The contact and her family were unable to exit through the front door due to the fire and used the sliding kitchen door to exit the residence. The fire department was called to the scene. Per the contact, three fire trucks were at the scene. The fire department informed the contact that the fire went up to 3,000 degrees. The fire department concluded that the fire started from underneath the driver's back panel. The contact stated that the gas tank was underneath that area and believed the gas tank may have contributed to the fire. The towing company towed the vehicle to a towing yard, and it was later picked up and towed to an unknown location. The insurance company was notified and declared the vehicle a total loss. The manufacturer was made aware of the failure and advised the contact that an inspection was needed on the vehicle and asked for the location of the vehicle. Issue: There were no warning messages to indicate a problem with the fluid level until immediately before the catastrophic failure; we were able to get the car to the side of the road right before the wheels locked but had we not, a serious crash could have occurred. Toyota tells us there is no alert for fluids since they have designed the transaxle as a "closed system". However, conditions exist that could compromise this closed system so consumers (and service dealerships) should have a way to proactively identify a fluid level problem so it can be remedied to avoid potential safety hazards related to an abrupt failure.Read more

When I put the vehicle in reverse the backup camera is displaying a black screen. I looked up recalls and see there is a recall on RAV4’s for this very issue, but my VIN shows no recalls. I called service at Hubler Toyota in Greenwood IN and they confirmed my VIN is not included in the recall. I want to know why it is not included. I am taking it to Hubler tomorrow for diagnostic and repair.

6 speed manual transmission. 79K miles. Vehicle was checked at a dealer due to a sluggish clutch. Dealer found low brake fluid, but no problems with the clutch. The next day the clutch failed completely and car had to be towed off the highway. Slave cylinder for the clutch is a design flaw on these vehicles.

The instrument cluster (combination meter) failed completely and without warning, going entirely blank. This eliminated all speedometer, warning light, and indicator functionality. The failure began occurring intermittently before becoming persistent. No warning lamps or messages preceded the failure — the cluster simply went dark. The loss of speedometer functionality made the vehicle unsafe to

The loss of all warning indicators eliminated visibility into any safety-critical vehicle alerts that may have been active during the failure periods. The problem was confirmed by the dealership and is the subject of Safety Recall 25TA08 (Combination Meter May Go Blank). The vehicle was brought to a Toyota dealership where the recall was documented and the repair was eventually completed. The replacement part required VIN-specific production and programming by Toyota, resulting in an extended out-of-service period of nearly two months at the dealership, and approximately five months from the first occurrence of the failure. During the out-of-service period, the vehicle could not pass state inspection and was legally inoperable. The component has been replaced and is no longer available for inspection.Read more
