The vehicle's auto idle stop system is malfunctioning, resulting in the engine failing to restart from a complete stop. This total failure has occurred on five separate occasions, including while stopped in active traffic intersections. Prior to these total restart failures, the vehicle showed the following symptoms: 1) Initially, the vehicle experienced rough engine idling, abnormal vibrations,
... and a raw gasoline odor after driving. 2) This progressed to a noticeable delay between releasing the brake pedal and the engine attempting to ignite, during which the starter sometimes sounded strained. 3) This has culminated in the current defect where the engine shuts off normally when stopped at an intersection, but completely fails to immediately restart when the brake is released. This defect completely immobilizes the vehicle in active traffic lanes, exposing us to an immediate and severe risk of a rear-end collision. When the engine fails to restart, the vehicle locks the transmission out of drive and triggers a warning on the dashboard. It is a highly hazardous situation, especially with a lot of distracted drivers on the roadways.Read more

I took delivery of a brand new Tesla Model Y today in Austin, Texas. Within less than two hours of delivery, the vehicle experienced two separate and serious failures while driving. During the first incident, the vehicle suddenly stopped in the middle of the road without any warning while I was driving. The vehicle became completely unresponsive and came to a stop in active traffic. I immediately
... contacted Tesla service for assistance. The representative advised me not to worry and stated that the vehicle likely came to a safe stop location and that there appeared to be no issue since the system had recovered. I was told the vehicle was safe to continue driving. However, this information did not reflect what actually occurred. The vehicle had stopped in the middle of an active roadway, not in a safe location. After the system temporarily became operable again, I continued driving as instructed. However, during a second drive shortly after, the exact same issue occurred again within approximately 10–15 minutes, this time on a much larger and heavily trafficked road. When the vehicle stopped both times, it became completely electronically controlled and unresponsive. There was no manual way to move or safely reposition the vehicle. I was stranded in the middle of moving traffic on a major road, unable to control or move the car in any way. I felt that my life was in danger due to the inability to regain control or remove the vehicle from traffic. Both incidents occurred on the same day of delivery within a very short time frame, making the vehicle completely unreliable and unsafe to operate. At least one of the incidents occurred while using self-driving features. Due to the repeated failures, the severe safety risk, and the incorrect reassurance provided during the first incident, I no longer feel safe operating this vehicle. I am requesting a buyback or refund rather than repair, as this issue occurred immediately after delivery and represents a critical safety conceRead more

My 2019 Toyota 4Runner has experienced an ongoing steering and suspension issue since early in its life. The vehicle required an alignment at approximately 6,800 miles and has required repeated alignments since. At approximately 48,000 miles, a separate repair facility identified lower control arm bushing concerns. The issue was not properly corrected, and the vehicle continued to require repeate
... d alignment adjustments. At approximately 66,000+ miles, the condition remains unresolved and continues to affect steering stability. The vehicle can pull or require correction to maintain proper lane position during normal driving. There have been multiple repair attempts, but the root cause has not been identified or repaired. This condition may present a safety concern due to reduced steering stability and potential loss of proper vehicle control.Read more

I am experiencing multiple unresolved defects with my 2026 Chevrolet Equinox affecting drivability, braking behavior, and vehicle structure/fit and finish. The primary issue is a persistent vibration, shimmy, or wobble during moderate acceleration from a stop. This condition is felt through the steering wheel, seat, and floor, and occurs consistently during low‑speed launch and normal accelerati
... on. In addition, the vehicle produces a snapping or grunting noise during hard braking, as well as rough and loud movement noises from underneath the vehicle during normal operation, which raises concerns about braking and drivetrain integrity. There are also body and interior fitment defects. The front passenger door does not sit flush with the rear door panel and appears slightly open when closed. The dealership service manager stated that correcting this condition would require removing and pulling the rear door panel outward so both doors align. The glove box door also does not sit flush when closed, and the service manager advised that replacement might make the condition worse. These concerns were reported to an authorized Chevrolet dealer. The dealer confirmed that General Motors has acknowledged the acceleration vibration condition in a Preliminary Information service bulletin (PIT6466) affecting 2025–2026 Equinox vehicles and advised that GM engineering is investigating the issue and that no repair is currently available. The combination of drivability issues, abnormal noises during braking, underbody movement sensations, and body misalignment affects the use, value, and safety perception of the vehicle. The vehicle has very low mileage, and several of these issues have been present since early ownership. I am submitting this complaint to document these conditions and request that NHTSA monitor potential powertrain, braking, suspension, and vehicle structure concerns if a defect trend exists. I purchased a 2020 Ford F-150 XLT with a 5.0L engine from Quirk Auto Group in Maine approximately six months prior to discovering a major engine defect. At the time of sale, I was provided documentation stating there were no known mechanical issues or prior defects with the vehicle. Shortly after purchase, I brought the vehicle back to the dealership due to a noticeable rattling noise and perform
... ance concerns. I was told the noise was caused by debris (rocks) stuck in the undercarriage, and no further action was taken. This concern was documented during that visit. Over time, the vehicle began consuming excessive amounts of oil, approximately three quarts per month which is far beyond normal operating conditions. I later brought the vehicle to a certified Ford dealership for further diagnosis. After inspection, I was informed that the engine had significant internal wear, including worn cylinder walls, and required a full engine replacement (long block), with an estimated cost of approximately $15,785. I have since learned that this engine issue is consistent with a known defect documented in Ford Technical Service Bulletins (TSBs) as well as an open class actions lawsuit against ford, and records indicate this issue had been present with the previous owner. This information was not disclosed to me at the time of sale. I have made multiple attempts to resolve this issue directly with both the selling dealership and Ford Motor Company. Ford has offered partial financial assistance toward the repair, which further indicates acknowledgment of a defect. However, the dealership has not taken responsibility for selling a vehicle with a pre-existing condition that was either known or should have been known.Read more

