023, while traveling on I-81 South in Virginia with my family of seven, the van stalled suddenly and emitted white smoke from under the hood, then the engine completely failed. The vehicle had only 111,000 miles.
... The repair would have cost $15,000, and I had to sell the van in 2024 for $4,000, suffering significant financial loss. Toyota informed me no recall applies to my VIN, although many 2017 Siennas experienced fuel pump failures causing stalling. As a person with disability and serious health issues, this incident caused extreme stress, hardship, and risk to my family. I request that NHTSA investigate this potential safety defect to determine whether other 2017 Siennas are affected. Sincerely, Your Name Phone Number Address INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
