A car attempted to make a left hand turn across our lane of traffic. We didn't have any chance to break to avoid the collision and all front airbags were deployed. The front passenger airbag didn't seem to deploy properly and seems that it inflated upwards to the windshield and not toward the passenger. The windshield was shattered by the airbags sending glass into the passenger compartment.

I am submitting this complaint to report what I believe to be a manufacturing or design defect involving the sunroof of my Jeep Grand Cherokee. On or around December 1, 2025, I discovered a severe water leak originating from the sunroof area during normal rainfall. Water entered the vehicle interior without warning and caused substantial damage to the headliner, interior trim, and other interior
... components. The amount of water intrusion was significant and far beyond what would be expected from normal wear. The vehicle has only around 45,000 miles and is regularly maintained. The leak occurred without any visible damage to the sunroof glass and without prior indication of a problem. The sunroof was fully closed at the time. This suggests a defect related to the sunroof sealing system, drainage system, or overall design rather than misuse or external damage. This defect presents a serious safety concern. Water intrusion inside the vehicle creates the risk of: Electrical system damage or failure, Mold and health hazards, Impaired visibility due to water dripping near the windshield, Distraction to the driver while operating the vehicle. In heavy rain conditions, this defect could reasonably occur while driving, creating a dangerous situation for the driver and passengers. As a result of this defect, my vehicle sustained significant interior damage, requiring repairs. I believe this issue reflects a broader defect in the sunroof design or drainage system rather than an isolated incident. I am concerned that other Jeep Grand Cherokee owners may be experiencing similar failures. I am requesting that NHTSA investigate this issue for potential safety-related defects involving sunroof leakage in Jeep Grand Cherokee vehicles. The severity of the water intrusion and resulting damage indicates a defect that could pose a risk to vehicle safety and occupant well-being.Read more

I am reporting a defective CVT (Continuously Variable Transmission) in my Toyota vehicle that completely failed at approximately 85,000 miles under normal driving conditions. The transmission did not exhibit abusive use or unusual load beyond everyday driving — just standard highway and city miles. At ~85,000 miles, the transmission began to exhibit significant performance degradation and mecha
... nical failure, including (describe what happened — e.g., whining, slipping, failure to engage, loss of drive). Eventually it failed outright, leaving the vehicle inoperable and requiring a replacement transmission at substantial cost. Toyota has used this CVT design across multiple models, and there are numerous owner complaints and documented powertrain failures tied to Toyota’s CVT, including reports of failures well below 100,000 miles. Independent sources and consumer complaint databases show this is not isolated — many owners report early transmission failures and extremely high replacement costs when out of warranty. CarComplaints +1 I believe this is a safety and reliability defect because a transmission giving out at moderate mileage can cause loss of propulsion, unexpected behavior while driving, and potentially hazardous situations if it fails while the vehicle is in motion. This issue should be investigated for potential recall or formal safety action, as the frequency and severity of failures impose undue financial hardship and risk to consumers. I am submitting this complaint to alert NHTSA to the pattern of CVT failures in Toyota vehicles and to ensure that this problem receives appropriate investigationRead more

