I was driving on the highway when there was a sudden drop in power, accompanied by a flashing engine light. I was unable to proceed at normal highway speeds and was worried about a complete loss of power. I was able to coast/stay in a low gear to get off the highway and find a safe place to park the vehicle. Shortly after parking, the engine stopped and would not engage on attempts to restart.
... It was hauled to the local Honda Dealer who confirmed that the engine was completely seized and would need to be replaced. They have not disassembled it yet to confirm the precise failure point. Prior to the loss of power, there appeared to be some light, intermittent clicking sounds that I was trying to localize, but appeared to be coming from the front of the vehicle. This lasted for several miles. At the time of the decrease in power, this was accompanied by increased noise/knocking form the engine area. On stopping the vehicle was noted to be idling very rough. This vehicle experienced a failed torque converter around 2021 that Honda eventually had to extend their warranty to cover. In 2023, it had issues with misfiring fuel injectors which was also subject to an extended warranty by Honda to repair. Due to parts shortages, Honda was unable to fix the fuel injectors for over 8 months. This vehicle would be available for inspection on request.Read more

While driving at highway speeds (approx. 65 MPH), the vehicle suffered a sudden and catastrophic loss of motive power. The steering wheel locked, making it extremely difficult to maneuver the vehicle to the shoulder in a dark, remote
... area with freezing temperatures. This presented an immediate life-safety risk to all occupants. Following the incident, the vehicle was presented to two separate authorized Toyota dealerships in the US for emergency assistance. Both failed to provide the standard of care required for a high-voltage safety failure. The first dealer refused to perform even a basic safety inspection or intake the vehicle, stating they had no hybrid technicians on weekends. They turned the vehicle away without safety warnings, forcing the operation of a chemically failing vehicle on public roads. The second dealer attempted to charge a diagnostic fee. However, when questioned, they admitted they also did not have a hybrid-certified technician on site. They attempted to bill for a diagnostic service they knowingly could not perform. This fraudulent charge was only rescinded after it was challenged. They subsequently refused further assistance, leaving the occupants stranded. The vehicle was eventually towed to a third dealer (in Canada) where it was diagnosed with severe corrosion of the High-Voltage Floor Under Wire Harness (MGR Cable), a known defect. The dealer confirmed the rear motor cable was corroded and required replacement. The repair involved replacing the cable and installing a "splash guard" cap. No prior warning signs occurred before the sudden highway shutdown. This failure is a result of the manufacturer's failure to adequately seal high-voltage components against corrosion and the dealer network's gross negligence in refusing emergency safety support.Read more

The main water pump on my 2018 Porsche Panamera 4S failed prematurely. This is not a simple external coolant leak; the internal seal failed, allowing pressurized engine coolant to migrate into the vehicle's vacuum system. The coolant traveled through the vacuum lines and contaminated the vacuum solenoids and change-over valves. This failure mode presents a significant safety hazard because the va
... cuum system controls critical engine components, including turbocharger wastegates and exhaust flaps. Contamination of these components can lead to sudden loss of power, "limp mode" activation while driving at highway speeds, or unexpected engine behavior, increasing the risk of a crash. Cross-Reference to Known Defects: This vehicle utilizes the EA839 V6 engine platform, which is shared with Volkswagen and Audi vehicles (specifically the Audi S4, S5, SQ5, and Q7). Audi and Volkswagen have already acknowledged this specific manufacturing defect in the class action settlement Fiscina v. Volkswagen Group of America, Inc. (Case No. 2:21-cv-11251). Under that settlement, Audi extended the warranty on this water pump to 10 years/80,000+ miles for the exact same failure mode (coolant migration into the vacuum system). Despite my vehicle using the same engine and suffering the exact same mechanical failure, Porsche refuses to offer coverage or acknowledge the defect. I am requesting an investigation into why Porsche models equipped with the defective EA839 engine are excluded from the safety remedies and warranty extensions provided to Audi owners.Read more

My 2022 Ram 1500 TRX has recurring safety-related failures of the infotainment system affecting both Uconnect 5 (12” screen) and earlier Uconnect 4 units. These issues are widespread in 2021–2024 Ram TRX and Ram 1500 models and began or worsened after forced over-the-air updates. The primary defect is black-screen failures while driving. When the screen goes black or freezes, the truck loses
... all functions controlled through the display, including HVAC/defrost, backup camera, vehicle settings, seat controls, and performance gauges. This creates an immediate safety hazard: I cannot adjust climate/defrost, cannot see the required rear-view camera when shifting to reverse, and cannot access key vehicle information. Owners across multiple years consistently report: • Black screens, freezes, and reboots • Loss of HVAC control and dual-zone functions • Backup camera not displaying • Vehicle/Apps tabs failing to open • Uconnect error messages • Smaller, harder-to-read gauges after updates • Wireless CarPlay/Android Auto failures • No option to decline or roll back OTA updates These failures distract the driver, remove access to federally required equipment (rear camera), and eliminate visibility and climate controls that directly affect safe operation. The system becomes unpredictable and unstable despite the vehicle being in motion. My vehicle experiences the same issues, including: Radio reboots without notice(sometimes volume is very high), black screen while driving, loss of HVAC, camera not available, system rebooting, etc. Because this affects essential safety-related controls and visibility, I believe this is a safety defect in Ram/Stellantis Uconnect 4 and Uconnect 5 systems. I request NHTSA open a defect investigation and require Ram to correct these failures.Read more

