I own a 2019 Audi Q8. The vehicle was in the dealership’s possession for approximately three (3) weeks for service. During that time, I was not informed of any significant mechanical concerns or issues. After retrieving the vehicle, it displayed an oil pressure warning while in operation. I immediately returned the vehicle to the dealership for evaluation. At that time, the dealership added oil
... (“topped off” the vehicle) and released it back to me without identifying or addressing the underlying cause of the warning. Following this, I made multiple attempts to have the vehicle properly diagnosed. I experienced difficulty obtaining assistance, including calls being disconnected, and I was not provided meaningful support or a thorough evaluation of the issue. Shortly after leaving the dealership, the vehicle began producing abnormal knocking noises and rapidly became unsafe to operate, requiring it to be towed. This situation presents a significant safety concern, as a sudden loss of power or drivability while driving can increase the risk of an accident, particularly in traffic conditions where maintaining speed is necessary. The condition has since been confirmed by a service provider as a major mechanical failure. Additionally, I have received documentation from the manufacturer indicating a known issue related to piston skirt defects associated with this vehicle. Despite the vehicle being in the dealership’s possession for an extended period, this issue was not identified, diagnosed, or addressed. I was also not provided clear assistance or guidance regarding potential warranty consideration related to this known issue. The vehicle is currently available for inspection upon request.Read more

The 2023 Toyota Tundra Hybrid’s powertrain design may be insufficient to maintain basic safety maneuvers (specifically "limp mode" or emergency clearing) during an engine failure while operating under high-load conditions in mountainous terrain. This creates a life-threatening hazard for the occupants and other motorists. Statement of Fact: As an owner of a 2023 Toyota Tundra Hybrid, I primar
... ily utilize this vehicle to tow a 9,000 lbs travel trailer through the high-altitude, steep-grade regions of Utah. These routes frequently consist of narrow, two-lane mountain passes with no shoulders and steep drop-offs. Failure Under Stress: Engineering logic suggests that engine stress is highest during heavy towing on steep grades. If a primary engine failure occurs in these conditions, the hybrid system’s battery and electric motor capacity are, in my experience, inadequate to move a combined Gross Combined Vehicle Weight (GCVW) of approximately 15,000+ lbs to a safe location. While the hybrid system may allow a light, unladen vehicle to limp to a shoulder under "ideal conditions," it lacks the torque and sustained power to move a heavy trailer on a grade once the internal combustion engine is compromised. In the event of a failure, the vehicle would likely become "dead in the water" in the middle of a live, narrow lane on a blind mountain curve. Without the ability to reach a turnout or even a marginal shoulder due to the weight of the load, the vehicle becomes a stationary target for other high-speed mountain traffic, posing a severe risk of rear-end collisions and multi-car accidents. I am requesting that the NHTSA investigate the performance of the Tundra Hybrid’s "fail-safe" modes specifically under maximum towing capacity on grades.Read more

My CX-5 has 86,120 miles. I noticed a burnt oil smell coming into the car and later found oil droplets on my garage floor. I took it in to my neighborhood garage and they found an oil leak coming from the cylinder head. They recommended I take to my Mazda dealer. I had it towed to Pearson Mazda, Richmond,VA. They inspected it and stated I had a cracked cylinder head. I have a 100K extended warrant
... y that will cover most of the cost to repair it. However, I found the Mazda CX-5 2018-2021 models have a cylinder head defect. I found a Mazda Service Alert, #SA-031/21 and on Reddit a Mazda CX-5 owners page that has many stories of the same problem, cracked cylinder head. Some have had Mazda cover the repair cost or given a major discount and provided an extended warranty after repair. I reviewed this with Pearson Mazda and they stated that my car does not fall in the vehicle numbers that they will cover. This is a known issue and they will not acknowledge it for my car. I have contacted Mazda Corporate and waiting for a callback. Litigation has been filed for the CX-5 Turbo customers and they have received restitution. This is also a major issue on non-Turbo customer’s SUVs. I have read that the 2018-2021 manufacturing (casting) of the cylinder head was defective. However, Mazda is not admitting it because a RECALL would cost them millions to fix the cylinder head issue on all the affected vehicles. But, they are at fault and should work with all customers that have been affected by this issue. Therefore, I am requesting NHTSA to make this a public safety matter and have Mazda be accountable for the issue. Thank you!Read more