The brakes on my 2008 Toyota Tacoma were failing so I took it to a mechanic, who said that the brake lines had failed and needed to be replaced. When I asked the mechanic about this, he said that he didn't have anything to do with sales and we needed to speak to the owner (who is out on medical leave). The mechanic said that when the dealership purchased the vehicle at auction, the frame must have been undercoated and so they missed it. When I mentioned the recall, he told me to call Toyota. I have checked the VIN and it is not showing an open recall for this vehicle. However, it is experiencing the exact same safety issues as the recalls for leaf spring corrosion and frame rust and cracking. I have only had this truck for a little over one year and do not know what to do. It is too dangerous to drive as it could cause a serious accident. On June 20, 2025, my 2023 Toyota Tacoma (VIN [XXX] ed a sudden loss of effective braking during normal driving conditions (dry roadway, clear weather). The vehicle was being driven by my adult son. While slowing for a turn, the instrument cluster displayed a Pre-Collision System (PCS) malfunction warning and a Vehicle Stability Control (VSC) warning. The Driver applied the brake pedal contin
... uously, but the vehicle did no decelerate as expected. ABS did not activate, and braking effectiveness was insufficient. During braking, the driver experienced reduced steering responsiveness. In order to avoid entering a busy roadway without adequate braking capability, the driver applied maximum steering input, fully loading the left side of the steering wheel with as much force as possible in an attempt to redirect the vehicle. Any reduction in speed occurred primarily due to the change in direction and tire to road friction, not effective braking. The vehicle was towed to a Toyota dealership. Dealer intake records document a PCS warning and no brakes upon arrival. Event Data Recorder (EDR/CDR) data shows continuous brake pedal application for approximately 5 seconds prior to the event, accelerator at 0%, and brake system hydraulic pressure reaching approximately 12.14 MPa (maximum recorded). Despite this, the vehicle did not stop as expected. No frontal crash pulse recorded. Toyota Motor North America later denied responsibility, asserting incorrect vehicle speed and stating that a CARFAX report indicated prior front-end damage. The CARFAX vehicle history report, dealer purchase disclosures, and service records show no reported accidents or damage. Toyota declined to provide brake system ECU logs (ABS/VSC/brake control modules). This incident raises concerns regarding a potential safety defect involving electronically controlled braking and its interaction with PCS stability control systems. UNKNOWN Dear Toyota Motor Sales, U.S.A., Inc. Consumer Affairs Department: I am writing to formally demand a full buyback and refund for the above-referenced 2025 Toyota Sienna (the "Vehicle") under the Arkansas New Motor Vehicle Quality Assurance Act (the "Arkansas Lemon Law"), Ark. Code Ann. §§ 4-90-401 et seq. The Vehicle is subject to a critical safety recall (25TA12, with interim notice 25
... TB12) involving improperly welded second-row seat rails, which poses a substantial risk to the safety of myself, my passengers, and others on the road. As of the date of this letter, Toyota has issued no remedy for this defect and provided no estimated timeframe for when a repair will become available. This renders the Vehicle unusable for its intended business purpose and out of service indefinitely, in violation of the implied warranties of merchantability and fitness for a particular purpose, as well as the express warranties provided with the Vehicle. Background and Defect: I purchased the vehicle on [XXX], new from Landers Toyota NWA, Rogers AR for $49,058. The Vehicle has been registered and primarily used in Arkansas since acquisition. On or about October 7, 2025, Toyota issued Recall 25TA12, notifying owners that the second-row seats in certain 2025 Sienna models, including my VIN , are mounted on seat rails that may have been improperly welded. This defect could cause the seats to detach during a crash, increasing the risk of injury or death. An interim notice (25TB12) followed, confirming the issue but explicitly stating that no repair remedy is currently available and no loaner vehicles or temporary fixes are being provided. Since receiving notice of the recall, the Vehicle has been effectively out of service for safe operation. I cannot, in good conscience, use this vehicle with a known safety defect that compromises occupant protection in the event of an accident. Toyota has failed to provide any repair, temporary solution, or estimated completion date, leaving me without a functional vehicle for commercial puRead more

The heating system in my 2024 Tesla Model Y failed after only one winter season. Under normal HVAC operation, the vehicle blows cold air and does not provide cabin heat. Heat only intermittently works when defrost mode is selected. This failure affects the vehicle’s ability to safely operate in cold weather, including proper windshield defogging and defrosting. A loss of heating presents a safet
... y risk to occupants and reduces visibility. Tesla diagnosed the issue and provided an estimated repair cost of approximately $3,235.00. The vehicle is just outside the warranty period, and this appears to be a premature failure of the heating or heat pump system, not normal wear and tear. Given the age of the vehicle and prior reports of Tesla HVAC and heat pump failures in Model Y vehicles, I believe this may indicate a design or component defect that warrants investigation.Read more

Car had 59,522 miles when I purchased it (now has 64,320), and after driving 1,500 miles, it began hot-idle stall with oil light indicator lit. Restarted and drove OK, but when slowing, it kept stalling. This also happened on the roads when slowing for a light or for traffic, posing a risk to myself and others. After 6-7 times and doing some research, this problem exactly matches the symptoms in K
... ia’s TSB ENG222 Rev 6. I had it towed to a KIA service center due to the concern that it can throw a bearing during stall (as well as be unsafe). Dealer performed combustion chamber cleaning and replaced the PCV valve ($1,000!). They asked me to keep a close eye on oil consumption. Over the next less-than-1,000 miles of driving, engine consumed 3 quarts, well in excess of Kia’s expectation of no more than 1 quart per 1,000 miles. The dealer contacted Kia’s technical assistance for help and was denied an engine replacement due to me being the second owner, out of power train warranty and just purchasing the vehicle, as well as indicating that the engine had previously been “fixed” pursuant to the TSB; however, clearly it is not fixed. I now unfortunately know that this problem is associated with the poor design of the engine and up to 20% of the owners who have had the problem “fixed” continue to have the same problem recur. The engine has barely more than 60,000 miles on it and was maintained, according to CarFax, rather flawlessly by the previous (and only other) owner. There is no way that properly built and appropriately maintained engine should fail with less than 60,000 miles on it unless there is something inherently wrong with the design or build of the engine. It also seems like a huge copout on Kia’s part to not honor their 100,000 mile, 10 year powertrain warranty simply because the car transferred owners (once), especially with the history of known appropriate servicing AND that the car’s engine has already failed the repair directed by TSB ENG222 Rev 6.Read more

