I own a 2018 Toyota Camry XSE. The car is equipped with a radar cruise control and automatic braking system that will brake the car if programming believes there is risk of an accident. The problem is this is all front-based monitory and no rear based monitoring. Recently I was traveling the speed limit on a 4-lane highway when a car stopped abruptly in the turn lane in which I was following.
... There was no need for my car to brake abruptly as I was not traveling so close behind the car in front of me to justify that. Rather, I was more concerned about a car behind me that was tailgating me. I wanted to brake gradually to give the car behind me time to stop. But the auto braking system in my car braked so suddenly against my will causing the car behind me to veer off the road onto the shoulder in order to avoid rear ending me. It also made me worry that the driver of the car behind me might believe I intentionally braked hard to make him back away from my car when this was not the case. The 2018 Camry XSE auto braking system is a hazard because it looks forward with the radar but did not consider the car tailgating me and it nearly caused me to be rear ended were it not for evasive action of the car behind me veering onto the shoulder. I can turn it off when I drive the car but it always comes back on when I start the car. I am undecided about liability as well if I must permit this defective system to run or whether I am protected legally by turning it off. It is such a hassle to constantly turn it off each time I start the car so I just let the car do it's thing most of the time thinking I might be better legally protected with it on than with it off.Read more

Experiencing an ongoing issue where multiple warning lights appear on the dashboard at the same time. The lights include several safety and system warnings, making it look like many different systems in the vehicle are malfunctioning simultaneously. Even after repairs were made, the dashboard warning lights continue to stay on or return shortly after. It has become frustrating because the vehicle
... continues to show problems even when something has already been fixed. THIS IS A SAFETY CONCERN. How can we know if something is actually malfunctioning if codes are being thrown, constantly?? We also can’t use the safety features included with the vehicle when this is happening. Having so many warning lights on at once also creates concern about the reliability and safety of the vehicle, especially since several of the warnings relate to important systems such as braking, driver assistance features, and engine performance. Repairs aren’t helping and I’m seeing sooooo many other 2018 accord owners with the same issue, as has been documented with this site, NHTSARead more

I am filing this complaint to report a serious safety failure involving the service braking system and the failure of Forward Collision‑Avoidance Assist (FCA) and Automatic Emergency Braking (AEB) on my 2021 Kia Soul. During an imminent collision event, the vehicle failed to brake as expected, and FCA/AEB did not activate, despite conditions that should have triggered emergency braking. This cre
... ated an extremely dangerous situation and placed me and other motorists at risk of serious injury or death. The vehicle is currently subject to an active recall and has now been inoperable at the dealership for approximately one month under the lemon law process. The failure involved federally regulated safety systems, not supplemental or convenience features. I reported this incident to Kia America Customer Care, but the manufacturer has declined to conduct a manufacturer‑level safety investigation unless I personally pay for removal and shipment of the ACU/Event Data Recorder (EDR). Kia America has also declined to provide any transportation assistance while the vehicle remains unsafe and inoperable and has referred the matter back to the dealership and my insurance instead. Kia America has stated in writing that they rely solely on dealerships for diagnosis and repair and that they will not investigate further unless I incur additional costs. This refusal to independently investigate a braking system failure raises serious safety concerns, particularly given the potential risk to other drivers if this defect is not addressed. Due to the prolonged loss of use of the vehicle, I am experiencing significant financial hardship and risk to my employment, as I can no longer afford a rental vehicle. I am submitting this complaint so that NHTSA is aware of a potential defect involving brake system failure and non‑functioning collision‑avoidance systems on this model.Read more

NHTSA COMPLAINT NARRATIVE — SAFERCAR.GOV Vehicle: 2024 Tesla Model Y Component/System: Full Self-Driving (Supervised) Software, Version 14.2.2.5 On March 14, 2026, at approximately 5:22 PM, a 2024 Tesla Model Y was traveling on U.S. Route 3 in Franconia, New Hampshire, at approximately 45 mph within the posted speed limit. The vehicle was operating with Full Self-Driving (Supervised) software v
... ersion 14.2.2.5 actively engaged. Three occupants were present in the vehicle, including two minor passengers. All occupants were properly restrained with seat belts. Component/System That Failed: The Full Self-Driving (Supervised) system, version 14.2.2.5, failed to safely detect and respond to a snow-covered road surface. The system lost control of the vehicle upon encountering a snow patch on the roadway, causing the vehicle to strike a tree. The FSD system is the primary failed component. The vehicle has been towed and is available for inspection upon request. How Safety Was Put at Risk: The FSD system was in active control of the vehicle's steering, braking, and acceleration at the time of the failure. The system provided no auditory alert, no visual warning, and no driver takeover request prior to the loss of control event. The failure occurred without any warning whatsoever, leaving insufficient time for the driver to intervene and prevent the collision, despite the driver actively supervising the system in full compliance with Tesla's own supervision requirements. Three occupants were placed in immediate risk of serious injury or death. The vehicle struck a tree and was totaled. Emergency services responded and evaluated all occupants on scene. An official police report was filed. Prior Warning Lamps, Messages, or Symptoms: None. The FSD system issued zero warnings — no auditory alerts, no visual prompts, no haptic feedback, and no takeover requests — at any point prior to or during the loss of control event. The system was operating normally by all displayed indicatoRead more

Service required Following these alerts, the vehicle became undriveable and could not be shifted properly, remaining stuck in Neutral. The vehicle therefore could not be driven safely. The vehicle was previously taken to a Tesla service center after an earlier breakdown. Tesla reported cleaning corrosion during service, but shortly after the vehicle was returned, the problem occurred again with the same or similar safety warnings. This issue is concerning because multiple braking and safety appear to fail simultaneously, including ABS, braking assist, and restraint system alerts. GM used thicker oil to "cushion" a physical manufacturing defect in the crankshaft and bearings just to avoid the multi-billion dollar cost of engine replacements.
