The harness webbing/strap on the Evenflo Revolve360 Slim tore without any known incident or accident causing the damage. The defect was discovered on April 15, 2026 when the seat was temporarily removed from the vehicle. Upon inspection, the harness strap was found to be nearly completely torn through and would likely not have restrained the child occupant in the event of a crash. The seat had bee
... n in regular use and had not been involved in any collision. No unusual event or misuse is known to have caused the failure. It is suspected that the rotating mechanism of the seat placed repeated stress on the harness webbing over normal use, causing it to deteriorate and tear. The seat has not yet been inspected by the manufacturer, law enforcement, or insurance representatives, but is available for inspection upon request. The seat has been taken out of service and is not being used.Read more

After I purchased my 2021 Toyota Tacoma TRD Sport with 4 miles on it and at 1,332 miles on 11/26/21, the Check Engine light with Traction Control Off light came on while I was driving. I immediately pulled over and on the truck media screen it read this: "A malfunction in the electronic control of the engine, throttle or automatic transmission has been detected. Contact your Toyota dealer and have
... your vehicle inspected." The truck was rough idling and had no acceleration. I had the truck towed to dealership where it was purchased and upon diagnostic, it was determined that #3-cylinder injector was reading "Open Circuit" with code P020313. Being under warranty the dealership repaired the bad injector. No further issues until 4/22/26. On 4/22/26 with mileage at 48,361, I started my truck in my driveway and immediately recognized that the truck was rough idling and the Check Engine Light along with the Traction Control Light Off being on. The exact same message displayed on the screen as above, "A malfunction in the electronic control of the engine, throttle or automatic transmission has been detected. Contact your Toyota dealer and have your vehicle inspected.". The truck was towed back to the same dealership and upon diagnostic the same issue of #3-cylinder injector "open circuit" with code P020313 was revealed. It was recommended to replace the bad #3-cylinder injector and also recommended to go ahead and replace all of the spark plugs. The total repair bill we be ~$2600 when I go to pick it up in 3 days. This is not just a coincidence that the exact same #3-cylinder injector has gone bad at 1,332 miles and then again at 48,361 miles. Seems to be a connection between the injector going bad and the issues that they have had with the High Pressure Fuel Pumps. Toyota needs to acknowledge this problem.Read more

Vehicle: 2024 Toyota Corolla Cross (gas‑powered, non‑hybrid) System/Component: Electrical System → Start/Stop System → Driver Information Display Issue Type: Design Defect / Driver Distraction I am reporting a recurring issue with my 2024 Toyota Corolla Cross (gas‑powered, not a hybrid). The vehicle frequently displays a “Battery Charging” message on the dashboard whenever I come to
... a stop at traffic lights or stop signs. This message appears even when the vehicle is operating normally and there is no actual charging system problem. According to Toyota documentation and owner discussions, this message is triggered when the auto start/stop system decides not to shut off the engine due to the 12‑volt battery’s state of charge. However, the message is unnecessary, cannot be disabled, and provides no actionable information to the driver. It functions as a non‑critical status notification but is presented with the same prominence as a warning. This creates a driver distraction, as the message repeatedly draws attention away from the road for a condition that is not a fault, not a safety issue, and not something the driver can address. The message appears frequently during normal driving and cannot be suppressed. I believe this is a design flaw in Toyota’s start/stop implementation and dashboard messaging. The system should not display a warning‑style message for normal operating behavior, especially when it cannot be corrected by the driver. I am requesting that NHTSA log this issue as a potential driver‑distraction concern and encourage Toyota to revise the software to eliminate or suppress this unnecessary message.Read more

While driving on the highway under normal conditions with no road hazards present, the engine under cover (part #51441-42210) detached from my vehicle and the bumper clips broke, causing both the under cover and bumper to become damaged and requiring replacement. There was no collision, no debris, no pothole, or any other road hazard involved. The failure occurred solely under routine highway driv
... ing conditions. Upon taking the vehicle to my Toyota dealership, the service team acknowledged this is a known and recurring issue with the bZ4X model. Research and owner forums confirm multiple 2023 bZ4X owners have experienced the identical failure — undercarriage cover and bumper detachment under normal driving and rain conditions. Toyota dealers have repeatedly told affected owners ‘this happens all the time,’ yet Toyota refuses to cover repairs under warranty, claiming it is not a manufacturing defect. I believe this represents a systematic design or manufacturing defect in the 2023 Toyota bZ4X. The undercarriage cover and bumper mounting system fails under normal driving conditions, which poses a safety risk to the driver and other vehicles on the road. I am requesting a formal investigation into this issue and urge NHTSA to consider a recall to protect other bZ4X owners.Read more

