The 2026 toyota suddenly accelerated when pulling into a handicap space. Suddenly the vehicle jumped the concrete barrier and sidewalk and hit a building. The airbags did not deploy and the seat belts came off as normal. The building was not damaged but my wife sustained a shoulder injury and whiplash. The vechicle was towed from the scene and is at Collison Center where the collison occurred.

The outcome could have resulted in a life altering injury or death. The problem has not been reproduced as the vehicle is not driveable. The vehicle was purchased the 14 miles on it. As of today 1/16/2026, we are not aware of any vehicle inspection. Our insurance company told us they would not pay for the download of the black box but we could if we would like. We decided to hire a person to download the black box and give us the results of the download. This is to occur on January 20, 2026. There were no warning indicators prior to the sudden acceleration. On December 4th I noticed a rattling sound when I was driving. It worsened when my car would rev above 2RPMs and before shifting. It also would make a terrible grinding sound when I started the car. I had about 1/4 of a tank of gas and thought that maybe that was part of the issue. After filling up, the sound was not as loud, but was still present when I started the car and upon accelerating.
... I took my car to my Acura dealership on December 5th. Within an hour of having the car they called to tell me that the engine needed to be replaced due to a connecting rod failure. They told me that the car is no longer safe to drive and that the repairs would be required in order to continue to use the vehicle safely. There was no warning sign. My only other concern is that the downshifters of this vehicle have never worked properly. I have mentioned this to the dealership multiple times and even drove the vehicle with their representative to recreate the issue. The car would accelerate and lurch forward upon utilizing the downshift feature. This would also cause the vehicle to over-rev which I am concerned was problematic to the connecting rod failure. Additionally, the vehicle has always made a grinding noise upon starting the engine, but the dealership assured me this was normal. Acura is unwilling to cover the full cost of replacement even though this vehicle has been maintained per the manufacturer's recommendations. The contact later sought treatment from her doctor, where an MRI was performed, which showed that the contact had suffered a concussion. The MRI also showed that the contact had suffered a traumatic brain injury, as white matter changes were found in her brain. In addition, the contact was informed that parts of her vertebrae were also damaged, which required surgery. The surgery involved a bone fusion of bone C5 to C6 and a disc replacement of bone C4 to C5. The contact mentioned she was a registered nurse for 25 years, and due to the severity of her injuries, she can no longer return to her profession. The contact's husband was also treated by his doctor for a dislocated right wrist. The injuries of the other driver were unknown if any; however her father, who was a police officer, picked her up from the scene of the crash. A police report was filed. As a result of the accident, the vehicle was totaled and was towed to an independent wrecking yard. The dealer nor the manufacturer had yet been notified of the failure. The vehicle was destroyed. Seat Belt – Rear Center Passenger The rear center passenger seat belt upper cover can be easily pulled off by passengers, which breaks the plastic retaining tab. When the tab breaks, plastic fragments fall into the seat belt mechanism, causing the belt to jam and fail to retract properly. This results in impaired seat belt function, which is a safety concern for rear passengers. The vehicle has not been modified, and the failure occurred during normal use. Vehicle mileage at time of incident was approximately 150,000 miles. The rear center passenger seat belt issue has been present since the vehicle was new. The rear center passenger seat belt mechanism was replaced twice by the dealer during the warranty period, with one replacement performed as a goodwill repair after Toyota Corporate declined coverage. Despite these repairs, the same issue reoccurred, indicating a persistent design-related vulnerability rather than isolated misuse. My vehicle has experienced repeated and unresolved software/firmware failures that cause loss of essential vehicle functionality and repeated vehicle immobilization. Since purchase, the vehicle has been taken out of service multiple times for software-related repairs, including firmware updates and system resets, yet the problems persist and the vehicle cannot be relied upon to operate normally.