My 2023 Ford F-150 XLT (base 4×4—2H, 4H, 4L only) is experiencing a front-axle/IWE malfunction that causes the hubs to partially engage on their own in cold weather. When temperatures drop below freezing, the truck produces a very loud, physical pop when turning in 2H and a mechanical grinding/rumble noise from the front axle while driving straight. This happens even though the roads are dry a
... nd I have not engaged 4WD. The noise immediately disappears when I switch into 4H, which indicates the integrated wheel ends (IWEs) are partially engaging due to vacuum loss or actuator failure. When the hubs fully lock in 4H, the grinding stops. This confirms the IWEs are not disengaging properly in 2H. This occurs only in freezing temperatures, to date, and it feels as if the front axle is binding or locking during turns. The loud popping feels violent enough that it could lead to CV axle, differential, or IWE gear failure. My Ford dealer stated they have “not heard of this issue,” but this condition matches the symptoms described in Ford TSB 25-2513 for incomplete IWE disengagement on 2021–2025 F-150 trucks. Despite Ford saying this is not a known problem, there is significant owner evidence online showing that many 2021–2024 F-150 owners experience the exact same symptoms. Multiple threads on F150Forum, F150Gen14.com, and Reddit describe identical cold-weather popping and grinding caused by IWEs partially engaging in 2H. In many of those cases, owners report that dealers dismissed the issue or stated it was “normal,” even though the symptoms are widespread and consistent. This is a safety concern because the front axle partially engages without driver input, causing binding, loss of smooth steering, loud mechanical pops, and grinding while in 2WD. This could lead to sudden front drivetrain failure, loss of control, or damage to steering/braking components. I am submitting this complaint so NHTSA is aware that this issue is occurring F-150s and may not be isolated.Read more

My fuel was low so I went to fuel up and after wards, my car struggled really hard to start. And every time since that days it’s progressively gotten worse. Last week I kept my car running while I fueled up and as I drove away from the gas station, my car just stalled out completely. It took me about an hour and a half before my car would start again. I was waiting for the tow truck to come get me and thank God inc checked it one more time and it started. Now just recently, when I put my car in park the engine immediately Reva up super high. It started to wine really loud and rpm go through the roof. Until I shut the car off. So I am unable to sit in idle at all anymore. I explained issue, requested a total diagnosis of everything that was wrong but most importantly, I wanted the total power loss issue diagnosed, pinpointed and resolved. because I don't feel safe driving until issue is truly diagnosed and fixed. I gave them my lifetimecarmaxplus warranty . The techs found a list of parts that needed to be fixed besides the power loss problem, but told me they couldn't fix the total power loss because no codes had been thrown. I explained that since there was a total power loss it was reported by other Dodge techs that no codes could be retrieved. I asked the service manager to ask his tech what diagnostics were used and to what extent to determine the problem. He said he would call me after talking to his tech but never did. I went to the dealership 2 weeks later and asked what he found out from his tech. He was confused and apparently never did anything more about the situation. On [XXX], we were driving from Coalinga, CA to Phoenix, AZ for Thanksgiving. As we began to descend into Los Angeles on [XXX], three warning indicators suddenly went on: the ABS warning light, the traction control light and the brake warning light. The brakes still worked but the brake assist was weakened. We pulled over as soon as possible at the nearest AutoZone. The employee performed a sta
... ndard handheld OBD-II scan of the vehicle and did not detect any issues. We continued on our way through Los Angeles and to San Bernadino where we were spending the night. At one point the indicator lights went off for a few minutes but then came back on. We spent the night in San Bernadino, and the next day (11/26/2025), we took the car into the San Bernadino Toyota Dealership as we felt it was unsafe to keep driving. The dealer conducted a thorough full brake and hybrid system diagnostic and confirmed that the failure was inside the brake booster system. The repair estimate states: "Brake booster assembly with master cylinder and brake booster pump assembly have an internal leak." Thus, Toyota's own technician identified an internal hydraulic leak within the booster system, affecting the booster assembly, master cylinder, and booster pump assembly. The total repair estimate is $3,959.99. This is a major safety defect affecting sealed hydraulic components that are not part of routine wear-and-tear. There were 4 people in the car. Fortunately, the brake failure did not result in an accident, but it could have if we had kept driving on to our destination. We contacted the Toyota Brand Engagement Center to ask for Goodwill Assistance, but they were unable to help us. In addition to the repair costs, we have incurred rental car costs. This makes the vehicle unsafe to drive. ⸻ 4. Despite these unresolved issues, the dealership insists the vehicle is “repaired” and demands that I pick it up. They refused to provide frame measurement reports or explanation for the unequal wheelbase. Returning a vehicle with unresolved steering and chassis geometry defects constitutes unsafe vehicle delivery. ⸻ Conclusion Based on unequal wheelbase, abnormal caster geometry, and inability to drive safely, this vehicle likely contains a structural or assembly defect. I formally request investigation, suspension geometry verification, and required action from the manufacturer, as this is a matter of public road safety.Read more

