My engine shut off while driving down a highway. I had to coast into an uneven, steep ditch. If someone would have been behind me, it could have caused an accident. I have the 6.2 engine and followed the recall instructions. Dealer said motor was fine and around 7,000 miles later it failed. The oil weight change is not fixing these issues.

This issue matches the known defect related to the Denso fuel pump recall affecting many Toyota vehicles from similar model years. However, my VIN is not included in the recall. This is a serious safety concern because the engine can shut off unexpectedly while driving, which could lead to an accident. I believe this vehicle may have the same defective component as those included in the recall, even though it is not currently listed. I am filing a complaint regarding my inability to receive a safety recall remedy without first paying for a separate component replacement. Vehicle: Tesla Model 3 VIN: [XXX] My vehicle has an active safety recall related to hood latch functionality, which Tesla stated requires a firmware (OTA) update. Tesla informed me this update could not be installed unless the vehicle’s car computer (
... MCU/CPU) was replaced. Tesla classified this component as out of warranty and required me to pay $1,687.66 out of pocket (after a partial discount). Separately, my vehicle developed a charging failure requiring HV battery repair. Tesla confirmed in writing that they could not proceed with the HV battery repair or install required firmware unless the same computer was replaced. Tesla also confirmed that if I did not approve and pay for the computer replacement, they would not proceed with either the battery repair or the recall-related update. At the time, my vehicle was not drivable, and Tesla indicated the loaner vehicle would need to be returned if I did not approve the repair. This would have left me without a functional vehicle, leaving no practical alternative but to proceed. As a result, I approved and paid for the repair in order to restore vehicle functionality and receive the recall remedy. This payment was made under protest. Concern: This situation conditions access to a safety recall remedy on a customer-paid repair, creating a barrier to receiving a mandated safety fix. I request review of whether a manufacturer can require payment for a separate component in order to enable a safety recall remedy and essential repairs. I have written documentation supporting these statements. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

We will never know exactly why the Moon roof exploded. I tapped the glass moon roof extensively and we drove home. There was no warning. I was driving west bound on I40 with the adaptive cruise control, and lane assist on at 75 MPH. It was around 8:30 PM when I seen the instrument panel flash, just drop for less than half a second, and then come back up but dimmer, like when the headlights come on, except they already were on. At that point all engine noises stopped and the car felt like it was actively braking. I literally lurched
... forward in my seat. I noticed that the RPM's had dropped to zero and I didn't see anything on the dash that indicated any warning. All green icons and nothing red. Stepped on the gas pedal, nothing. Tried hitting the cancel on the adaptive controls, nothing. I started turning the car from the inside lane to get to the outside shoulder while shifting the car into neutral to get more drift and tried toggling the key from off to start position and the engine. No rev, no RPM's. Car stalls in the outside lane. I turned on the flashers and then put the vehicle in park, removed the key and then re-entered the key and tried to start. Nothing. Attempted to push the car out of the lane in neutral but I wasn't able to get it to move. A couple semi's go by and I about pooped my pants. Waited for highway patrol to come by and get the car towed. Very fortunate no one was hurt or killed. I believe the automatic emergency braking activated or the electronic parking brake activated? All of the interior systems still had power.Read more

The three diagnosis I have had done to check for multiple opinions have all come back the same with a collapsed AFM lifter causing camshaft damage, overall failure of an AFM/DOD deactiviating lifter in the 6.2L L86 engine leading to engine damage. It is available for inspection. The problem has been confirmed by multiple i
... ndependent service providers and confirmed to be an issue in ongoing litigation against GM. It was initially diagnosed by a mobile mechanic and then taken to a independent service mechanic who confirmed the initial diagnosis. It was then confirmed again by a third party independent mechanic. The engine light did not appear until the noise started and what I now know as when the lifters failed, at which after my vehicle alerted me with the engine light. When the problem first occurred, I was driving around 35 MPH down a public road, I had to immediately slow down and pull over. I then limped the vehicle to a safe destination where the mobile mechanic was able to check it out. Luckily, I was not going high speeds and the public road I was driving on was not busy so I was able to pull over safely upon the issue the start of the warning lamp flashing. The engine component failure would have been a much higher safety risk for myself and others if I was driving on a busy road or highway. I have spoke with both GM and Cadillac, they are aware of this defect, yet refuse to provide relief. I have been quoted $13,000-$17,000 to begin repairs to the engine.Read more