I am filing a complaint regarding a 2021 Volkswagen ID.4 involving loss of propulsion and unresolved defects following manufacturer-directed repairs. On February 19, 2026, the vehicle lost propulsion twice, including in an intersection. The vehicle powered on and shifted into gear but would not move, requiring it to be pushed out of traffic. This is a serious safety hazard. The vehicle was repaire
... d at a Volkswagen dealer under corporate technical direction, including replacement of the rear drive motor, replacement of control modules (gateway), and multiple programming attempts, including non-standard programming. After repair, new faults appeared. The vehicle now shows inconsistent odometer behavior, including “-- mi” and conflicting mileage values. The system continues to calculate range but does not reliably display total mileage. The dealer confirmed in writing that this issue is related to the prior repair, meaning the vehicle was returned defective. Despite prior loss-of-propulsion events and current system faults, both the dealer and Volkswagen corporate representative have advised me to continue driving the vehicle and have refused to arrange transport, even though the defect resulted from their repair. I have serious concerns the vehicle may again lose propulsion and that control systems and mileage recording are unreliable. The vehicle has been out of service over 60 days with no confirmed resolution. I request NHTSA investigate these failures and Volkswagen’s handling of this safety issue.Read more

2024 Kia EV6 leased January 2025. Vehicle is subject to NHTSA Recall 24V-867 / Kia campaign SC327 for the Integrated Charging Control Unit (ICCU). The recall remedy was performed at an authorized Kia dealer in July 2025 (software / operational logic update). The defect was not cured. Three months after the recall remedy was performed, the vehicle suffered complete loss of electrical power and woul
... d not move. It was towed to the Kia dealer. The dealer's own diagnostic recorded code P1A9096 — the exact diagnostic code Kia's recall instructs technicians to check for — and replaced the ICCU and high-voltage fuse. This is the recall-defined failure occurring AFTER the recall remedy was applied. Approximately three months later, the vehicle again suffered total loss of power and would not start in cold weather. It was towed in. The dealer replaced only the 12-volt battery and did not address the ICCU, despite the documented prior ICCU failure on this VIN. The 12V battery failure is a known downstream symptom of ICCU failure on this platform. Approximately one month after that, the vehicle returned for charging system failure ("vehicle charges briefly then stops") and a "check electrical/hybrid system" warning that appeared while driving. Diagnosed as charging port failure. Vehicle was out of service approximately 27 consecutive days. Total cumulative days out of service across four visits: approximately 35-40. Two safety-related stranding incidents resulted from these failures. Both occurred in winter conditions in Pennsylvania. In the first, the driver was stranded in a parking lot at night during a snowstorm in sub-zero conditions; three tow trucks were dispatched before a flatbed could recover the vehicle because the EV could not be towed conventionally with no power. In the second, the vehicle died in a school parking lot during a snowstorm; the first dispatched tow truck could not recover an EV with no power, and the driver had to walk multiple miles home in freezingRead more

The car then gave warning on the Ford app saying " Your air bag warning is on due to an issue with the front passenger air bag system. The front passenger airbag may not work properly." Another notification says " Your air ab warning is on because the syst
... em has detected a crash." The police and paramedics at the scene where very shocked that the airbags did not deploy. I was taken to the ER by ambulance due to this. Please look into it, I would hate for someone to get hurt or killed due to this issue.Read more

