The vehicle was hit in the front end while my girlfriend was driving it. Someone ran a red light and smacked into the front end of the van at approximately 50 miles per hour. The airbags did not deploy despite significant force. The car that ran the red light was small but the force was significant enough that she hit her head and sustained medically verified extreme whiplash.

On May 3, 2026, while operating my 2024 BMW i5 on public roads, the vehicle suffered a sudden and complete loss of power steering with no prior warning. I could not turn the steering wheel at a stop. The instrument cluster simultaneously displayed: "Vehicle may roll. Brake manually," "Drive carefully. Changed steering effort," "Drive carefully. Chassis stabilization," and "Parking assistance malfu
... nction." Auto Hold and the electronic parking brake's hold function were disabled, requiring manual brake application to prevent rollaway. Mileage at incident: approximately 21,344 miles. The simultaneous failure of Electric Power Steering, Dynamic Stability Control, and the Electronic Parking Brake while the vehicle was in motion in traffic created an immediate safety hazard. No crash or injury occurred, but a recurrence at highway speed, on a wet road, or during an evasive maneuver could foreseeably cause loss of vehicle control, crash, injury, or death. The vehicle was taken to an authorized BMW dealer. The technician stated this failure is a known issue following a recent vehicle software update, that BMW has no repair available, and that the only action taken was to power-cycle the vehicle and clear stored fault codes. No software remedy, parts replacement, or TSB was provided. I was given no assurance the failure will not recur and no timeline for a fix. To my knowledge, no recall has been issued for this defect on the i5 platform. I request that NHTSA evaluate the scope of this issue across the i5 fleet and determine whether a defect investigation or recall is warranted.Read more

Vehicle Information: Year/Make/Model: 2014 Hyundai Sonata Mileage: less than 120,000 miles Ownership: Second owner Complaint: I am filing a complaint regarding a serious engine failure in my 2014 Hyundai Sonata. My vehicle experienced issues beginning with a faulty knock sensor, which escalated into complete engine failure. This issue appears to be identical to problems that have already been i
... dentified in recalls involving similar Hyundai Sonata models. Despite experiencing the same symptoms and failure pattern, my specific VIN is not included in the recall. My vehicle has been well maintained, has under 120,000 miles, and should not have experienced a total engine failure under normal conditions. Based on the known recall issues related to engine defects and knock sensor failures, I believe my vehicle is affected by the same underlying defect. I am concerned that vehicles like mine are being excluded from the recall despite having identical safety-related issues. Engine failure while driving presents a serious safety risk, including potential loss of power, stalling, or inability to accelerate in traffic. I have had the car looked by a professional and advised of the engine failure and this is a common issue with this make and model.Read more

We bought our car in April, 2024 from Wellesley Volkswagen, [XXX] . The car has a problem with the windshield wiper Control (safety standard 101?). The car’s windshield wiper stalk is on the right side of the steering wheel. Moving the stalk up or down activates the front wipers. Pulling the stalk will activate the front wiper washer. Pushing the stalk away will activate the rear wiper and wash
... er. Moving the stalk up from the off position will provide 1 wipe. Moving the stalk down from the neutral position will first activate rain sensing/interval wiping then slow continuous wiping and finally fast continuous wiping. There is also slider button on the stalk to vary the sensitivity of the rain sensing/ interval wiping mode. The first wiping position can be configured to operate at either interval wiping or rain sensing through the car’s customization setting (screen interface). We have the wipers set for rain sensing. The rain sensing is triggered by the stalk being in the first position and rain being “seen” by the sensor (camera) at the top center of the windshield. The slider adjusts the sensitivity so that it activates occasionally or aggressively depending on the rain level and the driver’s preference. From the time we bought the car to present day, we find the rain sensing will not work about 25% of the time: the wipers will not turn on when in the sensing position and it is raining. On one recent occasion during a light rain, the wipers went on full speed in the sensing position, and I had to move the stalk to the 2nd position (continuous slow wiping). There was nothing blocking the sensor which would cause this behavior. After a couple minutes I move it back to the sensing position and the sensing performed normally. I brought it to the dealer’s attention a few times over the past 2 years, but made them officially write it up and check on how to fix this at the April 13, 2026, 2nd year service with no resolution. "What component malfunctioned...?" - Electrical system (suspected) "How was safety put at risk?" - We were unaware if the dashboard instruments were reading correctly. We also couldn't lock/unlock the doors or open/close the windows. The tailgate would also not open. "Has problem been reproduced?" - Not yet - plan to have the service center inspect. "Has vehicle been inspected by the manufac
... turer?" - No "Were there warning messages or symptoms?" - Yes, when the issue occurred. The vehicle reported 13 warnings/alerts. Happened for the first time on 5/2/2026 GENERAL DESCRIPTION: While the vehicle is parked or idling after sitting for a period of time, the dashboard suddenly displays approximately 13 warning alerts simultaneously, including a message stating "Check Emergency Stopping function" among others. During this condition, multiple vehicle systems become inoperative, including: - Power windows - could not open/close - Door locks - could not lock/unlock - Tailgate operation - could not open (it was closed) The vehicle appears to experience a system-wide electronic failure affecting both body controls and safety systems. The condition is intermittent but has occurred multiple times. Restarting the vehicle does NOT immediately resolve the issue. After driving for a period of time, the vehicle eventually returns to normal operation. This is a safety concern because: - Critical safety systems (automatic emergency braking / driver assist features) are (potentially) disabled - Basic vehicle functions (locks/windows) are unavailable. This is a concern if the door can't be opened and one needed to exit through the window in an emergency. - The failure occurs without warning and cannot be reliably reproduced on demand This appears to be a broader electrical or communication system failure rather than a single component issue.Read more

