My car gave me a "Check electrical vehicle system" message, which I have seen online may mean a problem with the ICCU. As the dealership is close - about five miles from my home, on residential roads - I decided to drive there. The car lost power very quickly and then died in the road shortly after leaving my driveway. Thankfully I wasn't on a busy street or an intersection and was able to safel

The engine in my 2026 Toyota Tundra has significant throttle delay/lag when its cold - even if I let it warm up - it still has significant delay in throttle response - the truck has poor light throttle response and then lurches (even with the most minute progressive throttle input). This almost caused an accident where I nearly got T-boned because the vehicle crept a bit and then suddenly lurched
... quickly. The vehicle was also hesitant to reach 60mph (even though it was warmed up) after about 10 minutes of driving the vehicle started operating normally. I have a concern for safety everytime I drive this vehicle.Read more

While traveling on a rough road I experienced a side curtain airbag deployment on our Ineos Grenadier. There was no impact or roll over. There was only one person in the vehicle, the driver, yet both the drivers side and passenger side airbags deployed along with the drivers seatbelt locking mechanism. The airbags never deflated, leaving the driver trapped in the vehicle. To exit the vehicle a
... knife had to be used to deflate the drivers side airbag. Over one hour later the passenger side airbag had never deflated and also required a knife to puncture it. If there was an actual roll over and water was involved, I fear that the occupants of the vehicle could not get out. Something is faulty with this system.Read more

Component or system that malfunctioned: The braking system on my 2022 Toyota Highlander Hybrid, specifically the transition between regenerative braking and hydraulic friction braking, appears to malfunction intermittently. During braking, the vehicle sometimes lurches or surges forward instead of slowing smoothly, especially at low speeds or when braking downhill. The vehicle is available for ins
... pection upon request. How safety was put at risk: The unexpected forward surge while braking increases stopping distance and creates uncertainty about braking performance, particularly in traffic or parking situations. This creates a risk of rear-end or low-speed collisions and reduces my confidence in the braking system. Problem reproduced or confirmed by dealer or service center: The vehicle was inspected by a Toyota dealership, and a Technical Service Bulletin (TSB) software update intended to address braking behavior was performed at approximately 50,000 miles. However, the braking problem continues unchanged after the repair. I had to pay the dealer to perform this TSB update. The dealer was initially reluctant to apply the TSB and did not associate my braking complaint with the known issue. I had to research the TSB myself and request that it be applied. The invoice notes that the dealer reproduced the issue. Vehicle or component inspected by manufacturer or others: The vehicle has been inspected by a Toyota dealership service department. It has not been inspected by police or insurance representatives, as no accident has occurred. Warning lamps or prior symptoms: No warning lights or messages appear when the problem occurs. The issue happens intermittently without warning and has occurred since the vehicle was new. The condition appears to be getting progressively worse. The issue seems more likely to occur when braking while the hybrid battery is near or at full charge, suggesting a possible problem in the transition between regenerative and friction braking.Read more

To the Office of Defects Investigation, I am submitting this formal complaint regarding NHTSA Recall 22V703000 (Kia Recall SC249) involving my 2020 Kia Sorento (VIN: [XXX] ). This recall identifies a fire risk due to a potential electrical short in the tow hitch harness module. The recall notice specifically warns that a vehicle fire may occur while parked or driving. After receiving the recall
... notification, we made multiple attempts to schedule the required repair with Pompano Kia, the authorized dealership in our area. Despite repeated efforts, we were not provided with a confirmed repair appointment, and the recall remained incomplete. On [1-19-2026], before the recall remedy was performed, the vehicle caught fire while being driven on a highway in Florida. The vehicle became fully engulfed and was declared a total loss. Law enforcement and/or fire services responded. The incident appears consistent with the fire risk described in the recall. Following the incident, we visited the dealership to report the fire and the unresolved recall. We were told the matter would be reviewed and that we would receive a follow-up call. No follow-up was received. Attempts to contact the dealership through publicly listed email addresses resulted in delivery failure notices. According to NHTSA records, this recall affects tens of thousands of vehicles nationwide. We are concerned about: • Timeliness and availability of recall repairs • Dealership accessibility and communication • Risk to consumers when recall remedies are not completed before failure occurs We also attempted to consult several attorneys regarding this incident. We were advised that due to the volume of recall-related fire cases involving this manufacturer, representation may not be feasible unless severe bodily injury is involved. This further raises concern about the scale and seriousness of the issue. We respectfully request that NHTSA review: • Whether recall 22V703000 is being remedied INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATIONRead more