In April 13th, 2026 while driving from the house through community, I suddenly stopped at the cross roads due to unexpected conditions. During this event, a coffee mug placed in the cup holder tipped over and spilled liquid onto the center console area—near the phone charging pad and critical control panel used for gear selection and essential driving functions. In about 15 minutes after the spi
... ll, when I reached the highway at about 70 mph, the vehicle experienced a rapid and severe system-wide failure. The dashboard displayed multiple warnings and errors, including: * “Steering assist reduced – Steering may require increased effort” * “Center console buttons to select gears unavailable – Use touchscreen to select gears – Schedule service” * Repeated alerts indicating braking and system limitations * Conflicting and inaccurate readings, including the speedometer displaying “0 mph” while the vehicle was still moving at highway speed At the same time: * Braking response was severely compromised despite full pedal application * Hazard lights and turn signals were non-functional * Gear selection controls became unavailable or unreliable * Driver assistance and safety systems were disabled * The system issued instructions to “press and hold brake” while braking performance was inconsistent This resulted in a complete loss of situational awareness and control while I was in the car with my older parents and my child. l. I was unable to signal to surrounding vehicles while attempting to maneuver into a narrow emergency lane at highway speed. My family and I had fear for our lives. Critically, the vehicle instructed to exit immediately while we were still in a dangerous roadside position without functioning hazard lights. We were exposed to active highway traffic without any ability to alert others. was eventually able to maneuver the vehicle into the emergency lane under extremely dangerous conditions. The vehicle was towed, and the issue was reported immediately.Read more

The 10-speed automatic transmission in my 2020 Ford F150 slips between gears and has failed to engage from a stop in multiple instances. There are no warning lamps or other indications that anything is wrong with the vehicle. The issue is known as a very common problem with this transmission, as confirmed by multiple mechanics when inquiring about a potential fix.

Date of Accident: April 13, 2026 I drove into our church parking lot and then parked next to my husband’s car and was totally stopped and in the parking space. I thought I had my foot on the brake. After a split second, it accelerated and jumped the curb of the parking space at a very fast speed. I had no control of the car. I sheared off a heavy metal sidewalk railing and was headed for the br
... ick church (church is on a berm above parking lot), but I managed to steer away to miss the church and a large tree. I steered off the berm and into empty parking lot – still traveling at high speed. I finally threw it into neutral to get it to stop because it was continuing toward a busy street with lots of traffic. Neutral stopped it. No air bags deployed. The front and side of the car that hit the metal railing sustained $6,500 damage. Towed it to collision repair. When we asked at the Columbia, MO, Honda dealer’s service department, they said because the air bags didn’t deploy, that it was probably operator’s error and nothing they could do. We didn’t feel that it was operator error and reported to the assistant General Manager of Honda in a personal meeting. We asked to have the Honda field engineer exam the car. Instead, they had their district service supervisor exam the car. Their conclusion: they could find nothing wrong with the car. After reading online reports of almost identical accidents with this model, we decided it was too dangerous to drive and sold it back to the Honda dealer.Read more

I am experiencing the exact issues identified in NHTS Campaign Number: PE25009. The steering system on my 2018 Chrysler Pacifica exhibits a sticking sensation during gradual turns, particularly when the steering wheel is positioned near the 11 o'clock or 1 o'clock position. It is most prevalent at speeds over 60MPH. The safety of everyone in the van has been put at risk by requiring increased stee
... ring force in the opposite direction that could easily be overcompensated for by the driver. The first time it occurred, I overcompensated which easily could have caused a crash into a guardrail or vehicle. Fortunately, it did not and I was aware of the issue and how to react when it continued during the drive. There were no warning lamps, messages, or other indicators that there's a problem with the van. After parking the vehicle, I saw online that it could be possible to reset the steering sensors by turning the wheel all the way in each direction several times. Not sure if it was a coincidence, but I did not experience the same sticking for the remainder of the drive even with reaching the same speed and taking similar, gradual turns. A service center inspection will come next.Read more

The contact owns a 2020 Chevrolet Silverado 1500. The contact stated that while driving 35 MPH, the vehicle shuddered and lost automotive power. The check engine, ABS, and several other unknown warning lights were displayed before an engine misfire. The vehicle regained automotive power before hesitated while depressing the accelerator pedal. Additionally, there was an abnormal humming sound comin
... g from the engine. The vehicle was eventually driven to the dealer where a diagnostic test was performed. The dealer determined there was a lifter failure in cylinder #2, and there was debris in both the engine and transmission, which caused engine and transmission failures. The dealer agreed to a goodwill repair, to cover part of the cost of the repair. The manufacturer was notified of the failure and transferred the contact to the NHTSA to report the failure. The vehicle was not repaired and remained with the dealer. The failure mileage was approximately 70,000.Read more
