While driving at a moderate speed (june 7, 2025), multiple critical systems of the vehicle suddenly failed. Both dashboard screens (driver and center display) went black, and numerous warning lights appeared afterward, including oil pressure, engine, lane departure, tire pressure, and airbag (SRS) system malfunction. The vehicle accelerated involuntarily, the brakes did not respond, the steering b

The police report attributes the crash to a vehicle system malfunction. Two days before the incident (June 5, 2025), the vehicle had been serviced by the dealer, and the electronic screen issue had been reported but not addressed. This failure put our lives at serious risk and demonstrates a catastrophic malfunction across safety-critical systems. Despite reporting this safety issue immediately, manufacturer Genesis Motor America failed to disclose that this was a known defect covered under Technical Service Bulletin 24-BD-008G. Customer service representatives did not mention existing TSB and refused emergency service appointments, forcing 2-month wait for service scheduling despite life-threatening electrical malfunctions. Customer had to independently discover TSB documentation through research. Only after customer escalated with TSB evidence did Genesis authorize repair. Dealer confirmed drainage system failure and completed repairs per TSB guidelines that manufacturer should have immediately referenced. Genesis's failure to disclose known safety defect and refusal of emergency service created extended exposure to dangerous electrical malfunctions that could result in accidents.Read more

On Thursday, [XXX], I took my vehicle to the authorized dealership Alberic Chrysler Jeep Dodge, located on Kennedy Avenue in San Juan, Puerto Rico, for routine maintenance. While handling the vehicle, the service personnel lifted it using the shop's lift equipment, raising it all the way to the ceiling, which caused severe structural damage to the vehicle's roof. The dealership contacted the Puert
... o Rico State Police, who filed report number [XXX]. Additionally, I was asked to complete a written damage claim at the dealership. The vehicle was under full manufacturer's warranty at the time of the incident. However, the structural damage has now compromised the vehicle's integrity, specifically: - The rear doors became misaligned. - The roof suffered a visible crack, which exposed the interior of the vehicle to weather conditions. - The damage potentially affects warranty coverage, especially related to corrosion protection. We requested that the dealership retain the vehicle on-site to allow for inspection by the manufacturer's technical team. The dealership refused to do so unless I agreed to pay a $25 daily storage fee. We also requested substitute transportation, which they denied. Additionally, according to the NHTSA database, this vehicle currently has an open safety recall. I was never informed of this recall by the dealership at the time of the maintenance appointment, which is a serious failure in the dealership's responsibility to notify the owner and address potential safety risks. The vehicle is used for both personal and professional transportation, and at the time of the incident, it had 8,402 miles. This incident raises significant safety concerns regarding the vehicle's structural integrity, the open recall, and the dealership's handling of the matter. I respectfully request NHTSA's assistance in investigating this situation to ensure that both the safety standards and consumer protections are upheld. My 2023 Toyota RAV4 Hybrid has had squeaky brakes for many months (from 4,000 miles onward and consistently continues to be an annoyance, nuisance, and safety hazard while intensifying in frequency), but despite taking the vehicle to the Toyota dealer more than 4 times, they have been unable to repair the issue. At least twice the air bag sensors have come on for the backseat when there was nobody
... seated in the backseat, distracting me from safely driving. Toyota has done nothing to right the wrong mechanically, refund me for the vehicle (a lease) or offer a replacement. I have a 14 page document filed with the Florida Attorney General Lemon Law Division thoroughly indicating why these ongoing mechanical failures, safety hazards, nuisances and distractions to me the driver substantially impair the use, value, and safety of the vehicle—fulfilling key criteria under Florida Statutes § 681.102 and § 681.104, which I am happy to provide all consumers and attorneys willing to launch a CLASS ACTION against Toyota. Florida law defines "impairment of value" not by market collapse, but by any reasonable reduction in consumer confidence. Consumer disclosure laws require honesty about defects. Florida Statute § 681.102(15) defines a “nonconformity” as a defect that “substantially impairs the use, value, or safety of the motor vehicle.” My vehicle has been subject to repair more than 4 times for the same defect within 24 months and thus qualifies for relief. The manufacturer’s failure to permanently correct the squeaky brakes—even after attempted repairs—demonstrates a pattern of unresolved nonconformity. The squeaky brake issue is not a cosmetic inconvenience—it affects the vehicle's functionality, resale integrity, and safety assurance. Toyota acknowledges the problem exists (via TSB) but has failed to provide a lasting solution. If unresolved, this defect would materially concern any reasonable consumer and diminish confidence in the vehicle’s reliability and safety.Read more

However, the frameless windows did not automatically lower as they normally would when opening and closing the doors, As a result, when we attempted to close the doors, both front door windows shattered, as the glass was unable to lower and collided with the door frame. This incident clearly resulted from a critical system failure that disabled essential safety and design functions. I am requesting the following: A full investigation into the root cause of the power system failure Immediate replacement of both shattered front windows at no cost A written explanation of what caused the vehicle to become unresponsive This issue raises serious concerns about vehicle safety and reliability, especially when essential functions like glass auto-drop are disabled without any warning, leaving the glass vulnerable to breakage.