After service visits, the vehicle is often returned with the same or new software-related issues, requiring additional service visits and extended downtime. The cumulative time out of service now exceeds 30 days. An authorized Polestar dealer principal, who also serves on the brand’s national leadership board, explained to me that U.S.-spec vehicles are running materially different firmware than vehicles sold in other markets. According to this explanation, Chinese-developed firmware was removed for U.S. regulatory reasons and replaced with U.S.-developed firmware that was not fully validated at the time of sale. As a result, U.S. customers are effectively acting as beta testers for unstable software. Because modern EV software controls safety-critical vehicle functions, a vehicle that repeatedly fails, becomes immobilized, or cannot be reliably restored to service after software updates presents a safety and reliability concern. My vehicle all of a sudden showed a "power steering assist fault," luckily it was parked. This is a problem with the 2012 Ford Fusion and I called Koon's Ford in Baltimore to see if this would be covered since my vehicle has never had steering issues before. They said yes, but after a couple days they called to tell me that Ford denied the claim since it is outside of the ten year warranty window.

The software for the ProSystem is flawed. When using the "lane keeping" part of the software, the vehicle, during a curve, will go to the left or right solid or dotted line, as appropriate, and exceed that limit by at least 2 feet, then sharply turn and go to the opposite solid or dotted line and exceed that limit by at least 2 feet. When driving in a straight line with either solid or dotted lines delineating the lane, the steering wheel will continues to oscillate to a minor degree. I have turned the system off and do not use it. I have returned the vehicle to the dealer, Round Rock Nissan, Georgetown, TX for repair and was told the system is working correctly and do modifications are necessary. Opinion: In my opinion, the software for the ProSystem is flawed causing the vehicle to be unsafe. The software need to be recalled, rewritten, real world tested extensively and reinstalled in all vehicles equipped with this software. This should be done immediately. I am reporting a safety-related defect involving the 10R80 transmission in my 2019 Ford F-150. The vehicle experiences erratic shifting and loss of power that appears to be caused by a known defect in the transmission’s CDF clutch drum assembly. Ford has issued TSBs for this problem (including TSB 25-2126), which describe axial movement of the CDF drum sleeve leading to hydraulic pressure loss.
... However, this issue has not been addressed through a recall. My vehicle exhibits symptoms consistent with this defect, including: •Harsh and unexpected shifts •Delayed engagement when accelerating •RPM flares and slipping between gears •Loss of power or hesitation during passing, merging, or turning •Sudden or unpredictable shift behavior that affects vehicle control These symptoms occur at random and create unsafe driving conditions, especially when entering traffic or making left turns. The loss of power and inconsistent shift timing can leave the vehicle unable to accelerate when necessary, which creates a potential crash hazard. This appears to be a widespread issue affecting Ford vehicles equipped with the 10R80 transmission from multiple model years. Many owners have reported the same symptoms, and transmission specialists have documented a design flaw in the original CDF drum, where the internal bushing moves out of position and causes hydraulic circuit failure. Ford has quietly released a revised CDF drum, which further indicates that the original design may be defective. Because this defect can cause unpredictable loss of power or abnormal shifting that affects driver control, I believe it constitutes a safety risk and should be investigated. A TSB does not ensure repair unless a failure is already happening, which puts drivers and the public at risk. Throttle hesitation. Seems to be a widely experienced issue with many third gen tundra owners. Press the gas, expecting the truck to drive, but it just lags a few seconds, then will finally engage and begin driving. Has put me in some close calls in traffic.

Continue getting a 1-2 second delay when accelerating from a dead stop after pushing on gas pedal with normal acceleration- not just an “lead foot” acceleration trying to quickly clear my intersection & merge into traffic. Happens when engine is at normal operating system or just pulling out of my neighborhood onto a 45mph road on a normal temp day.

While pulling out into traffic, my truck will suddenly hesitate. It also caused a vehicle to hit me as the truck pulled out into the lane, then suddenly completely lost power. Dealer was unable to replicate. Has happened to me several times when at a stop light. This is slightly safer as traffic behind me is stopped or slowed. It feels like a throttle issue as it completely looses power
