I experienced an unexpected Loss of Motive Power (LOMP) event. Late afternoon traffic was fast, estimate 75-80 MPH. I attempted to change lanes get to the righthand shoulder and was very nearly involved in a high-speed collision with a semi-petroleum tank truck. He applied his brakes to avoid hitting me and started to jackknife, but regained control and continued. The truck missed by inches.
... At this point I had NO remaining power and was essentially coasting until I managed to cross the remaining two lanes of traffic to the shoulder emergency lane. I reset the error codes and continued home with no further occurrences. The incident produced the following warning lamps: Electronic Throttle Control Failure, Start/Stop System Failure and Check Engine, then a few seconds later the Limp Mode/LOMP event. I carry an OBD Scanning Tool with me to be able to clear the error codes when I encounter them. They are typically a cascading event that can result in LOMP. I have experienced a total of 26 of these error lamp events since 4/19/22 which have resulted in 3 LOMP's. The remaining 23 events I was able to reset before the inevitable LOMP. This is the first LOMP that nearly caused a fatal accident, the remainder I was able to safely find a place to stop to perform the resets. There is an active Manufacturer Recall Number93C, NHTSA Recall Number25V-586 that may cover this issue. The Recall Status is "Incomplete, remedy not yet available." The Alfa Romeo dealership has noted the P0191-00 error and replaced the fuel pressure sensor on 2/23/23, but this did not solve the problem. Owner's forums, and now the Manufacturer/NHTSA recall have reported this as a known problem, so I have been clearing my own error codes in order to at least have a car to drive. In my mind, this nuisance has now become a potentially very dangerous condition.Read more

Safety Complaint – 2019 Ford Edge Transmission and Engine Shutdown Issues To Whom It May Concern, I am submitting this complaint to report ongoing safety-related defects with my 2019 Ford Edge, which I purchased new. At 30,000 miles, the auto on/off stopped working and was not fixed. After approximately 50,000 miles, the vehicle began shutting off after refueling. At 68,000 miles, the tr
... ansmission started shuddering during acceleration. There was no warning light, I just noticed that something was wrong. This behavior is extremely concerning, as it creates a risk of sudden loss of power while driving and could result in an accident. Ford is aware of these issues but has not issued a recall. I paid to have the torque converter replaced; however, the same transmission problem has returned. Ford has declined to provide further assistance. Additionally, the check engine light is now illuminated, which Ford attributes to a software issue. Despite multiple repair attempts and clear safety concerns, these problems remain unresolved. I respectfully request that the National Highway Traffic Safety Administration review this matter, as the defects appear to be systemic and pose a potential risk to drivers. Thank you for your time and attention to this matter.Read more

My SUV pulls hard to the right when braking at speeds above 50 mph and much more on the interstates at 65 mph and above. I first experienced this after three weeks of ownership on a drive up I-95 on a trip to NC. It has happened numerous times while traveling on the interstates and has almost caused loss of control when having to brake hard when cut off by another driver or traffic suddenly coming
... to a stop. The vehicle has been to the service department numerous times....at first they suspected a braking issue and have had both calipers replaced, all brake lines replaced, lower control arms replaced and the right side strut replaced. None of these repairs have solved the problem. The factory service rep who took over the situation tested 3 other GMC 2025 Terrain suvs and all had the same problem. He reported the issue on a safety report to GM in Michigan and the response from GM was that it is a normal condition for the Terrain models. I have never owned, driven, or ridden in another vehicle in over 50 years of driving which performed like this, this is not a normal condition in my opinion.Read more