Component: Driver-side door latch and lock assembly. The driver-side door latch appears to have failed, preventing the door from opening from both the inside and outside and creating an occupant entrapment risk. The failure is fully reproducible and ongoing. The condition first occurred without any prior warning signs, messages, or symptoms and has persisted for more than a day without resolving
... . Although the vehicle responds to unlock commands from the key fob, keypad, and interior door controls with normal audible confirmation, the driver door frequently remains unable to open. Failure behavior includes: •The driver door cannot be opened from inside the vehicle, preventing occupant exit. •The door also cannot be opened from the outside during these events. •An occupant became trapped inside and had to crawl across the vehicle and exit through the passenger side. •The vertical lock indicator moves between locked and unlocked positions, but the latch does not release. •The electronic systems appear functional, yet the mechanical latch does not respond. •Using the physical key can sometimes force the lock cylinder past a mechanical click point, temporarily allowing the door to open; however, this places the mechanism into abnormal states where the door may remain permanently unlocked or, when turned in the opposite direction, cannot be opened at all regardless of unlock attempts. •All other vehicle doors operate normally. Video evidence documenting the failure is available. This condition presents a serious safety concern because occupants could become trapped inside the vehicle during an emergency or after a crash, delaying rescue. The latch mechanism is a critical safety component that consumers cannot reasonably inspect, predict, or maintain. Given the life-safety implications of a door that cannot be opened from either side, this issue warrants investigation for potential defect trends.Read more

I am an automotive engineer. I have been involved with vehicle development programs in the disabled vehicle conversion community for over two decades and have numerous patents in that space. I have been involved with vehicle testing and calibration as well. My vehicle demonstrates a SIGNIFICANT HESITATION when the accelerator is pressed that is not consistent. I live in a roundabout dense l
... ocation where timing an entry into a roundabout is easy using a normal vehicle. The 1 to 2 second hesitation where there is throttle input and no action at all from the vehicle (engine does not rev, no motive power observed) is very dangerous. I took this up with Toyota and my vehicle was checked out and "learning transmission" memory was wiped clean with me being the only driver was the remedy. The issue got better for a short time but is back and this morning I pulled out thinking there was plenty of time in front of a bus a good distance away and the lack of motive power for a full second or two resulted in a near miss. This vehicle has about 20,000 miles and was purchased by me new. There are NO WARNING LAMPS, and no DTCs thrown when this happens. I have seen several videos where people cite the same experience and a website where there is a class action lawsuit in place regarding this issue. This is completely separate from the engine recall issue. I feel as though this issue is a significant safety risk.Read more

I asked why not and was told that it depended where the Jeep was made. They also told me that the repair will be at my expense. This doesn't seem fair to me because the recall is a safety issue to replace the instrument panel cluster free of charge. A bolt securing the rear subframe assembly is loose and missing its nut. This condition has caused damage to the rear subframe assembly, which now requires replacement. The vehicle was inspected by a licensed dealer who confirmed on video that no impact damage, scraping, bending, or collision damage is present on any surrounding components. The only affected area is the bolt and nut connection poi
... nt. The dealer's professional assessment is that this is consistent with a factory assembly defect — either the nut was never installed or was not properly torqued during manufacturing. No routine service or alignment would require access to this fastener. Tesla's service records for this VIN should confirm no prior work has been performed in this area, meaning the fastener has not been touched since the vehicle left the factory. Factory-installed suspension bolts of this type are typically secured with a locking mechanism or thread locker; a properly installed nut does not detach through normal vehicle operation. This constitutes a safety defect. A rear subframe fastener that was not properly secured at the factory could lead to suspension component failure, loss of vehicle control, and a serious accident. Tesla's service center has denied warranty coverage for this repair despite the vehicle being within its warranty period, citing that "a manufacturing defect would have presented itself earlier" and that because I am not the original owner, they "don't know what might have happened." Tesla quoted $2,812.49 to replace the damaged rear subframe assembly.Read more