Persistent unresolved front-suspension defect on a 2022 Polestar 2 (Polestar Approved CPO, purchased 03/20/2025). Vehicle pulls right under normal driving on flat, level pavement — a directional control hazard requiring constant steering correction at highway speed. A front-suspension noise over bumps developed after recent repairs. The Polestar-authorized dealer (University Volvo / Polestar Ch
... arlotte) made FIVE repair attempts on this same complaint without resolving it: 10/23/2025; 11/04/2025; 04/01/2026; 04/15/2026; and 04/17–04/27/2026. Five alignments performed. Both upper strut bearings replaced 04/15/2026 (Inv #XXX , $1,499.44). On the next visit the dealer replaced a control arm and did additional shock and bearing work as no-charge line items (Inv #263452, $279.19) — implicitly acknowledging the prior repair was inadequate. Within one block of leaving the dealer on 04/15, vehicle developed a new front-right rattle that did not exist before. Dealer's own technician notes on Inv #XXX state verbatim: "test drove vehicle and could not verify any pulling while driving" — repair performed WITHOUT confirmed symptom reproduction. As of 05/04/2026 (six days after pickup), vehicle is again pulling right and now has a NEW noise over bumps. Condition is materially worse than before any repair. This is the subject of Polestar Technical Journal TJ XXX (NHTSA document MC-XXX-0001), which acknowledges creaking/knocking from the upper front suspension on Polestar 2 MY 2021–2024 and states a redesigned upper bearing was released Q4 2023, with a further redesign in week 20 of 2024. Polestar has acknowledged in writing this component was defective and redesigned TWICE. Polestar Customer Engagement has refused to escalate to a contact with authority to remedy. Incident date: May 2, 2026
I own a 2016 Model S P100D equipped with the Large Drive Unit (LDU). The component that fails is the rotor coolant seal in the rear drive unit motor. Coolant leaks internally into the motor and inverter, causing bearing wear, corrosion, isolation faults, and eventual complete drive unit failure. The affected drive units are still installed and available for inspection upon request. This defect put
... s my safety and the safety of others at risk because it can cause sudden loss of propulsion (especially at highway speeds), power reduction, or total drivetrain shutdown, potentially leading to a crash. Coolant in high-voltage areas also raises electrical fault/fire risks. The problem is widely reproduced and confirmed: Tesla service centers are familiar with it and replace failed units with updated “-U” revisions that include a coolant bypass. Independent EV shops confirm the exact root cause (rotor seal failure) via encoder inspections showing coolant intrusion and through disassembly. This is a documented pattern failure in all 2012–2020 LDU vehicles. No police or insurance inspections have occurred yet, as no crash or total failure has happened. Tesla is aware of the recurring issue through service records on similar vehicles. Early symptoms before full failure typically include rear motor whining/grinding (especially under load), reduced power warnings, “Rear Drive Unit Fault” messages, and visible coolant on the speed sensor. These can appear gradually. My vehicles have not yet experienced total failure, but the defect is known to be inevitable. I am proactively monitoring encoder sensors. Because this is a systemic design defect affecting thousands of vehicles with clear safety implications, I request NHTSA open an investigation and require Tesla to issue a recall for free repairs/replacements on all affected 2012–2020 LDU vehicles.Read more

The dashboard cover has come unglued from the part of near the front windshield. It first bubbled up then became unglued and rolled up. There’s white material underneath that appears to have swollen up like wet cork board. Also, this part of the dashboard is no longer flush like the parts to right and left of the distressed area. Additionally, the exposed white part reflects in my line of sight
... causing limited visibility which is a safety concern to me. Based on my research, this issue is being reported by owners of the 2017 and 2018 Ford Platinum Explorer models particularly. This issue is not something that I can repair as a DIY and since others are reporting the same exact issue with the same models, I tend to believe there’s a defect.Read more

It is unknown exactly what causes the issue. But intermittently when taking off from a stop, the powertrain hesitates to respond to my input on the throttle. The hesitation lasts anywhere from half a second to a full second. This puts my safety and other driver’s safety at risk because I cannot reliably tell whether I will be able to pull out quickly and safely in front of an oncoming vehicle. T
