When I pulled into a parking lot, it smelled like my engine was on fire, and dark smoke was coming from my rear exhaust pipe. I called a flatbed tow truck and had the car returned to the dealership. Mileage: 52,285 GMC 2021Yukon Denali XL 6.2L V8 engine Well-maintained with regular oil changes. The dealership told me there was a lifter failure and that the engine could be damaged and would need to be replaced if the cam, pushrods, etc., were damaged. They also told me I was out of warranty by less than 60 days. The dealership said my warranty expired in October 2025, even though my vehicle is a 2021 with only 52,285 miles. They just informed me that the left lifter was replaced at 6,000 miles and that they will replace the right lifter. I am reporting a failure by Volkswagen Group of America to provide a safe and timely remedy for Safety Recall 23V-215. Per the manufacturer's notice, the front passenger seat is unsafe for use until repaired. I physically surrendered my vehicle to Merced VW on Dec 29, 2025, due to this grounding safety risk. VW Corporate has denied reimbursement, citing a '3-hour' labor time, while ignoring the actual grounding period caused by parts backorders. My 2020 Toyota Highlander Hybrid has a braking issue identified in Toyota Service Bulletin T-SB-0059-23, Hybrid Regenerative Brake Transition Feeling Improvement. When going downhill and applying the brake, the vehicle lurches forward temporarily as the braking system transitions from regenerative to hydraulic braking. The first time my vehicle did this, it was very alarming and I had to take addi
... tional measures to slow my vehicle as I was driving downhill. My vehicle frequently demonstrates this problem, however, I did not report it to my local Toyota dealer because I read in an online chat group that it was a characteristic of this vehicle. Although the TSB recommends a Reprogram of the Brake Actuator Assembly Control Module, the repair is only covered under the Toyota Basic Warranty which is in effect for 36 months or 36k miles, whichever occurs first. My vehicle is past the warranty limit, and because I did not report it to my dealer previously, this repair is not covered under the TSB or the Toyota Basic Warranty. Toyota Highlander Hybrid model years 2020-2022 are affected. The Technical Service Bulletin was issued August 29, 2023, so if a 2020 model Highlander Hybrid has this problem, 2020 models would have already passed the warranty limit by the time the TSB was issued and thus would not be covered. This is a safety risk as drivers can lose control of the vehicle when braking, particularly when going downhill. The Warranty should be extended, or this problem should be elevated to a RECALL so that older affected vehicles, or those with higher mileage can be covered under warranty. Drivers should not have to pay for a recognized problem that affects the safety of this vehicle and has been identified in a Technical Service Bulletin. This problem puts drivers at risk of losing control of the vehicle and the problem can be mediated with a software reprogram. T-SB-0059.23 should be elevated to a RECALL, or extended regardless of age or mileage.Read more

The front trunk (frunk) on my 2026 Tesla Model Y Juniper lacks both internal lighting and an emergency safety release mechanism. This design flaw creates an unnecessary and severe safety risk, as a small adult or child could easily become trapped in the frunk with no way to signal for help or escape. In low-light conditions—such as at night or in a garage without adequate illumination—the abse
... nce of a light would exacerbate disorientation and panic for anyone trapped. Without an internal release latch or glow-in-the-dark handle (standard in many vehicle trunks to prevent entrapment deaths), this could lead to a life-threatening suffocation or injury situation, especially if the frunk is accidentally closed on a person during loading/unloading. I discovered this issue while inspecting the frunk during routine cleaning and noticed that my four-year-old sat very comfortably inside the frunk listening to music while I cleaned the car. No incident has occurred yet, but the potential for harm is clear and warrants immediate investigation, as it violates basic entrapment-prevention standards observed in rear trunks and other vehicles. This defect compromises occupant safety and should be addressed through a recall or a retrofit that includes an illuminated emergency release. Please investigate similar complaints for this model year.Read more

