It has been occurring ever since I purchased the truck in October 2023. I finally thought I should say something. Usually, the truck is at a complete stop. The truck can be warm or cold. As I press the accelerator down to move in the forward direction, there is a hesitation. No immediate acceleration. It lasts for at least one second before the acceleration begins. This can create a serious sa

When I had my truck in for a recall on 4/19/26, I asked a Toyota service manager if this is a common problem. He told me it is very common in the 2024 Toyota Tundra, with a V6 twin turbo. This leads me to believe that the Toyota Corporation is aware of the problem. As per the service manager, Toyota has not done anything about this issue and there isn’t a repair for it.Read more

Vehicle Information Vehicle: 2024 Toyota Tacoma VIN: [XXX] ate: January 26, 2025 Repair Completed: October 1, 2025 Safety Complaint Description Beginning on or about February 5, 2025, my 2024 Toyota Tacoma developed a serious braking-related safety issue that substantially affected the safe operation of the vehicle. When I brought the vehicle to the dealership, I reported braking concerns and
... was advised by dealership personnel that the vehicle may not be safe to drive due to concerns that the rear brakes might not function properly under certain conditions. I was specifically told by dealership staff that they would not recommend driving the vehicle due to safety risks. Despite this warning, I was forced to continue operating the vehicle because no repair remedy was available at that time and no equivalent loaner vehicle or safe alternative transportation was provided. Over the course of several months, I repeatedly contacted the dealership regarding the issue. Each time, I was told that no repair remedy was available and that nothing could be done until Toyota released a repair solution. I was also advised that my only option was to leave the vehicle at the dealership indefinitely, which was not practical because the vehicle was my only means of transportation. As a result, I was forced to continue driving a vehicle that had been identified as unsafe. The defect remained unresolved for approximately eight months. During this time, I experienced significant safety concerns and loss of reliable vehicle use. Because of the prolonged delay in repair and lack of alternative transportation, I was ultimately forced to purchase another vehicle in order to maintain necessary transportation. The braking-related condition was not permanently corrected until approximately October 1, 2025. This situation created significant safety concerns because I was forced to operate a vehicle that dealership personnel advised may not be safe to drive, yet INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIARead more

While driving on a two-lane highway at approximately 80–85 mph and attempting to pass another vehicle, the engine suddenly shut off without warning. The vehicle lost all propulsion, and steering assist was significantly reduced, creating a dangerous situation and nearly resulting in a head-on collision with oncoming traffic. Warning lights including check engine and low power steering illuminat
... ed immediately after the stall. The vehicle would not restart for several minutes and was temporarily disabled in the roadway. The vehicle was later inspected by a dealership, which confirmed through diagnostic data that multiple engine stall events occurred at highway speeds. The inspection also identified system-wide electrical voltage drops and multiple communication faults between control modules. The dealership attributed the issue to a loose battery terminal; however, the diagnostic report indicates prior voltage irregularities and multiple affected systems, suggesting a broader failure condition. This incident is consistent with an active safety recall affecting this vehicle, which states that the engine may stall during driving. The failure has not been reproduced since the initial incident, but the vehicle has documented evidence of multiple stall events and remains a safety concern. The vehicle is available for inspection upon request. Due to the sudden loss of power at highway speed and the inability to safely control or accelerate the vehicle, this issue poses a serious risk to the driver and others on the road.Read more

Component/System Failure: The failure involves the engine cooling system, specifically the radiator, fan shroud, and oil cooling lines. The oil cooling lines are improperly routed or positioned, causing them to press against the plastic fan shroud. Over time, this pressure deforms the shroud inward, leading it to rub against the aluminum radiator and create pinhole leaks. The damaged components ar
... e available for inspection upon request. Safety Risk: This defect poses a serious safety risk due to sudden coolant loss and potential engine overheating. This can result in loss of power, engine failure, and unexpected breakdowns in traffic, increasing the risk of accidents, especially at highway speeds. Reproduction/Confirmation: This issue has been widely reported among 2023 Toyota Tundra owners and confirmed through visual inspection by owners and independent mechanics. The consistent nature of the failure indicates a design defect rather than isolated incidents. Inspection History: The vehicle has been inspected, and deformation of the fan shroud along with visible contact and wear on the radiator confirms the issue. Dealerships have acknowledged the condition but refused warranty repairs. Toyota has not issued a Technical Service Bulletin (TSB). Warning Signs/Symptoms: There are typically no warning lights or messages prior to failure. In some cases, coolant loss or overheating occurs only after radiator damage has already developed. Additional Statement: This is a widespread defect caused by improper routing or clearance of oil cooling lines. Despite numerous similar reports, Toyota has not issued a TSB or recall and has denied warranty coverage, leaving owners responsible for repairs related to an apparent design flaw.Read more