I own a 2022 GMC Sierra 1500 AT4. The vehicle developed a clear engine knocking noise. I first brought the vehicle to the dealership while under 150,000 miles for inspection related to a recall. The dealership only performed a computer scan and oil change and stated no issue was found. No mechanical inspection was performed despite the noise. The issue continued and worsened. I returned to the de
... alership and the vehicle was kept for three days. During that time, the dealership again denied the existence of the noise and stated the vehicle was operating normally. When I picked up the vehicle, I immediately noticed the same noise and requested the technician to inspect it again. At that time, the technician acknowledged the noise and confirmed it was related to the recall. Despite this acknowledgment, General Motors declined to repair the engine and closed the case, offering only partial assistance toward purchasing a new vehicle instead of fixing the defect. The failure to properly diagnose the issue during the initial visit and the subsequent denial during the second inspection allowed a safety-related mechanical defect to worsen. This condition may result in engine failure or sudden loss of power while driving, creating a serious safety risk. The engine noise indicates a potential internal failure which could lead to sudden loss of power while driving, creating a safety risk. The initial failure to properly diagnose the issue may have allowed a safety-related defect to worsen beyond repair. I am extremely concerned about continued operation of the vehicle while this documented condition remains unresolved. In addition to this safety-related axle issue, this vehicle has a broader history of serious mechanical problems. On May 13, 2022, after a recent service visit at DCH Brunswick Toyota, I reported that when I pressed the gas the vehicle was making an unusual noise like an old car, that the car delayed to speed when I pressed the gas, that I had to press the gas all the way in order for the car to move faster, and that it was not like this prior to service. At approximately 25,000 miles, I also asked that the brakes be checked because when we drove the vehicle and used the brakes, the car made a startling sound. In 2023, I also reported hard/heavy steering and jerking while driving. I reported transmission-related abnormal noises and drivability concerns while the vehicle was still within Toyota’s powertrain warranty period, but the condition was not properly diagnosed at that time. The transmission has since been confirmed by authorized Toyota dealerships to require full replacement. The vehicle also has a documented engine oil leak, leaking shock absorber, and prior recall history involving a stalling condition. I've filed this complaint with NHTSA due to the axle-related safety risk. Toyota is now on written notice of that condition. My vehicle has experience repeated electrical system failures resulting in no-start condition and a loss of reliability. The first incident was in Nov 2025. The vehicle would not start after being parked at work from 0630-1530. The batteries had been drained due to what the dealer identified as a radio frequency module issue. The technician explained it to having gotten "stuck" in update mode. Th
... e vehicle was under warranty at the time and required a replacement of the module and two new batteries. I asked what I should look for in case this happens again in hopes of getting it to the dealer before it drains the batteries and I was told there are no real indicators. Early in April, the auto started not working consistently and the FOB would not open the doors. I replaced the battery in the FOB but the issue continued. Since that time I have monitored the voltage gauge. The vehicle was taken to the dealer again mid-April. The tech said he could not get the auto start issue to duplicate, stated there were two software updates that they updated and that it should be good to go. Monday, April 27, 2026 I parked the truck after work with the voltage reading 14.3. Tuesday, April 28, when I attempted to get in the truck to go to work at 0615, the failed to start or respond to the key FOB in any way. Once again, the truck was towed to the dealer where it is currently. The tech has stated they are having to replace the batteries again. This pattern indicates an electrical system failure due to battery drain and repeated no-start conditions. The failures occur without warning and have resulted in the vehicle being inoperable on more than one occasion. This repeated issue presents a safety concern if the vehicle can become stranded unexpectedly due to this failure. The issue does not appear to be resolved. The tech informed me that the company is still trying to figure it out.Read more

