multiple times this has been an issue. I stepped on the break and the car essentially leaps forward typically when going up down or straight. In November 2024 it happened while going up a hill slowly and I bumped into the vehicle ahead of me. My vehicle had no damage, but the car in front of me had to get a new skid plate and my insurance doubled because of it. Since the incident in November 2024,
... my car has unintentionally accelerated intermittently. finally, April 2025 this happened my car started shaking jolted forward, engine revving, and then my check engine light came on then immediately contacted my local dealership and scheduled an appointment to get my car in. They have had my vehicle since then and have said it is not safe to give it back to me so I am currently driving a loaner vehicle.Read more

Incident date: Apr 1, 2024
Catastrophic engine failure; unless it could happen very quickly, the vehicle likely will not be available for inspection. As background, our family purchased this vehicle used from a MI dealer in July 2023. It was a trade-in by the previous owner, and the dealer did not disclose any known issues. This vehicle belongs to our daughter, who is a graduate student in upstate NY. After driving only ~6,
... 000 miles, her safety was put at risk when she lost acceleration as gray smoke poured out of her tailpipe (had she been on a highway, both she and other drivers would have been at grave risk). She had the vehicle towed to a local service station and was told the engine needed to be replaced. After searching Google for problems with this vehicle's engine, a 1.5L Ecoboost, we discovered that not only is this a known issue (coolant intrusion) for which Ford did not issue a recall (instead, "Program #21N12 one-time repair for vehicles included in 19B37"), but there also is a class action lawsuit being litigated by Capstone Law APC. Given this, we towed the vehicle to the local Ford dealer, who also advised we need to replace the engine, we will receive no financial relief from Ford, and it will cost $12,000 for the replacement (we paid $16,000 for this vehicle less than two years ago). We have spoken to various individuals at the national Ford customer satisfaction organization, and they will not help us. So we now have an inoperable vehicle that my daughter can't afford to repair, and since we are not getting any financial relief from Ford, she is forced to buy a much older, high-mileage vehicle that may also put her safety at risk. The only indication of a problem was when the check engine light came on, but since the vehicle was driving fine, our daughter did not realize she should take it for service right way (this is her first vehicle, and she lives close to campus so drives only infrequently). She is unsure how long the light was on, but likely several weeks.Read more

I am submitting this report to formally document a severe safety-related failure involving my 2013 Volvo S60, VIN # [XXX] . This vehicle is part of the Volvo brand, widely recognized and marketed as among the safest in the world. Incident Summary: On [XXX], at approximately [XXX] in Norwalk, Connecticut, the vehicle experienced a catastrophic mechanical failure while in operation. The auxiliary
... (serpentine) belt suddenly failed, shredded itself, and wrapped around the timing pulley. Debris from the failed belt entered the timing belt housing — a supposedly sealed and protected compartment — causing the timing belt to jump. This resulted in an immediate loss of engine power while driving, placing me in a highly vulnerable position. Fortunately, I was not on a multi-lane highway or in fast-moving traffic. Had I been traveling on a busier route, the sudden loss of power could have led to a serious crash and potential injury. This failure represented a genuine threat to my safety. Further inspection revealed that: The lower crankshaft sprocket had skipped three teeth due to the intrusion of the auxiliary belt. The timing belt, although not broken, was off track. A new timing belt was installed in an attempt to restart the engine. The engine would crank but failed to start. A compression test revealed zero compression in all cylinders. The valve train was severely damaged (bent valves), and the engine sustained internal mechanical failure beyond repair. Conclusion and Context: This is not an isolated incident. Upon further research, I discovered that this failure — specifically the auxiliary belt disintegrating and compromising the timing belt system — is a known and well-documented problem in vehicles equipped with the B5254T12 Turbo engine. Volvo has been previously alerted to similar cases before. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I was driving home in exceptionally heavy rain. Before getting home, I decided to stop at a coffee drive thru. It was still raining pretty heavily as I drove up to the pick up window. I had my foot on the brake as I waited for my order. A few minutes went by when suddenly the car shook a bit and completely shut off. A warning sign came up on the dash screen that read “engine stopped, steering po
... wer low” and instructed to put the car in park. That warning message would come up every time I tried to put the gearshift back in drive. I kept trying to restart the car many times, the engine cracked but no start. At one point I even tried putting the car in neutral to attempt to push it out of drive thru lane but the car and steering wheel were locked in place. There was no way to move either. It’s still raining at this point but it has significantly lightened up. About 15 - 20 minutes later, the car started up like normal and I was able to put the gearshift in drive. The car is less than a year old and nothing like this has ever happened before. There were absolutely no warning signs prior to this happening, everything on the dashboard and the mobile app associated with the car showed the car was in “good health.” I am SO incredibly lucky this happened at a drive thru and not in the middle of a highway or busy intersection. You are left completely helpless since the engine completely shuts off and the steering wheel locks. If this happened on a highway, it would immediately create a multi vehicle accident or potentially worse. This is a HUGE safety risk that needs to be addressed immediately. I plan to take the vehicle into the dealership for inspection because I do not feel 100% safe anymore.Read more