My 2023 Lincoln Aviator has a grinding noise when I back up. Also a rattle has begun at all speeds. I had vehicled check and the axles were found to be totally rusted. Prior to having it towed to the detail i did some research and found the following -- A 2023 Lincoln Aviator recall regarding a "rusted" or breaking axle generally refers to a serious issue where the rear axle bolt can fracture,
... causing the rear subframe to disconnect, potentially leading to a loss of control or rollaway . This issue often stems from stress corrosion, prompting recalls for subframe inspections and repairs. [XXX] +4 Key Information regarding 2023 Lincoln Aviator Recalls: Safety Risk: A fractured rear axle bolt can cause the axle to disconnect, causing sudden power loss, severe vibration, and increased crash risk. Scope: While often associated with the 2020-2022 Ford Explorer/Aviator, owners should check for 2023 model year issues relating to rear axle subframe integrity. What to Do: Visit the official Lincoln Recall Page and enter your 17-digit Vehicle Identification Number (VIN) to check for this or other active recalls. Repair: Authorized Lincoln Dealerships will inspect the axle bolt and rear subframe, performing necessary repairs at no cost to the owner. Incident date: Mar 18, 2026
The Transmission mount failed and the transmission was loose and clunking around in the vehicle when I applied the breaks and occasionally when I accelerated. The car has less than 50K miles and the transmission mount should not fail. There was no accident or rust and no reason for it to fail. I believe it is a safety hazard that Honda should do a recall or at least cover the cost of the repair.

I reported problems with my truck to Beck and Masten South GMC. I was put off a number of times with ridiculous excuses. My vehicle sat in the shop for a month to get my OnStar safety equipment repaired. I also had arguments with them regarding the oil light coming on and it burning excessive amounts of oil. I was told this was normal. I know better. Also, I needed to get it back into the shop for
... service and inspection for a squeeling noise under the hood. And to have them check the oul consumption. I was told it idle too often, and this was the reason oil was dissapearing. I was told they would put me on the loaner list so I could have means to get to my oncologist for cancer care. Well, I called numerous times because they never furnished a loaner for these issues I was told a third, then forth time I was next in line for a loaner so I could drop it off for repairs and recall work. After 8 months of these lies I finally went to Monument Chevrolet. Beck and Masten never intended to help me. Total I had tried and was told by newer service advisors, the manager, and GMC this would be remedied over 10 times in over 6 months. I finally gave up and moved on to never return to this dealer I had purchased two brand new GMC 1500 trucks from in a two year time frame. I did get my OnStar gateway repaired by replacement. It had stopped working again, shortly after getting it back. I kept getting put off. They stated it needed another Gateway for OnStar. I kept calling, and was told they would contact me ince it came in. I ended up troubleshooting the entire modem/router assy. I found inconsitant configuration parametrs. So, I reconfigured it all from start to finish after clearing the memory by taking battery cables off and allowing it to power down and clear memory. Then I did perform a reconfiguration and username/password change as well. I repaired it myself. The Onstar has never acted up again. These were my first issues. I am submitting this complaint regarding an active fire‑risk recall on my Mercedes‑Benz EQB and MBUSA’s refusal to provide a safety remedy. My vehicle was previously placed under a no‑drive instruction due to the fire‑risk recall. MBUSA attempted a remedy, but that remedy failed, and the safety impairment remains. Despite this, MBUSA Customer Care has declined to confirm or deny a safet
... y‑related repurchase and has stated that their goodwill options (loaner, rental, storage, and cash considerations) are their final position. These goodwill measures do not address the underlying safety defect. I have repeatedly asked MBUSA to confirm whether they are denying a safety‑related repurchase under the active fire‑risk recall, the prior no‑drive instruction, the failed remedy attempt, and the ongoing safety impairment. They have refused to answer this question and have stated there is no further escalation within their department. I am requesting NHTSA’s assistance in reviewing this matter, as the manufacturer has not provided a safety remedy and is refusing to address the safety‑related repurchase question. I am reporting a Failure of the Remedy for NHTSA Recall 23V634000 (Kia Recall SC281). The original recall attempted to "fix" a High-Pressure Fuel Pump (HPFP) issue via a software update to the Engine Control Unit (ECU). This remedy has failed. My vehicle is currently experiencing the exact safety hazard described in the recall: loss of motive power and stalling at various speeds due to HPFP failur
... e. Despite the software "patch" performed in 2024, the physical hardware (HPFP) has now suffered a mechanical failure, rendering the vehicle unsafe to drive. Description of the Incident: On or about March 15, 2026, the vehicle began lose power and stuttered while in motion, posing a significant risk of a rear-end collision. Diagnostic scans (OBD-II) confirmed code [Insert Code if known, e.g., P0087 for Low Fuel Rail Pressure]. A secondary diagnosis at Courtesy Kia (Mesa, AZ) on March 18, 2026, confirmed the HPFP has failed. Failure of Manufacturer to Act: The manufacturer (Kia America, Case #XXX) is currently refusing to replace the failed component under the recall mandate. They are categorizing the repair as "Goodwill" because the recall was "closed" by the unsuccessful software update. The manufacturer’s refusal to remediate a known mechanical defect that causes stalling in traffic is a direct violation of the spirit and legal requirement of the National Traffic and Motor Vehicle Safety Act. Following a 25-day repair at a certified collision center due to an earlier accident, the vehicle's Full Self-Driving (FSD) and Active Safety Systems began failing. The vehicle repeatedly failed to keep speed and failed to stay centered in the lane. On March 25, 2026, while traveling at low speed on a public roadway with FSD actively engaged, the system failed to maintain a stable path and incorr
... ectly steered the vehicle into a curb, causing property damage to the wheel. This failure put my safety and the safety of surrounding traffic at risk by executing an unpredictable steering maneuver that the active safety systems failed to prevent. The vehicle was taken to the manufacturer's service center twice with this specific complaint prior to the curb strike. The service center investigated and officially concluded that this was not a software issue, stating that they could do nothing else. They deferred the resolution back to the collision shop, citing likely camera/sensor physical misalignment from the 25-day body repair. The collision shop is now refusing to review the vehicle or inspect the hardware, claiming lack of visible exterior damage despite the internal service team stating this is a latent hardware alignment defect. The vehicle is currently in the same failing condition and available for inspection upon request. No warning lamps or error messages appeared on the screen prior to the failure or the curb strike. The symptom of unpredictable lane tracking first appeared immediately upon receiving the vehicle back from the collision repair earlier this year. Note: I have a formal data privacy request pending with Tesla to pull the exact chat logs showing their refusal to service this hardware defect. I will provide these to the investigator upon follow-up.Read more