I have not received any response or confirmation of receipt. I want to correct my earlier characterization — after further research, I understand that 20B39 is a Customer Satisfaction Program, not a safety recall. That said, the core issue and my position remain the same, and I want to lay out where things stand clearly. The situation: - My vehicle is covered under CSP 20B39 (Front View and CHMSL Camera — PCB conductivity issue causing intermittent camera operation) - The program is in effect through November 30, 2030 or 150,000 miles, and the repair is to be performed at no charge to the vehicle owner - My servicing dealer, Santa Margarita Ford, attempted to complete the CSP work but could not because the vehicle's Gateway Module is not communicating properly - The dealer has advised that the Gateway Module must be replaced before the camera work under XXX can be completed My position: Ford issued CSP XXX and committed to completing this repair at no charge. If the Gateway Module must be functional in order for Ford's own program to be executed, then resolving the Gateway Module issue is Ford's prerequisite to meet — not an out-of-pocket cost to push to the customer. The brake pedal was depressed, and the vehicle stopped. The vehicle was diagnosed, and the contact was informed that the wheel speed sensor needed to be replaced. The vehicle was repaired. On another occasion, while attempting to park the vehicle with the parking brake activated, the parking brake failed to engage, and the vehicle rolled forward. The vehicle was taken to a dealer, where the failure could not be duplicated; however, the EBS module was replaced. The vehicle was repaired, but the failure recurred intermittently. The vehicle was taken to another dealer; however, the failure was not duplicated. 1) What component or system failed? The issue appears to involve the vehicle’s electrical system and hybrid system, as well as brake-related electronic systems. The vehicle displayed multiple warnings including “Hybrid System Malfunction” and “Critical Vehicle Malfunction.” The electronic parking brake system was also affected during the incident, including a situation where the brake co
... uld not be released while the vehicle was in motion. The affected components are available for inspection upon request. 2) How was safety put at risk? Safety was put at risk because the malfunction occurred while driving and involved critical system warnings and brake-related issues, including the inability to release the electronic parking brake. The sudden appearance of a “Critical Vehicle Malfunction” warning during operation created concern about potential loss of vehicle control and safe braking capability in traffic. 3) Has the problem been reproduced or confirmed by a dealer? The issue has not been fully resolved. The vehicle previously underwent service for a high-voltage hybrid battery replacement, but the current symptoms continue to occur repeatedly. The problem has not yet been confirmed as permanently resolved by the dealer. 4) Has the vehicle been inspected by manufacturer, police, insurance, etc.? I was driving my daughter to work then all of a sudden 6 different vehicle alerts came on my dashboard. I pulled over to a doctor's office parking lot. The car would not go into reverse. Once again, I had to turn it off and on. I had to eventually put it into neutral and cruise all the way home. Once home the last alert came on which was the check engine light. I had the car towed to my nearest Toyota dealership. Toyota kept my car for over 24 hours. I had to go back up there to inquire about my car. That was when I was told it was my transmission and they quoted me $9,338.96 ($2,479.87 labor for 12.4 hours, $ 6,808.92 part, $50 for shop supplies) to get my car repaired.Read more

I've had my car for 3 years now and it randomly decided to throw a SRS airbag malfunction warning that will not go away. I know most of the last gen es350 had airbag recalls but the 2019 model year really didn't and I think its something similar. The car runs fine but the fact that my airbags wont deploy because of this warning makes it very unsafe.

While driving in traffic, my 2023 Toyota Mirai suffered a sudden and total loss of motive power. The vehicle became completely unresponsive while in 'Drive', presenting a severe safety hazard. The dashboard displayed multiple cascading warnings, including 'FCV System Malfunction', 'Pre-Collision System Malfunction', and 'PKSB Malfunction'. The vehicle had to be towed to Dublin Toyota (Repair Order
... #653103). The dealership retrieved critical diagnostic code P1DC500 (FC AIR PRESSURE TOO HIGH) and verified the vehicle would not go into 'Ready' mode. The dealer opened Toyota TAS Case #TA26079027. However, the dealership performed no mechanical repairs to address the root cause of the sudden power loss. They simply cleared the safety code, test drove it, and explicitly wrote on the final invoice: 'ADVICE CUSTOMER TO DRIVE VEHICLE UNTIL PROBLEM OCCUR'. Furthermore, they refused to document my stated concerns that the vehicle is unsafe to drive. Returning a vehicle to a customer with an unresolved, life-threatening defect that causes total loss of power at speed is incredibly dangerous.Read more