On February 25, 2025, I took my 2023 Toyota Tundra VIN# [XXX] to Stokes Toyota Beaufort, SC, for its 45,000-mile service. Just 300 meters after leaving the dealership, my truck caught fire, causing significant damage and putting my safety at risk. I am sharing pictures of the incident and the warning messages I received on the dashboard. I was on my way back to the dealership when this happened.
... At first, the truck was shaking and losing power. I managed to make a U-turn to return to the dealership, but then the engine stalled. At that moment, I started seeing warning messages on the dashboard. I immediately called the dealership to inform them that I was experiencing issues with my truck. While still on the phone with the dealership, I noticed smoke coming from the passenger side of the hood. Moments later, flames started emerging from the hood. I quickly hung up and called 911. 1- What component or system failed or malfunctioned? I believe the engine and possibly the brakes failed (based on the dashboard warning messages). 2- How was your safety or the safety of others put at risk? Within minutes, my truck burned completely. I am grateful I didn't have my children with me at the time. 3- Has the problem been reproduced or confirmed by a dealer or independent service center? I received formal letters from Toyota Manufacturer, Toyota Distributor, and the dealership, stating that they are sorry for what happened but cannot offer any assistance with my claim. 4- Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? Yes, the insurance company, fire marshal, dealership, Toyota, and Engineering Analysis Associates (EAA) have all inspected the vehicle. 5- Were there any warning lamps, messages, or other symptoms of the problem prior to the failure? Yes, the truck was shaking, losing power, and eventually stopped completely, displaying warning messages on the dashboard. I had the fuel pump recall (NHTSA ID 24V-113) performed on my 2019 Honda CBR600RR at a certified dealership. After the repair, I discovered multiple serious safety issues that made the motorcycle significantly more dangerous than it was before the recall: The fuel tank vent line was crushed under the tank during reassembly, completely blocking the vent system. This caused a vacuum lock that m
... ade the fuel cap impossible to open after riding, creating a risk of fuel starvation, fuel pump failure, or even tank deformation. The fuel level float arm was improperly installed and became jammed against an internal pipe inside the tank. The result is a fuel gauge stuck at “Full,” giving no indication of actual fuel level and preventing safe monitoring or refueling. When I contacted the dealership, they told me to ride the bike 30 miles back to them — despite the tank being sealed and the fuel level unknown. I also contacted Honda Powersports, who stated they have no responsibility and that only the dealer can address the issue, despite this being a federally mandated safety recall. I have not yet corrected the problem myself due to the serious safety implications and the need for proper accountability. The technician errors during this recall service introduced a more immediate and severe crash risk than the original defect the recall was intended to fix. Attached are photos showing the fuel level sensor float arm hung up on a hard pipe inside the tank, and the repair order from the dealership which performed the repair.Read more