Office of Defects Investigation National Highway Traffic Safety Administration 1200 New Jersey Avenue SE Washington, DC 20590 Re: Safety Defect Complaint — Ram ProMaster (2018–2025) — Cowl Water Intrusion into Engine Compartment To Whom It May Concern, I am writing to formally report what I believe to be a serious, systemic, and unresolved design defect in the Ram ProMaster van, affecting
... model years 2018 through at least 2025. I am requesting that the Office of Defects Investigation open or expand a formal investigation into this matter. Nature of the Defect: The windshield cowl on the Ram ProMaster is designed in a manner that permits significant water intrusion directly into the engine compartment. Water enters through the cowl seam and surrounding gaps, bypassing any drainage system, and flows onto critical engine and electrical components including but not limited to: the serpentine belt and pulley system, the engine fuse box, transmission components, and wiring harnesses. I brought my vehicle to two separate authorized Ram dealerships. The second dealer confirmed — based on testing conducted on multiple ProMaster vans dating back to 2018 — that this water intrusion behavior is consistent across the model line. In other words, the dealer has effectively confirmed that this is not a defect isolated to my vehicle, but a design characteristic of the ProMaster platform itself. This confirmation does not reassure me. It alarms me. A design that has permitted consistent water intrusion onto safety-critical engine and electrical components across a major commercial vehicle platform for over a decade is not an acceptable engineering standard. Given the size and weight of these vehicles — many operated commercially and carrying passengers or heavy cargo — the consequences of power steering loss, electrical failure, or mechanical failure are severe. We have tried dealing with Stellantis as well. Any help would be greatly appreciated. [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIRead more