To Whom It May Concern, I am submitting this complaint to report a serious mechanical issue involving the transfer case on my 2022 Toyota Highlander. This problem raises concerns regarding the safety, reliability, and overall performance of the vehicle. While driving, I began hearing a grinding noise coming from the vehicle. Concerned, I brought the vehicle to an authorized dealership for a diag
... nostic inspection. I was informed that there is a transmission-related issue and that the vehicle requires a transfer case replacement. At the time of diagnosis, the vehicle had approximately 77,000 miles and has been primarily driven locally under normal conditions. This type of major drivetrain failure at such mileage is unexpected and concerning, especially given that the vehicle has been properly maintained. The failure of a critical component such as the transfer case can significantly affect vehicle control and drivability, posing a potential safety risk. This raises concerns about a possible defect in the drivetrain system or a broader issue affecting similar vehicles. I respectfully request that this matter be investigated to determine whether this is an isolated incident or part of a wider pattern. If necessary, appropriate corrective actions should be taken to ensure consumer safety. Thank you for your attention to this matter. I am available to provide any additional documentation, including service records and diagnostic reports, upon request.Read more

I am filing a complaint regarding an unresolved safety recall for my 2025 Toyota Sienna involving the second-row seat welds. VIN: [XXX] NHTSA Recall ID: 25V668 Recall Date: October 7, 2025 Manufacturer/Dealer Recall Reference: 25TB12 This recall involves the structural integrity of the second-row seat welds. ... a crash or sudden stop. As of today, more than six months after the recall was issued, no remedy has been made available. I have not been provided with a repair, a timeline for when a fix will be available, or an adequate interim solution. On February 25, 2026, I brought my vehicle to the dealership, and it was placed out of service due to this recall. I was provided with a smaller loaner vehicle, which is not comparable to my Toyota Sienna and does not meet my transportation needs. This situation has left me without use of my vehicle for an extended period of time while also dealing with a known safety defect. I request that NHTSA review whether the delay in providing a remedy is reasonable and compliant with federal safety requirements, and whether additional action is necessary to protect vehicle occupants. 2023 Toyota Sienna occasionally, without warning, "lurches" forward at low speeds when braking. ... ith the transition between regenerative braking (vehicle is a hybrid) and the hydraulic braking. I have spoken to Toyota, and they don't have any recall. People posting on online forums seem to believe there is a software update (T-SB-0047-24) that helps address the concern. Toyota charges to diagnose and apply software updates as the vehicle is out of warranty. I believe this to be a safety issue and something Toyota should cover. Based on my online search, there seems to be a lot of people with the same issue. This has been an ongoing issue the last couple of years and I'm thinking it should be reported. I have not brought this to a dealer to have diagnosed. These are largely family vehicles and my wife, and I don't feel very comfortable every time it happens.Read more

Incident Detail: In April 2026, my 2022 Toyota RAV4 Prime (VIN: [XXX] ) suffered a critical failure where the charging cable became physically locked in the vehicle’s high-voltage (HV) charging port. I transported the vehicle to Ken Garff Cheyenne (authorized Toyota dealer), where management allowed non-certified, non-technical staff to perform unauthorized physical interventions on the HV port.
... No manufacturer-certified technician was provided for a safe diagnostic. Safety Hazard and Immediate Consequence: Following these unauthorized interventions, my wife (an educated automotive technician) determined the vehicle was in a hazardous state, risking thermal runaway or electrical fire. This hazard was so severe that the owner of the property where I was staying in Cheyenne—a [XXX] dependent senior—was too frightened to allow the vehicle to remain on the premises. Due to this immediate safety risk and the dealership's refusal to coordinate a safe tow, I had to have the vehicle towed to a neutral facility to protect the life and property of a vulnerable widow. Legal and Technical Context: I am a permanent resident of San Diego, California, and this vehicle is covered under the California Song-Beverly Act. The vehicle is also subject to Federal Recall 23V478 (DC-DC Converter Fire Risk). The dealership’s unauthorized tampering with the HV system, combined with ambient temperatures in Cheyenne often falling below the 41°F safety threshold for this system, created an unmitigated fire risk that the manufacturer has failed to address. Request: I am seeking a federal investigation into the dealer’s failure to follow High-Voltage safety protocols. I demand that Toyota Motor North America provide a Field Technical Specialist (FTS) to perform a safe diagnostic at the neutral facility where the car is now located, as the dealership’s actions rendered the vehicle a public safety hazard.