Vehicle Cannot Be Safely Driven – Structural / Suspension Defect Suspected – Dealer Claims “Repair Completed” Despite Unequal Wheelbase My vehicle is currently unsafe to operate, due to the following critical issues: ⸻ 1. Left–right wheelbase inconsistency The front and rear wheelbase measurements are not equal side-to-side. This is a strong indicator of structural deviation, such

This is a Thor Scope RV built on the Promaster 1500 platform. The incident involves the Onan gasoline powered generator, which is directly connected to the fuel tank. Next to the generator in the fuel line, there is a small white inline fuel filter connected by the fuel hose going over a nipple on the filter and reinforced by a hose clamp. In my case, the hose clamp was not installed sufficiently
... over the nipple (far enough back on the fuel hose that it did not completely engage the nipple), and presumably from vibration associated with the generator operation and driving, the hose fully detached from the nipple and spilled fuel in an uncontrolled manner until I smelled/noticed it coming out from under the vehicle. There was certainly sufficient fuel to have started a major fire if it had reached an ignition source. Fortunately, I was able to get under the vehicle and push the parts back together once the problem was noticed. But, this would not have been possible if there were any ignition sources nearby. Fortunately, the vehicle was parked and off when the spill occurred. Had I not been able to stop the leak, it is my understanding that 3/4 of the tank would have drained since I've been told that the intake is located at the 1/4 full level of the tank to prevent the generator from running the vehicle completely out of fuel. Photos attached show the part after I pushed the hose back on and tried to reposition the hose clamp closer to the center of the filter. This was very difficult to do while laying on the ground under the vehicle, so I didn't get it on as far as I would have preferred until I was able to get the vehicle to a place where I could elevate it and get better access.Read more

The contact owns a 2013 Hyundai Sonata. The contact stated that while driving at an undisclosed speed, the vehicle stalled, with an abnormal knocking sound coming from the engine compartment. The check engine warning light was illuminated. The contact was able to restart the vehicle; however, the failure persisted. The contact previously received notification of NHTSA Campaign Number: 17V226000 (E
... ngine and Engine Cooling). The vehicle was taken to the dealer, where it was diagnosed, and determined that the engine short block had failed and needed to be replaced. The contact was informed by the dealer that the vehicle would not be serviced under the recall. The contact was offered a buyback option for the vehicle and was also offered a loaner vehicle. The vehicle was not yet repaired. The manufacturer was made aware of the failure. The failure mileage was not available.Read more

Hello, My drivers side front left window goes up and down awkwardly, almost like it's going to brake the glass when I use it. If there's a recall on this I will gladly like to know. Thank you.

With only 32,885 miles, engine light came on due to (P0456) EVAP small leak. Went to an auto shop and Ford dealer for diagnostic scan. Both technicians found that there is a bit of smoke coming out from the top of the fuel tank due to a small stress crack at the port near the canister vent hose as well as a small crack below fuel pressure sensor tube. New fuel tank replacement was recommended as t
... he crack could not be repaired. Cost to fix is $1,400 parts and $1,200 labor. This small crack nears the fuel tank seems to be a major concern for owners as it could increase the risk of vehicle fire and should be a recall by Ford for immediate remedy.Read more

My Hyundai is experiencing severe and continuous engine oil consumption that creates a safety risk. The vehicle loses a significant amount of oil within 1,000–1,500 miles, sometimes showing almost no oil on the dipstick despite recent oil changes. There are no external leaks, no burning smell, and no visible oil under the car. The cause remains UNKNOWN, but it appears to be internal engine burni
... ng or a manufacturing defect. I have taken the vehicle to the Hyundai dealer multiple times. They performed oil consumption tests but either reported “normal results” or did not provide clear explanations. The issue continues exactly the same. I also contacted Hyundai corporate, but no meaningful assistance or goodwill support was provided. The dealer acknowledges no leaks but still refuses to repair or replace the engine. This problem is dangerous because the oil level drops without warning lights or error codes. If I do not manually check the dipstick, the engine could run dry and seize while driving. Sudden engine failure on highways or in traffic can cause accidents. I now have to frequently top up oil just to keep the engine alive. I have maintained the vehicle properly and completed all requested tests. The cause is still UNKNOWN, but based on similar Hyundai cases, it may involve piston rings, valve seals, or internal engine wear. Many Hyundai owners report the same issue, suggesting a broader defect. The dealer’s only advice is to “keep monitoring oil,” which is not a safe long-term solution. This defect poses a serious safety concern and financial burden. I am filing this complaint so NHTSA can investigate, as the engine may fail unexpectedly, putting me and others at risk.Read more