While driving under normal conditions, my 2023 Toyota Tundra TRD Pro Hybrid experienced a sudden and severe loss of propulsion. Multiple warning messages illuminated simultaneously, including: Hybrid System Ma
... lfunction, Check Engine, Smart Stop System Malfunction, and Advanced Ultrasonic Detection Malfunction. The hybrid system effectively shut down and the vehicle could not maintain normal road speed. I brought the vehicle to a Toyota dealership. The dealership performed only ECM and hybrid ECU software updates and cleared codes. No mechanical inspection, oil pressure analysis, particulate inspection, turbo system evaluation, hybrid motor synchronization testing, or engineering-level diagnostics were performed. Toyota Corporate subsequently stated that the software update was the “final” resolution and declined further diagnostics. I was advised to return to the dealership if the issue reoccurs. This failure mode matches the documented loss-of-propulsion behavior described in Toyota’s V35A engine machining-defect recall (sudden power loss, limp mode, cascading system failures). Hybrid models are currently excluded despite using the same engine platform and exhibiting identical symptoms. Sudden loss of propulsion at road speed presents a serious crash risk. This appears to be a safety-related defect that is not being adequately investigated or remedied for hybrid models.Read more

When driving vehicle, alert sounded lowered speed detection and my engine light came on. My vehicle then lagged power and it smelled like something was burning. Called dealer i bought vehicle from and i drove it in to them. I just bought the vehicle 8 months ago. I was just in to same dealer for a GPS tracker that was in my vehicle unknown to me that the Onstar stopped working and the dealer had s
... tating they missed it in the pre certified inspection and they had to replace the telemax module do to the unknown GPS Tracker. My vehicle is now in for the lowered speed detection problem which my GM app stated it was an air induction system problem. I do not have paper work for the air induction system yet but i have sent photo of what my GM app produced.Read more

We recently had an engine failure in our 2023 Toyota Tundra ([XXX]). Then on [XXX] the truck caught fire while we were driving in Arkansas. The fire caused the brakes to fail completely, and we wound up jumping from the still moving truck. The truck and contents were a total loss. Since then, Toyota has completed their investigation and stated they are unable to determine the cause of the fire I

Throttle lag truck stalls out when you try to go from a stop. It will cause an accident if this is not fixed. Also engines keep blowing up with out the hybrid system your truck will not have any port to get over to a safe spot on the road. Toyota is replacing engines but the ones they are putting in the trucks are the same ones that are failing.

When starting to accelerate, the automatic transmission shifts hart into second or third gear, and sometime has no power, like it is idling for 5 to 7 seconds before engaging. The problem has been reproduced and confirmed by Quirk Ford of Augusta, Maine who said I must have had a faulty transmission from the factory. The safety issue is hesitation from the occasional loss of power during the har
... d shift which could put one in danger of being rear ended. The were no warning indicators except the sudden jarring during hard shift. Other than the problem being diagnosed by the dealer, it has not been inspected. Hard shift first appeared around 75,000 miles. Dealer flushed and Changed fluid. Reappeared with loss of power during around 130,000 miles. Transmission being replaced 12/15/25 at $9200. Ford basically said it was my problem. I have had two ford rangers before that I drove for over 230;000 miles each without any transmission issues. Clearly the 10 speed transmission is flawed and needs a recall.Read more

When accelerating from a stop it has a severe delay. I’ve read now this is not normal. Sometimes the delay can be 1-2 seconds causing confusions at like stop signs and such as I’ve pressed the gas and get nothing. Not reported to dealership but will on next oil change as this has been a persistent problem since purchase.