I have a 2021 TYTA RAV4 HYBRID XLE PREMIUM AWD, the driver seat is very slow moving forward and backwards twisting and preventing to fully go forward and backwards. ... the driver. I have passengers to consider. This is absolutely unsafe. This is allowed? Where is the recall? This is unlike Toyota to do nothing. I have platinum coverage purchased at time of purchase of this brand new vehicle on 12/31/2021. Apparently, it causes it other damage to other components and parts. There is alot of other components and parts under the front driver seat. I went it to have it looked at on March 30, 2026. Service advisor states: They ordered the parts its a seat frame sub assembly with motor. $1785.92 Covered by platinum Coverage, not all the parts are covered that are damaged. it will cost me: $448.87 for components and parts damaged but are not covered plus labor for Shield (71812-0R140-C0) and Holder (7181B-0R010) After further investigation it is a symptom of a failed internal component within this assembly—most commonly a stripped plastic gear or a bent drive shaft. So looking it up many Toyota Brand owners are experiencing the same thing and are having to replace other parts caused by this. This is a dangerous situation for me and others. What if it fails to move forward or reverse or motor stops? A partial repair is unacceptable. Leaving damaged trim and holders creates a risk of wire-chaffing and further mechanical stress on the new assembly. For the safety of the driver and the longevity of the vehicle, the repair must be 100% complete. This would be out of pocket monies families don't have. $2,234.79 total repair cost acts as a "safety tax" that prevents families from fixing this critical defect. I can not afford: $448.Read more

My 2010 Prius has experienced the following error codes: C12341 and C1391, prompting the dealer to recommend replacing the brake booster assembly and master cylinder for $3938.89. This is a known issue with this vehicle and a Customer Support Program (CSP) was issued back in 2018, stating that if this vehicle displayed these codes, the repairs would be at no charge to the customer. Toyota extended
... this coverage to 10 years from date of first use or 150,000 miles (whichever came first). I never receied the notice. The details of the CSP are below. (I found on the internet.) "Toyota has received reports indicating various brake system related warning lamps illuminate due to internal malfunctions. The Brake Booster and Brake Booster Pump are covered by Toyota’s New Vehicle Limited Warranty for 3 years or 36,000 miles (whichever occurs first). However, because we at Toyota care about each customer’s ownership experience, Toyota is now offering a voluntary Customer Support Program that applies to various brake system related warning lamps of covered vehicles as a result of internal malfunctions, regardless of whether the vehicle is out of warranty. The timing of any repair under this voluntary Customer Support Program is subject to parts availability. The Customer Support Program will be offered for all vehicles until November 30, 2019, regardless of mileage. In addition, the Customer Support Program will be available for 10 years from the date of first use, or 150,000 miles whichever occurs first." My vehicle has about 132,000 miles and just displayed theses codes. Since it is 16 years old, the CSP has expired. It being a known issue, and one related to such a safety concern, I emplore you to make this a formal recall rather than a mere suggestion of repair. Back in 2018, my vehicle was not displaying these codes so was ineligible for the repair. Upon further research I am finding hundreds of Prius owners with the same issue. Clearly this is a defect in manufacturing.Read more

I received a Toyota recall notification in Dec 2015 for a frame rust inspection, RO Number 307016. I have since noticed fairly major frame rust, with a long 2-inch by 7-inch opening on the inside of the frame below the driver-side seat. I took the vehicle to Toyota on 20 APR 2026 for a leaf spring recall #302870. I asked them to take a look and was told the recall 307016 had expired. The original
... recall inspection for rust either failed to identify it or the inspector was inadequate at identifying it.Read more

The manufacturer was made aware of the
... issue. The contact stated the power sliding door unexpectedly opened while driving on a local roadway. The vehicle was not repaired. The approximate failure mileage was 200,000. VIN tool showed no open recall.Read more

Frame is rotting away. The tacomas, tundras, sequoia were recalled for this issue. The tacomas frame is very close in design to the 4runner. The main difference is the 4runner uses coil springs whereas the tacoma used leaf springs. Toyota stated that because the 4runner was made in Japan unlike the others that were made in the USA, that the 4runners were not part of the recall.most of the 4runners
... that are no longer drivable, are because of rotted frames. The repair kits that fix the rotted frames on the tacomas are the same kits that fix the 4runners. Now toyota won't even touch my vehicle for other repairs because of the rot holes.Read more
