Incident date: Jan 30, 2026
The vehicle has been brought to an authorized dealership for numerous service visits related to electrical system issues that ultimately affected the braking system. During one service visit, the vehicle was returned to me with multiple active dashboard warning lights illuminated related to the braking system, including Electronic Stability Control and Automatic Emergency Braking. I questioned th
... e presence of these warning at pickup and was advised by the dealer that the vehicle was safe to drive and that the warnings would clear on their own. After driving the vehicle for a short period, I became uncomfortable continuing to operate it with brake-related warning lights still active and returned it to the dealer. Upon further diagnosis by a service technician, it was discovered that the vehicle has significant brake system defects requiring replacement of major components, including the brake control module and master cylinder. During diagnosis, the brakes locked and the parking brake would not disengage, rendering the vehicle undrivable. No collision or injury occurred. However, the vehicle was released to me with active brake system warnings and later found to have serious brake defects. Due to the safety-critical nature of the braking system and the repeated electrical and brake-related service history, I am reporting this for documentation and review.Read more

The vehicle has experienced repeated electrical and engine-related malfunctions, including random engine shutdowns while driving and random no-start conditions. The thermostat and cooling system failed and were replaced in 2024. Multiple sensors have failed. The braking system remains noisy even after brake replacement. The lane detection safety system does not provide audible alerts. The vehicle
... is currently operational and available for inspection upon request. How was your safety or the safety of others put at risk? The vehicle has randomly shut off while driving on both highways and city roads in Azle, Texas. This occurred while my two children were in the vehicle. When the engine shut off, there was a risk of losing power steering and braking assist, which could have caused an accident, especially at highway speeds and in traffic. Has the problem been reproduced or confirmed by a dealer or independent service center? The thermostat and cooling system failure were confirmed and repaired by a service center in 2024. Electrical and sensor issues have occurred repeatedly but have not been fully diagnosed or resolved. Brake noise persists despite replacement. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? UNKNOWN. The vehicle has been serviced by repair facilities, but the manufacturer has not provided a definitive diagnosis for the engine shutdown and electrical failures. Were there any warning lamps, messages, or other symptoms prior to the failure? When did they first appear? Yes. Various warning lights and symptoms appeared prior to failures, including cooling system warnings and sensor alerts. Electrical issues and battery failures have occurred repeatedly, with batteries lasting only 2–3 years. Symptoms began after purchase and have continued throughout ownership. Additional details The brakes have been noisy even after replacement, and the lane detection system does not produce audible warniRead more

I am reporting a safety defect involving the front passenger door lock actuator on my 2021 Toyota RAV4 XLE Hybrid. The front passenger door will not lock or unlock using the key fob or the interior power door lock button. The door must now be locked manually. This defeats the vehicle’s automatic locking system and creates a safety and security concern. This condition prevents all doors from be
... ing secured properly when driving or when parked. A passenger may believe the door is locked when it is not, increasing the risk of unauthorized entry, theft, or passenger safety issues. The failure occurred during normal use and without warning. The vehicle has been properly maintained. I have learned that multiple Toyota RAV4 owners have reported similar door lock actuator failures, indicating a potential widespread defect. This issue occurred shortly after the factory warranty expired. Due to the safety implications, I believe this issue warrants investigation and possible recall or manufacturer repair program. “This failure occurred without warning and with no dashboard alerts.”Read more

Windshield cracked approximately 2” from the bottom all the way across while using the defroster to clear ice from it. I was still parked at the time. I am reporting a safety-related defect involving unintended vehicle movement on my Hyundai Ioniq 5 (Model Year 2023, SEL AWD). Observed behavior: With the vehicle in Drive and the Auto Hold function active (indicated by the white “AUTO HOLD” indicator in the instrument cluster), I brought the vehicle to a complete stop by pressing the brake pedal. Auto Hold engaged (text now green), the vehic
... le remained stationary, and I removed my foot from the brake. After approximately three minutes of remaining stopped, a driver-assistance system error message appeared on the instrument cluster. At that point, Auto Hold disengaged (the “AUTO HOLD” indicator disappeared), and the vehicle began to creep forward on its own. I did not command acceleration, did not press the accelerator pedal, and did not intentionally release the brake. The movement was unexpected and created a safety risk. The first occurrence happened in a school drop-off line with the vehicle stopped near other vehicles and pedestrians. I have since reproduced the behavior and captured it on video. Related observations: Similar driver-assistance system errors have occurred while the vehicle was in Park and in Drive, with or without Auto Hold active. In those cases, the vehicle did not move because it was either mechanically locked or I had my foot on the brake. In all cases, Auto Hold deactivated. Safety concern: This behavior raises concerns about the reliability of Auto Hold and its interaction with braking and driver-assistance systems, particularly after the vehicle has been stationary for an extended period. Unintended forward movement presents a significant safety hazard. I am reporting this issue for investigation of potential defects in software, sensors, or control logic. Video evidence is available.Read more