On 11-20-25 while driving vehicle home a loud noise was heard coming from rear of engine. After review and consultation with automative technician it was thought best to park truck in safe location at house and have towed to dealership as engine noise identified aligned with other reports of thrust bearing failures associated with this engine. Driving engine further was considered unsafe if furth
... er damage occurred the engine could seize up while on highway. The Dealership tested engine on 11-21-25 in accordance with GM technical service bulletin and confirmed a thrust bearing failure had occurred. A replacement engine and turbo were submitted for order, but not available to due to back order of these parts due to large number of failures of same engine this year. Only the GM dealership that I purchased car from has inspected vehicle at this time and there were no check engine lights in vehicle when failure occurred. I have since identified the GM technical service bulletin was published in September of 2025 and the Duramax 3.0 has had numerous failures throughout 2025, prior to my purchasing vehicle at end of October. GM manufacturing misrepresented the quality of vehicle during the new vehicle purchase and did not disclose the significant number of engine failures that have occurred this year that could have left me and my family in an extremely unsafe situation. Additionally there has been no communication on how the replacement engine equipment manufacturing has resolved the issue that created the initial failure and I am concerned of driving vehicle in future as the engine failure may occur again. NHTSA needs to record this incident and request to GM failure rate data this year and investigate GM for selling vehicles with known manufacturing issues that place driver/passengers in an unsafe condition when vehicle looses power while driving.Read more

I am submitting this complaint regarding Safety Recall 25TA12 (Interim Notice 25TB12) for my 2025 Toyota Sienna XLE Woodland Edition, VIN available upon request. Toyota has acknowledged that the second-row seat rails may have improper weld penetration, creating a risk that the seats may lose structural integrity in certain high-speed collisions, increasing the risk of injury. Toyota has issued wr
... itten instructions advising that no one should sit in or use a child restraint in any second-row seat while the vehicle is in motion, and has also issued a stop sale, indicating Toyota’s determination that the vehicle should not be sold or delivered in its current condition. There is no repair remedy available, no parts, and no estimated timeline for correction. Because this vehicle is used to transport four young children in car seats, Toyota’s instructions render the vehicle unsafe and unusable for its intended purpose. I have complied with Toyota’s guidance and removed the vehicle from service. The vehicle has been out of service continuously since October 8, 2025 through today, December 30, 2025, totaling 84 calendar days and 60 business days with no repair attempt possible due to Toyota’s inability to provide a remedy. Toyota has stated that a loaner vehicle substitutes for safe seating; however, loaners do not correct the underlying defect, do not restore the vehicle’s certified safety condition, and do not eliminate the ongoing risk or loss of use of the purchased vehicle. Toyota continues to sell similarly equipped vehicles while leaving affected owners without a path to repair. I am requesting NHTSA review whether Toyota’s handling of this recall—including extended delays with no remedy, reliance on stop-use instructions, and indefinite loss of safe use—meets federal safety obligations and adequately protects consumers and child passengers.Read more

What happened : The vehicle experienced a sudden electrical failure and loss of normal shutdown control. The vehicle would not start or turn over. After repeated attempts, it eventually started, at which point all dashboard lights began flashing uncontrollably. The engine continued running even after the ignition was turned off and the key was removed. The only way to shut the vehicle down was to

The vehicle was taken to an authorized dealership for inspection. Diagnostics confirmed a failure of the Body Control Module (BCM). The Steering Column Control Module (SCCM) was also found to be non-functional and may have been affected by the BCM failure. The vehicle remains inoperable and is awaiting replacement of the BCM, which is currently on backorder with no estimated availability. The failed components are available for inspection upon request. The issue has been confirmed by the dealership. No crash occurred, but the malfunction posed a potential safety risk due to uncontrolled electrical behavior and inability to shut down the vehicle normally.Read more

Per my mechanic (fyi: 56k miles on vehicle): I test drove the vehicle and can hear a noticeable whine type noise when accelerating the noise goes away when getting off throttle. I put it on lift and the noise is coming from the transmission pump and sounds like cavitation. I checked the fluid level and it was overfull, very dark and has some very fine metal debris in the fluid. Something is coming
... apart inside the transmission has caused a catastrophic failure. The transmission will need replaced. I called Toyota to check availability and these transmission are on a restricted order. The dealer would need to call corporate Toyota to see what is needed to even order one. Usually a restricted order means they are on the edge of a back-order or having problems with the part and are working on a resolution or update. I did not see any recalls, service campaigns, or technical service bulletins for this transmission, but it sounds like the 2017-2021 V6 AWD UA80F transmissions are having issues. It is a long shot, but given the low mileage, you could try contacting Toyota to see if there is anything they can do for you. I know the factory warranty is 60 months or 60,000 miles on the powertrain. You are out on the time, but if they are having a known issue, then maybe they will help you.Read more