Safety Defect: Data Communication Module (DCM) Failure Affecting Hands-Free Microphone and Safety Connect / SOS Emergency System Vehicle: 2019 Toyota RAV4 Hybrid LE The DCM (which controls both the CarPlay/Bluetooth hands-free microphone and the Safety Connect SOS emergency button) suddenly failed without any warning or accident. Symptoms: - No one can hear me during CarPlay or Bluetooth calls
... (microphone inoperative). - Safety Connect / SOS light became intermittent or inoperative (directly impacts emergency calling, crash notification, and roadside assistance). This is a known recurring issue documented in Toyota TSBs T-SB-0112-20 and T-SB-0089-21. Toyota of Hackensack diagnosed it and quoted $1,300 for DCM replacement/update while charging me $200 for diagnosis. They did not disclose during the multipoint inspection that this is a documented safety-related DCM fault. I performed the manufacturer-recommended 12V battery disconnect reset (standard TSB procedure), which fully restored the microphone and returned the SOS light to solid green. However, this is only a temporary fix for a known defect. This failure compromises hands-free calling while driving and the emergency SOS system — both critical safety features. I request NHTSA investigation and urge Toyota to issue a recall or service campaign for all affected 2019–2025 RAV4 Hybrids.Read more

SUPPLEMENTAL TO NHTSA COMPLAINT #11713668. This is a supplemental filing regarding the 2020 Volvo XC90 T8 Inscription, VIN [XXX] . As of this filing, the vehicle has been without hybrid functionality for over four months. The replacement part remains u
... navailable. Since the original complaint, the owner initiated civil proceedings against Volvo Car USA LLC and Volvo Car North America LLC in the State Court of Gwinnett County, Georgia (Case No. [XXX]). Through that litigation, the following facts have been established as deemed admissions under Georgia law, which neither Defendant contested: 1.This vehicle is excluded from Recall 25V-179, yet experienced a catastrophic hybrid battery cell module failure. 2.Battery failures in 2019-2022 XC90 T8 vehicles are not exclusively caused by the "LG supplier process deviation" identified in Recall 25V-179. 3.Volvo is aware of battery failures in 2019-2022 XC90 T8 vehicles outside the Recall 25V-179 population. 4.Volvo possesses internal engineering data showing the battery defect affects vehicles outside the population reported to NHTSA in Recall 25V-179. 5.Volvo has not issued a recall for the specific failure mode affecting this vehicle. 6.Expanding Recall 25V-179 to cover the full 2019-2022 XC90 T8 population would cost Volvo tens of millions of dollars. These facts were established against both Volvo entities through separate sets of Requests for Admission that went unanswered within the statutory deadline. The owner requests that NHTSA evaluate whether Recall 25V-179 is adequate to protect the full population of affected vehicles. Supporting documentation, including copies of the deemed admissions and service records, is being submitted by mail to the Office of Defect Investigation. This cost was a hardship for me because I'm still making a car payment. I also had to replace the battery every 1.5 years. I had my oil changed in Dec. 2025. I drove 1000+ miles w/no issue. Engine suddenly seized w/no engine light warning. Oil dumped all at once. Kia refuses to cover under warranty. THIS HAPPENED TO MY CAR: NTSA announced it had launched an investigation into Hyundai Motor Company and Kia Motor Company for problems with their oil pans and oil drain plugs... consumers have filed complaints their
... engine’s oil drain plug falls out, causing all the oil to drain out of their engine while driving. In multiple tests the oil drain plug was tightened according to factory torque specifications and the tightening was well documented. Yet, testing shows in multiple cases that while driving, the oil drain plug falls within a few thousand miles after the service. In those cases, the crankcase drained completely. The Hyundai and Kia drain plug falls out due to inferior oil pan material and the drain plug gasket. The testing and investigations appear to show that the oil drain plans are made with THIN METAL AND THE DRAIN PLUG MOUNTING IS POORLY REINFORCED. In addition, the oil pan is coated with a thick paint coating the bonds the gasket to the pan, so the gasket does not come off when the drain plug is removed. That gasket issue alone is not why the oil drain plug falls out, but it is a factor in the chain of events. The oil pan is the biggest problem Apparently, the thin metal, along with engine vibrations and expansion/contraction tend to loosen the drain plug without any other intervention. NHTSA is investigating this as a factory defect ... The problem is two-fold. First, even if the repair shop properly torques the drain plug, the oil drain plug falls out on its own. The customer then blames the shop for shoddy workmanship. Second, Hyundai and Kia dealers are telling customers that servicing their vehicle at an aftermarket shop automatically voids their factory warranty. This is totally false & actually illegal under the terms of the Magnuson-Moss Warranty Act.Read more
